Mortuary ARRANGEMENTS FOR FUNERAL SERVICES, INTERMENT & SHIPMENT GENERAL PRICE LIST EFFECTIVE February 1, 2011 (PRICE AND FEES SUBJECT TO CHANGE WITHOUT NOTICE) TO THOSE WE ARE PRIVILEGED TO SERVE Federal and State agencies have determined that itemization of funeral services is essential for the benefit of the general public, to assure that consumers are not asked to pay for services they do not receive. It is essential that each item of service, in addition to our basic fee for services, be identified. In all categories of service, a basic fee will be charged plus a fee for each requested or required item of service. Other options are also available. It is our sincere concern that every requirement you may have be fulfilled, in addition or apart from our standard offerings as listed here in. Please feel free to express yourself to our counselors. We will make every effort to meet your needs. The survivor of the deceased who is handling the funeral arrangements, or the responsible party, is entitled to receive, prior to drafting of any contract, a copy of any pre need agreement that has been signed and paid for, in full or in part, by or on behalf of the deceased, and that is in the possession of the funeral establishment. If cremated remains container cannot accommodate all cremated remains of the deceased, the crematory shall provide a larger cremated remains container at no additional cost, or place the excess in a second container that cannot easily come apart from the first, pursuant to section 8345 of the Health and Safety Code. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reasons in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you, such as cemetery or crematory services, flowers and newspaper notices. The prices for these items will be shown on your bill or the statement describing the funeral goods and services you selected. We charge you for our services in buying all these items except where prohibited by state law. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF AND OVERHEAD: This is a basic fee that we charge every family for our professional expertise in assisting them with their preferences. It includes a proportionate share of the taxes, licenses, utilities and business expenses required to serve the public. Our charge includes, but is not limited to, consultation with the family, administrative and clerical services, preparation of the death certificate of the attending physician or coroner, necessary permits, filing the death certificate with appropriate agencies, consultation with cemetery or common carrier, assistance with insurance papers, planning the funeral, placement of obituary notices. There will be a charge for each service requested or required. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for immediate burials and forwarding or receiving remains.) PROFESSIONAL SERVICES Basic Services of Funeral Director & Staff (as described above) ($3,000.00) Saturday, Sunday, Evening or Holiday Service Charge Add....($500) Transfer Of Human Remains To Funeral Home (20 miles radius) ($300) EMBALMING...($350) (Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you selected certain funeral arrangements such as funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you pay for as to it, Such as immediate burial.) SPECIAL CARE REQUIRED WHEN AUTOPSY PERFORMED.($250) ALTERNATIVE CARE (REFRIGERATION-Per day After 5 days)...($100) (If embalming is declined and final disposition of the deceased is not completed within twenty-four [24] hours from the time of death, we request refrigeration.) OTHER PREPARATION OF THE DECEASED ($400) (This service includes casketing and addition care of the decedent other than embalming or minimum care of remains for identification.) WASHING AND DISINFECTING UNEMBALMED REMAINS.($400) AUTOPSY REPAIR. ($175)

The only warranty on the casket sold in connection with the funeral service contemplated herein, is the expressed written warranty, if any, granted by the manufacturer. The funeral home makes no warranty, expressed or implied, with respect to the casket, including but not limited to the implied warranty of the merchantability or fitness for a particular purpose. BASIC CARE SERVICES FORWARDING OF DECEASED TO ANOTHER FUNERAL HOME.($1,000) This charge includes transfer of the deceased to our mortuary, basic services of the funeral director and staff, & local transportation. Additional charges for selected merchandise and shipping charges. RECEIVING DECEASED FROM ANOTHER FUNERAL HOME ($1,000) This charge includes basic services of the funeral director and staff, care of the deceased and transportation of the deceased to the cemetery for burial.

The Islamic Package a) The basic services of a Funeral Director and Staff. b) Removal from place of death, Los Angeles County. c) Meeting with family to obtain information on the deceased and sign legal documents and payment fees. d) Prepare death certificate and obtain Doctor s or Coroner s signature. e) File Death Certificate and obtain permit for burial. f) Coordinate with I.C.S.C for a grave in the Islamic section at Rose Hills and with Rose Hills to prepare with burial site. g) Transportation to Rose Hills for the burial in a van (NOT A FUNERAL COACH). h) Supply the approved and recommended casket (**) and Health Department fee for the burial permit. - Ziggler metal container is used along with Air Tray for overseas shipment (subject to requirements from the Air Line and receiving country s regulations) i) Preparing the body and use the facility for burial. The Islamic Center can arrange for a Muslim who is specially trained in preparing a body for burial to meet you at the mortuary a few hours before the burial. j) Supply granite Head stone. No Quranic verses are allowed on the stone. * The adult casket is an Industry Common Model Flare & Square Raised Flat top with no outer cloth covering. ** The child casket is an Industry Common Model Lamb available in appropriate size from 2/6 to 5/6 covering in white cloth. *** Certified legal copies of the death certificate are available through the L.A County Health Department at $12.00 each. The mortuary will order them for you. THE TOTAL PRICE FOR THIS PACKAGE IS: $ 3,913.00 Service Fee = $3,000.00 Casket = $250.00 Head Stone = $650.00 Permit = $13.00 TOTAL = $3,913.00 Items not included in the Islamic package: a) Grave cost obtained through The Islamic Center of Southern California. $1,500.00 b) Any Coroner s fee that may apply. c) Rose Hills charges (as of 1/1/2011) I. Opening and closing grave (Mon. thru Fri.) $1,075.00 II. Grave Liner (required by cemetery) $430.00 + 35.48 (tax) = $525.00 III. Witness interment $445.00 IV. Vault setting fee $270.00 V. DCA charge $ 8.50 VII Tax $ 55.09 Total Rose Hills Fees. $2,418.59 Additional Rose Hills items of service: Saturday interment Additional $540.00 Overseas Shipment: Service charge: $3,500.00 Embalming is required for oversees shipment: $350.00 Ziggler & Air Tray is used for overseas shipment. $1,200.00 Airline expenses varies by weight, destination determined by the carrier at the time of shipment. Additional charges from outside agencies may be added. Rose Hills requires that all decisions, financial arrangements and necessary signatures* be completed by no longer than NOON the day prior to the intended service/interment. In order to allow sufficient time to finalize all paperwork, an appointment should be made no longer than 10:00 am the day prior to the desired interment. I.C.S.C Mortuary makes no guarantee that we will be able to obtain and file the death certificate in time for an interment the following day. ** The next of kin, as governed by section 7100 of the California Health and Security Code, may sign the Interment Order at the interment site if necessary. However, should that person not be in attendance, we do not obtain the proper authorization, the interment cannot take place.

THERE IS NO SCIENTIFIC EVIDENCE THAT ANY CASKET WITH A SEALING DEVICE WILL PRESERVE HUMAN REMAINS. The description, Protective and Non-Protective, are the manufacturer s. Although certain caskets offered for sale by this funeral establishment may be of better quality construction and comprised of more durable materials than others, neither this funeral establishment nor any of its employees represents or implies that this casket will be airtight or water tight or will provide long-term preservation of the human remains.
