The goods and services on the following pages are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services. If legal or other requirements mean you must buy any items you did not specifically ask for, we would explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

Professional Services BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $1,850 Our fee for the basic services of funeral director and staff includes, but is not limited to: Responding to the initial request for service. Arrangement conference. Preparation of biographical and statistical data. Examination of financial settlement options. Preparation and filing of papers, permits and authorizations. Coordination with those providing other services. (Church, Cemetery, etc.) Preparation and placement of obituary notices. Retention and care of remains prior to commencement of visitations and/or funeral service. Coordination and supervision of funeral. Also covers overhead, including facility maintenance, equipment, insurance and governmental compliance. This fee for our basic services is added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) EMBALMING (Including use of preparation room and sanitary care) $995 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. (NO HUMAN REMAINS MAY REMAIN UNBURIED OR UNCREMATED IN NEW JERSEY FOR MORE THAN 48 HOURS UNLESS EMBALMED OR REFRIGERATED. N.J.A.C.8: 9-1.1) If you choose to have embalming performed, the decedent may be embalmed at a location other than this registered mortuary. OTHER PREPARATION OF THE BODY (Including use of preparation room but excluding embalming) 1. Dressing, casketing ($300) and cosmetology ($150) $450 2. Sanitary care (without embalming) $425 3. Restoration and other procedures (per hour) $200 4. Hairdresser $90 USE OF FACILITIES, STAFF AND EQUIPMENT FOR VIEWING/VISITATION This includes setup of the visitation area, placement of remains, positioning of floral arrangements, and supervision and attendance during the hours of visitation. 1. One Day $900 2. One Visitation (2 hours) $450 3. Visitation in facilities other than the funeral home $400 4. Private Family Only (No other Services - 1hr) $250 USE OF FACILITIES, STAFF AND EQUIPMENT FOR FUNERAL CEREMONY Our services include coordinating the arrangements for the ceremony, staff supervision, attendance, and the use of the funeral home when used for the ceremony. 1. At funeral home - includes 1 hr. visitation prior to service $450 2. At funeral home/to church-includes 1hr visitation prior to church $450 3. At church $350 2

USE OF FACILITIES, STAFF AND EQUIPMENT FOR MEMORIAL SERVICE (without body present) Our services include coordinating the arrangements for the memorial service, staff, supervision and attendance, and the use of the funeral home when used for the memorial service. 1. Additional miles will be charges at a rate of $3.00 per mile. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected. Optional Packaged Services RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME $1,950 This package charge includes the basic transportation of the remains to the funeral establishment (within a 15-mile radius) and/or place of local disposition, staff services, and securing necessary authorizations. The charges do not include additional items of services, merchandise, facility use, and livery that the customer may wish to select. IMMEDIATE BURIAL $2,180 Our package charges for immediate burial include basic services of funeral director and staff, the local transfer (within a 15-mile radius) of remains to the funeral establishment, securing necessary authorizations and local transportation to the cemetery. This charge does not include the use of the facilities and staff for any visitation or ceremony, public or private, prior to burial, nor does it include the actual cost of the grave at the cemetery, or the cemetery s charges for opening and closing of the grave, any required outer burial container or cash disbursements, or anything not expressly stated as being provided. 1. Immediate burial with container provided by purchaser $2,180 2. Immediate burial with casket selected from our funeral home (In addition to cost of Casket) $2,180 & UP FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME $2,400 This package charge includes removal of remains to this funeral firm (within a 15-mile radius), basic services of funeral director and staff, embalming, and securing necessary authorizations. It does not include a casket, alternative, or shipping container, the use of facilities and staff for any visitation or ceremony, public or private, before forwarding the remains or any sheltering for days on which no post death activities occurs. Nor does it include transferring the remains to the airport or other destination, or the charges of a common carrier, or anything not expressly stated as being provided. (For other options, please refer to the selections available on this General Price List) 4

DIRECT CREMATION Our package charges for direct cremation include some of the basic services of funeral director and staff, the local transfer (within a 10-mile radius) of remains to the funeral establishment, the obtaining of necessary permits and authorizations, and transportation to the crematory. This charge does not include the crematory charge, nor the use of facilities and staff for any visitation or ceremony, public or private, before cremation. Nor does it include cash disbursements or anything not expressly stated as being provided. $1,500 & UP If you want to arrange a direct cremation, you can use an alternative container. Alternative Containers encase the body and can be made of materials like fiberboard or composite material (with or without an outside covering0. The containers we provide are alternative containers. and other cremation containers, including containers made of wood, containers made of fiberboard and containers made of other composite materials. We also offer a selection of cremation oriented caskets. (Please see Cremation Catalog at funeral home) 1. Direct Cremation with a cremation container supplied by the purchaser $1,500 plus (All such containers must meet any applicable State or Cemetery Requirements) 2. Direct Cremation with casket selected from our funeral home (In addition to cost of casket) $1,500 plus 3.
