Since the founding of Lodi Funeral Home in 1932 by Samuel D. and Mildred Salas, it has always been our policy to provide full and complete information about funeral arrangements. It has also been our policy to assist the families we serve as expressed in our motto: Thoughtful Service Within the Means of All This motto has remained our utmost concern and goal, and our dedicated staff continues to this day to serve every family as our very own. Today, when individuals sometimes require a variety of funeral service alternatives, the families we serve discover the Lodi Funeral Home itemized funeral service charges to be informative and helpful when considering service details. Greater satisfaction will come to those who carefully consider the value of all the choices offered to fulfill their personal needs. The following price quotations are provided at this time to assist you during these considerations. Upon request, we will be pleased to discuss your inquiries in detail. FINANCIAL TERMS POLICY 1. PAYMENT POLICY: Our policy requires payment in full prior to services rendered. You may pay by cash, check, credit card (Visa, MasterCard, American Express and Discover Card) or insurance. 2. INSURANCE: Lodi Funeral Home will honor all valid, in force insurance coverage as an instrument of payment, applied to the funeral expenses. Proceeds in the amount due must be assigned to the Lodi Funeral Home. Paid receipt will not be given until benefit is paid to the funeral home. If Lodi Funeral Home accepts an insurance policy assignment as payment, our policy requires that an assignment processing fee of 4% or 100.00 (whichever is greater) be added to the contract total. DISCLOSURE OF PRENEED FUNERAL ARRANGEMENT Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that been signed and paid for, in full or in part, by or on behalf of the decedent.

GENERAL PRICE LIST Prices are effective as of July 28, 2015 and are subject to change without notice: The goods and services shown below are those we at Lodi Funeral Home are able to provide. You may choose only those items desired. However, any funeral arrangements selected will include a charge for our basic services and overhead. If for legal or other reasons you are required to purchase items or services you did not specifically request, we will explain the reason in writing on the statement we provide describing the funeral goods and services. The total cost for the following traditional type funeral services may include but may not be limited to items listed. No casket or container is included in any service costs listed on the following pages except where noted. TRADITIONAL FUNERAL SERVICE WITH GRAVESIDE CONCLUSION SERVICE or GRAVESIDE SERVICE FOLLOWED BY A MEMORIAL SERVICE $3,635.00 This traditional service includes the basic administrative fee and the costs for removal, embalming, other cosmetic preparation, visitation, the use of the Funeral Home Chapel and staff for a funeral or memorial service or the setup, staff and equipment for funeral or memorial services at a church or other facility, the staff for a graveside service, the use of the hearse, the use of the flower van, a standard register book, acknowledgment cards, memorial folders and recorded music at our facility.* # TRADITIONAL FUNERAL SERVICES WITH CHAPEL OR CHURCH CONCLUSION $3,305.00 This traditional service includes the basic administrative fee and the costs for removal, embalming, other cosmetic preparation, visitation, the use of the Funeral Home Chapel and staff for a funeral or memorial service or the setup, staff and equipment for funeral services at a church or other facility, the use of a delivery vehicle for final disposition, the use of the flower van, a standard register book, acknowledgement cards, memorial folders and recorded music at our facility.* # This traditional service may be followed by burial, entombment, or cremation. TRADITIONAL GRAVESIDE SERVICE $3,140.00 This traditional service includes the basic administrative fee and the costs for removal, embalming, other cosmetic preparation, visitation, the staff for a graveside service, the use of the hearse, the use of a flower van, a standard register book, acknowledgment cards, and memorial folders.* # CREMATION WITH MEMORIAL SERVICE $1,795.00 This service includes the basic administrative fee and the costs for removal, cremation fee, the use of the Funeral Home Chapel and staff for a funeral or memorial service or the setup, staff and equipment for funeral services at a church or other facility, a standard register book, acknowledgment cards, and memorial folders. * # * The use of limousines for any of the above services may be added for an additional $500.00 each. # Sales tax will be added for all retail merchandise including register book and memorial folders.

BASIC SERVICE FEE OF FUNERAL DIRECTOR AND ADMINISTRATIVE STAFF $1395.00 This is a basic fee we charge every family for our professional expertise in assisting them with their preferences. This fee includes a proportionate share of the taxes, licenses, utilities, and business expenses required to serve the public. Our charges include, but are not limited to, consultation with the family, administrative and clerical services, preparation of the death certificate and necessary permits, filing the death certificate with the appropriate government agencies, consultation with clergy, cemetery, crematory or common carrier, assistance with veterans, social security, union, and/or insurance papers, planning the funeral or memorial services, and placement of obituary notices. THIS FEE FOR OUR BASIC SERVICES AND OVERHEAD WILL BE ADDED TO THE TOTAL COST OF THE FUNERAL ARRANGEMENTS YOU SELECT. (This fee is included in our charges for immediate burials and direct cremations.) TRANSFER OF REMAINS TO FUNERAL HOME (15 mile radius, excess mileage $2.00 per mile)_ *255.00 *Second Attendant when deemed necessary by the Funeral Home add 95.00 PROFESSIONAL EMBALMING 365.00 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. OTHER PREPARATION OF THE BODY 195.00 May include, but not limited to, washing and disinfecting, special care for autopsied remains, restoration when necessary, hairdressing, dressing, and cosmetics. VIEWING OF UNEMBALMED REMAINS 150.00 No public viewing or open casket funeral service will be permitted of unembalmed remains. A brief viewing by immediate family primarily for the purposes of identification, if requested and Lodi Funeral Home agrees, of unembalmed remains will be permitted by appointment only. USE OF FACILITIES AND STAFF FOR VIEWING AND/OR VISITATION (One day) 225.00 Each additional day $112.50. Embalming, airtray, airport transfer or other services are an additional fee RECEIVING REMAINS FROM ANOTHER FUNERAL HOME, base 1,650.00 Includes airport transfer from Sacramento to Lodi Funeral Home and our basic service fee TRANSFER OF REMAINS TO OR FROM AIRPORT To or from: Sacramento airport 255.00 San Francisco airport 450.00 THERMAL REDUCTION UNIT FOR REMAINS, first 48 hours 365.00 Thereafter, per day 182.50 IMMEDIATE BURIALS RANGE IN PRICE FROM $1,660.00 TO $2,255.00 IMMEDIATE BURIAL WITH CASKET PROVIDED BY PURCHASER 1,660.00 IMMEDIATE BURIAL WITH CLOTH COVERED CASKET 2,255.00 (includes casket price of $595.00, sales tax will be added to the casket cost) IMMEDIATE BURIAL WITH OTHER CASKET SELECTED AT FUNERAL HOME 1,660.00 (the cost of the casket and sales tax will be added to this fee) DIRECT CREMATION ARRANGEMENTS WITH CREMATION FEE RANGE IN PRICE FROM $1,120.00 TO $1,215.00 additional fees do apply for rush service DIRECT CREMATION ARRANGEMENTS WITH CASKET PROVIDED BY PURCHASER 1,120.00 DIRECT CREMATION ARRANGEMENTS WITH CARDBOARD CONTAINER 1,215.00 (includes container price of $95.00, sales tax will be added to container cost) DIRECT CREMATION ARRANGEMENTS WITH OTHER CASKET SELECTED AT FUNERAL HOME 1,120.00 (the cost of the cremation container and sales tax will be added to this fee) MEMORIAL DVD TRIBUTE (INCLUDES 1 COPY) 250.00 ADMINISTRATIVE FEE FOR FORWARD OF REMAINS OUTSIDE THE UNITED STATES 550.00 This fee covers the costs for translating the necessary legal documents for shipping as well as the costs for driving to file and obtaining permission on the necessary documents at Consulates in Sacramento or San Francisco. SHIPPING OF CREMATED REMAINS WITHIN THE UNITED STATES 195.00 This fee covers the cost of packaging the urn for shipping in accordance with packaging rules stated by the United States Postal Service (USPS) for sending packages Priority Mail and the USPS costs for postage and Priority Mail fees. This fee includes, but may not be limited to, basic services of Funeral Director and Staff. This fee is included in Direct Burial and Direct Cremation fees which encompasses two days of thermal reduction. Arrangement fees do not include cemetery charges.

Honorariums for clergy, churches, music providers, lodges, and other organizations. 2. Additional transportation (such as limousines and out-of-town transportation). 3. Newspaper notices. 4. State and county health department fees. 5. Cemetery, burial, entombment, cremation and interment costs. 6. Clothing. 7. Flowers. The prices for any additional items and/or services will be shown on your bill or the statement describing the funeral goods and services selected. The survivor of the deceased who is handling the funeral arrangements, or the responsible party, is entitled to receive, prior to the drafting of any contract, a copy of any agreement that has been signed and paid for, in full or in part, by or on behalf of the deceased, and that is in the possession of the funeral establishment.
