The access drive north of the student lot is in poor condition with significant cracking and pavement failure. There are 2 entrances around the main high school building that do not have ADA compliant access. Large portions of sidewalks around the site have heaved, subsided, and/or cracked preventing ADA access to doors and various facilities. The side-slopes of the drainage channel along the southeast property line are too steep to maintain. Erosion and vegetation control is an issue along the channel. A drainage sump and pump to north of the softball field is clogged and erosion has exposed piping. The bottom of the grade beam along a portion of the southeast building face is exposed due to erosion. BUILDING CONDITION: EXTERIOR Lakeview Centennial consists of a brick structure with concrete accents. There are very few windows at this facility. The windows are single pane glass, and do not provide optimal energy efficiency as compared to a double pane window system. The brick walls are in need of repair, especially at the control joints. Sealant has deteriorated and is cracking or missing at most of the joints. These weak spots create possible areas of water infiltration to the wall systems. Several portions of brick walls around the structure indicate signs of moisture damage, as seen by discoloration of brick units and stains on the surface. Handrails at this facility do not currently meet code compliance. Rails are either missing, or do not have the correct dimensions or extensions, as required by code. Brick wall indicates possible water infiltration, as evident by the brick colors. Cracked pavement at side exit. Steps leading to doors do not have handrails and concrete has pulled away from walkway surface, leaving a gap at the bottom stair. Moisture stains are visible, running down the wall from the soffit. Stair is missing handrails at both sides; walkway has shifted, as seen by uneven treads at bottom step. Joints have areas of missing sealant. Material should be removed and replaced with new sealant. Brick walls at service area have damage to masonry units. A sizable gap exists between brick wall and face of concrete. Classroom doors have door knobs, which do not met code compliance. Additionally, classroom doors are not equipped with lock down capability. Metal doors are mostly in poor condition, with damage to the leaf and/or hardware. Exterior metal doors have full glass panels, which have sealant that has deteriorated and has been painted over repeatedly. Sealant should be removed and replaced with new material. BUILDING CONDITION: INTERIOR (INTERIOR FINISHES) Typical corridor finishes include terrazzo floors with brick walls and portions of ceramic tile at walls. Ceramic tile has broken and cracked pieces, particularly at the corners and edges. The majority of walls in this facility have vinyl wall covering that has been painted over. In most cases, the wall covering is peeling up at corners and at the base. Classrooms have VCT floors, which show heavy wear, especially at the science labs on the second level. BUILDING CONDITION: INTERIOR (INTERIOR FINISHES) Terrazzo flooring at the entrance lobby is stained from a previous fixture that has been removed. Ceramic wall tile at main corridor has cracked and broken tiles, especially at the base and corners. This area has been patched with tiles that are a different color. Terrazzo treads at stairways have chips near the nosings. Typical classroom finishes; VCT flooring and vinyl wall covering at walls. Wall coverings do not comply with district standards. Typical ceilings have 2x2 ceiling tiles. VCT flooring, particularly at the science labs, is in poor condition. Partitions have damage and/or vandalism, and some partitions have been painted over and patched together with other materials. BUILDING CONDITION: FINE ARTS Lakeview Centennial has a Band Hall and Choir Hall both rooms have carpeted floors with gyp board walls and regular lay-in ceiling tiles. Per district standards, VCT flooring should be used at these spaces. The instrument storage at the Band Hall consists of built-in plastic laminate shelves. Laminate has peeled off at the faces, revealing the material underneath. Some practice rooms have wall carpet, which is separating at the seams and in poor condition. BUILDING CONDITION: ATHLETICS Both the competition gym and the P.E. gym have hardwood floors, which are covered with several layers of a polyeurethane coating. Despite this coating, the floor have uneven surfaces and knicks. The athletic equipment, such as basketball goals and wall pads, are in satisfactory condition. Ceilings have acoustic panels inset in between the structural members, and the panels have stains from water damage. Doors to the gym abut a sloped area of floor, which is greater than the allowable slope per code. BUILDING CONDITION: ATHLETICS Hardwood flooring at competition gym has an uneven surface across the boards. Floor has been painted and has many layers of polyurethane applied. View of basketball goals, which are in good condition. Ceiling has acoustic panels, which have water stains. Fixed metal bleachers at competition gym are in good condition. Gym floor ramps down to meet the doors. Slope seems quite steep. BUILDING CODE AND TAS Typical door hardware at classrooms consists of door knobs, which do not meet code requirements. Additionally, several doors have thresholds which exceed the maimum height allowed between two surfaces. Handrails and guardrails are not compliant, and do not meet the required dimensions as outlined by code. Sinks do not provide knee clearance for occupants in a wheelchair. The Band Hall and Choir Hall currently do not have an accessible route. Both rooms have stairs at the entry by the doors. The elevator that serves these two rooms does not meet code compliance. BUILDING CODE AND TAS Water coolers do not meet the code requirement for knee space dimensions. Choir Hall and Band Hall both have stairs at entry of room. An elevator that services both spaces is not code compliant; thus these spaces are inaccessible. Door threshold from gym is too steep; thresholds should not be greater than ¼ height difference between two surfaces. Sink at faculty workroom/lounge is not code compliant and does not provide required knee clearances. Typical condition at second level stairwells; guardrails are not compliant. ENGINEERING REPORT - MECHANICAL / ELECTRICAL / PLUMBING Mechanical Systems Information The mechanical system at Lakeview Centennial High School (LCHS) is a two-pipe, chilled water hot water system that is comprised of numerous air handling units (AHUs), variable air volume boxes (VAV), and a central plant consisting of two water cooled chillers, two hot water boilers, and six associated pumps. The system also consists of several DX Gas rooftop units (RTUs) that serve various areas of the facility. Lakeview Centennial High School was constructed in 975. The classroom spaces located within this original section of the building are currently being served by a total of eight Trane VAV air handling units. These units are located in rooftop mechanical penthouses and were installed during the building s construction. They are in poor condition and have potentially reached the end of their useful life. The VAV AHUs serve the building s classroom spaces by providing conditioned air to numerous VAV boxes throughout the facility. These boxes are located above ceiling and regulate the flow of conditioned air into the spaces below. There are a total of 75 boxes on site at LCHS, all of which have been equipped with electric heat. They were manufactured by Tempmaster in 975 and are in poor condition. The gymnasiums, dressing rooms, cafeteria, admin office and resource center areas are currently being served by CHW/HW Trane constant volume air handling units. Rather than serve various spaces via VAV boxes, constant volume AHUs are dedicated to serving a single, particular space. The constant volume AHUs at LHCS are original to the building and are in poor condition. They have potentially reached the end of their useful life and may need to be replaced in the near future. Rooms 300 and 302 are served by two Trane CHW/HW fan coil units. These units were installed in 976 and are in poor condition. The facility s two water cooled chillers are 225 ton Trane units that are located in Mechanical Room 090. These units were installed in 2003 and are in good condition. Located just outside Mechanical Room 090 are two 999 cooling towers. These units are in good condition and should have several years of useful life left. Primary Chilled Water Pumps and 2 are Bell and Gossett pumps that are 3 Hp and 7.5 Hp units respectively. Secondary CHW/HW Pumps and 2 are 30 Hp Bell and Gossett units, and Condenser Water Pumps and 2 are 25 HP Bell and Gossett units. All pumps were installed in 2003 and are in good condition. The plant s two hot water boilers are Kewanee units that are original to the building. They are in poor condition and have reached the end of their useful life. The Technology Lab is currently being served by a 2.5 ton Carrier rooftop unit that was installed in 997. This RTU in poor condition and has reached the end of its useful life. The media studio and football dressing rooms are served by a total of seven Aaon DX GAS RTUs. These units were installed in 200 and are in moderate to poor condition. The Welding and Automotive Technology shops are served by three Lennox DX Gas RTUs that were installed in 2008. These units are in good condition and should have several years of useful life left. ENGINEERING REPORT - MECHANICAL / ELECTRICAL / PLUMBING Electrical Systems Information The electrical system at Lakeview Centennial High School has undergone several renovations since the building first opened in 976. Electrical distribution equipment typically has a 20 to 30 year useful life after which, retrofitting, refurbishing or replacement is necessary to maintain a reliable electrical system. There is a single 480/277 service entry for the facility, with the main switchgear being located in Mech-0. It is a 3000 amp Federal Pacific MSB and is in poor condition. There are a total of fifty-five Federal Pacific panels and transformers located throughout the building. This equipment is outdated and obsolete. It is in poor condition and is at the end of its useful life. The remainder of the electrical system is comprised of Challenger, Eaton and General Electric distribution equipment. These panels and transformers are in good to moderate condition and are working satisfactorily at this time. Plumbing Systems Information A majority of the plumbing fixtures and piping are original to the building. This being the case, there are many ADA compliancy issues throughout. Maintenance has reported a lack of isolation valves on site and that there have been issues with the sewer line. It has also been noted that the Art and Homemaking Room areas are in need of upgrading. Flush valves on toilets and urinals are a combination of Sloan and Zurn manufacturers. Approximately 25% of the flush valves are low-consumption units. A majority of the lavatory faucets throughout the facility are metered. The building s electric water coolers are a combination of Oasis, Elkay and Halsey-Taylor units, approximately half of which are ADA compliant. There are three water heaters on site at LCHS. All three are 00 gallon Lochinvar EWN models that were installed in 202. ENGINEERING REPORT - MECHANICAL / ELECTRICAL / PLUMBING Lighting Information Lighting was renovated in 2003. All ballasts are electronic and the standard fluorescent bulb size is T8 throughout the building. Gym lighting fixtures are T5HO high bay fluorescents. Exterior lighting is sufficient. Lighting is in good condition throughout the facility. Fire Alarm Information The fire alarm panel is manufactured by Harrington. The model of the panel is 3030. The panel is addressable and was installed in 2005. The panel was reporting trouble in zone. Fire Sprinkler System Information There is no fire sprinkler system currently in place at Lakeview Centennial High School. In order for this facility to be up to code, such a system should be installed. Energy Management System Information The controls on the CHW/HW system were renovated in 2003. Pneumatic valves and dampers on all AHUs, FCUs and VAV boxes remained in place and operational. These valves and dampers are controlled by DDC transducers. If this equipment is scheduled to be replaced in the near future, it may be beneficial to also replace controls at that time. ENGINEERING REPORT - ROOFING General Conditions The building features stucco wall panels and brick veneer exterior. All roof areas are covered with an asphalt gravel surfaced Built-Up roof system. Area A was replaced in 99. Areas C, D, E and F were re-roofed in 993, Areas B2, E2 and G in 998. The foyer and auditorium roof areas were replaced in 2007. Roof areas terminate into gravel guard edge conditions or walls. Drainage is provided by interior drains on most areas. Scuppers and downspouts were found on penthouse roofs in Areas D-F. The structural deck is lightweight concrete in most areas except in the gyms where tectum was found. Drainage/Slope Drainage on Areas A and G would be rated as satisfactory. Drainage on other sections would be graded as fair overall since there are locations around drains that retain some water as evidenced by discolored gravel. There is a large area on the North side of the auditorium roof that ponds water that could not be upgraded by adding drains or tapered insulation because it is directly above the stage and adjacent to fire hatches. Leaves and debris have accumulated around the drain next to the planetarium rise wall. Roof Membrane The roof membrane for the older sections is in satisfactory condition. The roof membrane covering the gym would be graded as good and the auditorium and foyer are in very good condition. Flashings Flashings at the base of walls and curbs in Areas A-C,D E and F are in fair condition. There appears to be water trapped behind base flashing along the rise wall in Area C4. Flashings at curbs and the base of walls in the more recent re-roof locations are in good condition. Surfacing Coverage of pea gravel surfacing is eroding along the edge to varying degrees depending on the age of the roof sections. Wind scour was observed at several areas of the older roof sections. Sheet Metal Gravel guards, counter flashings, copings, gutters and downspouts, for the older sections are in fair condition. Counter flashing and gravel guards on sections A2, A3, B and G are in good condition. A small corner piece of counter flashing is loose at a South side corner condition on Area F. ENGINEERING REPORT - STRUCTURAL The recommendations below are based on our Structural Team s review of the scans of existing documents available in Garland ISD Archives, interviews with Garland ISD Staff from the School Facilities Department and the Maintenance Department, and onsite visual surveys of representative areas that were accessible. As with any structural condition assessment, more detailed assessments can always be performed at an additional cost with more time and may lead to additional recommendations; recommendations for specific types of more detailed assessments are provided below. Specific data regarding structural descriptions and observations of current conditions is provided in the Engineering Report - Structural Appendix separately for each construction phase of each facility. Therefore, refer to the Engineering Report Structural Appendix for more detailed information on where the following recommendations apply. Relevelling or replacement of all or some of the existing foundation and grade beams/piers is recommended to address sloping floors throughout the school due to significant foundation movement. Installing soil retainers at grade beams and re-grading isolated areas along the building perimeter is recommended as to keep additional soil, debris, and water from going into the crawlspace beneath the building. We recommend chipping out the remainder of the topping slab at a door entry in the back of school and replacing with a new topping slab. If any modification to the Structurally Suspended Slab areas is desired, Special Analysis will be required and significant Architectural and Structural limitations may be required. Further Study is recommended to determine if the unreinforced masonry will have any net axial tension or excessive flexural tension under code required design wind forces, which is not permitted by the code. Retrofitting new structural components may be recommended. Further Study is recommended to determine the design and condition of the connectors between wythes. Installing new connectors may be recommended after study. Further Study is recommended to determine if there is sufficient lateral anchorage at the top of all exterior unreinforced masonry that are parallel to roof joists. Installing lateral restraints may be recommended after further study. If any modification to the Unreinforced Masonry is desired, Special Analysis will be required and significant Architectural and Structural limitations may be required. Installing new bottom chord net uplift horizontal bridging angles along the ends of the steel joist is recommended when an opportunity presents itself such as a building remodel. SECURITY Summary: This security assessment was performed on October 08, 203 at Lakeview Centennial High School in Garland, Texas. There was a brief interview with the campus assistant principal Dwight Lofton, however no campus personnel accompanied us with the walk through. General Statistics: The areas immediately surrounding the campus are densely packed single family residences, and open fields to the east. The roads bordering the campus are Hayman Drive, and Rowlett Road. Staff parking is provided on the southeast side of the campus and right in front of the main entrance. Parking spaces for administration are reserved and individually marked for each position. Entry into the campus is clearly marked with school zone signage, reduced speed signs, and clearly marked crosswalk striping at intersections surrounding the campus. There are two drives into and out of the campus, both of which are off of the first off Hayman Drive. There is a four foot perimeter fence around the athletic fields, which had gates that were secured. There were bike racks to the east of Door #3 which is not observable by staff. Preparedness and Life Safety: Campus Evacuations, Lockdown Procedures, and Drills o Students, faculty, and staff all practice the ISD s mandatory standard for fire drills, emergency lock down, and shelter in place for inclement weather. o It was indicated that the school practiced Reverse Evacuation drills for students outside of the building or interior area of the school. o Not all classrooms displayed the plans for emergency procedures of fire drill evacuation plans at the entry to the classroom. o In the event of a lock down, none of the exterior windows were marked to identify the classroom number for first responders. Severe Weather o The campus has a functioning weather radio system which includes exterior mounted devices used for monitoring the weather. However, there were not enough exterior PA speakers on the school building for the announcements to be heard. Automated External Defibrillator AED o A map depicting the location of AEDs should be placed at all exterior doors, gymnasium, cafeterias, and entrance into the school. o Not all faculty and staff are trained on the AED device. First Aid Kits and Safety Equipment o There was a first aid kit in the clinic, but not in the cafeteria or gymnasium. o The stage production area inside of Room # 00C had power equipment all round and no safety equipment was observed. o Inside of Room # 2233 was an enclosed prep area in the back of classroom that did not have any safety equipment noticed in the prep area or in the classroom. SECURITY o o o In Room #00G is storage for multiple fire extinguishers. In Welding Room # 323 was a number of canisters collected along the wall, opposite the office side of the shop. The cat walk above the stage and hand rails that were bent, along with the metal plates below. Outer Perimeter: There is a four foot high chain link fence around all of the athletic fields on the north end of the campus. Building Exterior: There were signs of graffiti and damage to school property at: o In the northeast corner of the Automotive Technology Room #325 is spray paint on the wall. o At the bottom of Stairs #2 is graffiti written on the brick leading toward Rm. #9. o There is graffiti on the exterior wall of Room #34. o There was a swastika just outside of Door #8. The portable buildings had damage to the following items: o There were electrical boxes that had unlocked on/off switches. There aren t any lights under the canopies which make it difficult for teachers and other staff to use this as a viable exit. The exterior lighting appears to be insufficient on the property including the play areas and canopies leading to the portables. Future investment in additional lighting may be required to support any future surveillance camera improvements. Doors and Vestibules: The main entry does not have a managed vestibule. Doorway #6 is a pair of solid metal doors and do not offer natural surveillance from the inside of the building. The exterior door out of Welding Shop #323 was found unlocked. In Room #32 and #305 are local alarms that are independent to these rooms. Just inside of Stair #9A is a communications rack that s not inside of a cabinet. Door # was found to be open and unlocked. Not all exterior doors had the number identifiers above the door. There was a metal enclosure that was open with welding materials, on the south exterior wall of the welding shop. Door #23 was propped open to allow students free access in and out of the building. In front of Classroom #22 is an open ceiling tile and junction box with wires hanging out of it. 66 Facility Assessment Report, Garland Independent School District

SECURITY Visitor Management: The main entry does utilize an existing intercom and surveillance camera system which is located on the exterior side of the entry. Visitors are required to press the intercom button near the front door to request entry. As visitors are identified, they are permitted to enter the office only, which is to the right as you enter the main entry. The administrative staff indicated that the LED display on the Aiphone was too dark and difficult to see the face of the person seeking entry. The campus currently utilizes a Raptor based system for checking in visitors. Electronic Safety Security / Communications: Surveillance The campus has an existing surveillance system that monitors both the interior and exterior of the building. The majority of cameras on campus are in the interior and are located at major entrances, all exit doors, and high traffic areas. o There appears to be insufficient camera coverage around the general exterior of the building including all play areas, pick up and drop off points and delivery entrances. o Citing an issue with difficulty recognizing faces on camera, the administrative staff requested that either the distance between cameras be reduced so that there are more or that resolution of the camera be increased. o Interior camera dome looking down Corridor #3 outside of Stairwell #3 had a hole in the dome, but could not tell if it affected the camera view or not. o There is multiple areas where corridors split off the main hall and created blind spots for surveillance views. o Most stairwells had a blind spot at the bottom Intrusion There is an existing intrusion alarm system used on campus. Motion detection is used as the primary means of detection which is located at exits, computer labs, cafeteria, gymnasium, and main administrative area. o Not all IDF rooms had motion detection within the rooms. Access Control The existing access control system on campus consists of card readers at strategically located doors; main entry and choice keyed exterior doors. The access control system is not used to monitor the condition of the exterior doors during the day. Intercom The campus has an intercom system throughout the school as well as several speakers installed around the exterior of the campus. Two Way Radios There were a limited number of handheld radios located on campus. o Radios were present in the main office, gymnasium, cafeteria and several select classrooms. o Not all personnel carried handheld radio at all times. o When talking to administrative staff, it was indicated that there weren t enough radios for teachers and key personnel. SECURITY Recommendations:. To increase visibility and visitor awareness, it s recommended that: a. The solid barriers between the main office and main entrance should be modified so as to provide a better line of site of the entryway for the administrative staff. b. Due to the fact that existing Aiphone door stations are installed per ADA required height it is difficult to see persons of average height. Use of other cameras located nearby is recommended. c. There is an existing exterior camera at the main entrance that has a side view of visitors requesting entry into the school. If access to this camera can be given to the administrative staff, they can use it in addition to the Aiphone, to view the main entrance and visitors requesting entrance. 2. Additional PA speakers need to be added to the exterior of the building. 3. To increase the ability for administrative staff and/or SRO to accurately identify a person of interest in a video, the resolution or quantity of interior and exterior cameras should be increased. 4. To help first responders locate themselves in the school during a lock down, the school district should standardize the identification of: a. Classroom Windows: A single exterior classroom window should be marked with either the classroom number or a unique identifier that can be given to a first responder. b. Exterior Doors: Every exterior door should be numbered on both the interior and exterior side of the door. 5. Secure all roof hatches by adding keyed locks and access control. Motion detectors should be added inside the room where roof hatch doors are located. 6. Prominently display the plans for emergency procedures inside each classroom. 7. To prevent the unwanted shut off of power to the school, secure all electrical utilities around the building with a padlock. 8. Remove all graffiti around the exterior and interior of the building, and portables. 9. All gates serving the school property should be secured during school hours. 0. First aid kits should be kept in the cafeteria, shop classes, and gymnasium.. Train all faculty and staff on how to use an AED device. 2. A map depicting the location of AEDs should be placed at all exterior doors, gymnasium, cafeterias, and entrance into the school. 3. Reverse evacuation drills should be performed on campus during recess to ensure students and staff can quickly re-enter the building when an outside threat occurs. The school district should oversee all exercises with local first responders. 4. All damaged camera domes should be fixed and or cleaned. 5. Need replace the existing catwalk above the stage in auditorium. 6. The IDF equipment inside of Stairwell # 9A needs to be placed in a locking cabinet so that equipment is protected from accidental damages.
