NCAPHA is recruiting a Community Systems Change Consultant to be placed at the Madison County Health Department. Community Systems Change Consultant (Innovative Approaches Coordinator) Job Description A. Primary Purpose of Organizational Unit: The Madison County Health Department is a service and regulatory agency within Madison County Government. The mission : The Madison County Health Department is dedicated to the purpose of disease prevention and health promotion for all Madison County residents. The mission is accomplished by means of provision of various direct preventive health services, provision of selected diagnostic and therapeutic services, and implementation of various regulatory activities. B. Primary Purpose of Position: This position is part of a four county pilot project called Innovative Approaches and is funded through the NC Division of Public Health, Children and Youth Branch in cooperation with the Madison County Health Department. This is a consultative position to improve the community-wide systems of care to best meet the needs of families of children and youth with special health care needs* (CYSHCN). The consultant staffs and maintains a steering committee of community agencies and parents to assess system issues affecting families of CYSHCN, facilitates the development of an action plan driven by parent and provider data, facilitates the completion of action items, and assists with all evaluation efforts. The system change outcomes are: Families of children and youth with special health care needs will partner in decision making at all levels, and will be satisfied with the services they receive. All children and youth with special health care needs will receive coordinated ongoing comprehensive care within a medical home. All children will be screened early and continuously for special health care needs. Services for children and youth with special health care needs and their families will be organized in ways that families can use them easily. All children and youth with special health care needs will receive the services necessary to make appropriate transitions. C. Primary Purpose of Innovative Approaches Initiative:
The purpose of the Innovative Approaches initiative is threefold: (1) to foster and support community strategies to create effective systems of care for families of CSHCN; (2) to identify promising practices, and then; (3) to help disseminate these practices to other communities. D. Work Schedule: Monday through Friday, 8:00 am to 5:00 pm, or flexed to best meet needs of the Division to equal 40 hours per week. Additional hours will be required in the event of an early morning or evening meeting in connection with the Innovative Approaches Initiative. E. Description Of Responsibilities And Duties: 1. Coordinates the development and maintenance of a steering committee to oversee the planning and implementation of this initiative. 2. Assures family involvement in identifying systems issues and addressing systems changes. 3. Helps provide focus groups, community meetings and other outreach activities to better understand parents concerns regarding care for their children with special health care needs. 4. Works with the Children and Youth Branch to coordinate state and local trainings, consultation and networking opportunities. 5. Helps the steering committee develop a logic model and a parent driven action plan to improve community systems of care for families of CYSHCN. 6. Serves as a liaison to the Madison County Health Director, CCNC Network Director, and family representative who co-chair the Innovative Approaches Steering Committee to help coordinate the Steering Committee meetings to assure outcomes are achieved. 7. Collaborates with agencies that serve children with special health care needs to engage them in this process. 8. Organizes and supervises programs for volunteers for the benefit of this initiative to identify needs, gaps and opportunities for improvements. 9. Conducts special studies to determine the needs of children and families and the type of community organizations which could be utilized in meeting these needs. 10. Participates in informational and educational programs in order to bring the needs to the attention of the community.
11. Coordinates data sharing with the Children and Youth Branch to allow for enhanced understanding and communication of work being done by this initiative. 12. Prepares quarterly activity reports and special reports as needed or requested. 13. Performs related work as required. 1. Supervision Received by Employee: Work is reviewed on an on-going basis with regular meetings and yearly performance evaluation by the Madison County Health Department, the NC Children and Youth Branch, and the Innovative Approaches Steering Committee. Performs work with considerable independence and autonomy. The Steering committee will have direct input into the position performance evaluation. The Steering committee will meet monthly. The employee independently plans for their daily work. Decisions will impact children with special health care needs and their families. 1. Physical Effort: Moderate physical strength and stamina is needed for this position, including walking, standing, stooping, and prolonged sitting. Transporting, moving supplies and equipment, rearranging furniture occurs periodically. 1. Work Environment and Conditions: Work is carried out at in the community. Office space is located at the Health Department or within a community based organization if needed. When conducting business outside the building, the employee may encounter inclement weather while driving. 1. Knowledge, Skills and Abilities: 1. Strong understanding of the systems change process; 2. Strong organizational leadership and planning skills; 3. Demonstrated ability to facilitate meetings, workshops, and discussions; bringing diverse viewpoints together toward a common goal; 4. Strong negotiation skills, and the ability to establish and maintain effective relationships with a wide variety of individuals and organizations; 5. Ability to work with a variety of health care professionals in the hospital, academic and community setting; 6. Superior verbal and written communication skills;
7. Understand the principles of quality improvement in the health care setting; 8. Understand the importance of evidence-based guidelines in quality improvement; 9. Experience with computer operations and programs including Microsoft Word, Excel, Access, Adobe Acrobat, Publisher, and Outlook; 10. Considerable knowledge of community resources and services available to children with special health care needs. 11. Ability to multi-task in a busy environment, self motivated; Considerable knowledge of methods and techniques in disseminating information to the public and facilities and media available. 12. Ability to organize and direct the activities of local committees for the development and promotion of community educational programs. 13. Ability to establish and maintain favorable working relationships with the community groups, pediatricians offices, CCNC Network, youth serving agencies, public and mental health personnel and the general public. 14.
