THE NEW HAMPSHIRE LOTTERY COMMISSION NHL HOCKEY GAME TICKETS SECOND-CHANCE DRAWING RULES 1. Any non-winning ticket for the $5 NHL HOCKEY instant scratch game (NH Lottery game number 1265) may be entered for one of two (2) drawings as detailed below. 2. To be eligible, the name and mailing address must be legibly printed in the spaces provided on the back of the ticket. The ticket must also be signed. A ticket that does not clearly identify the entrant s contact details will be discarded. A mailing label is acceptable as long as the ticket is also signed. The ticket must not contain any winnable prizes and must not be creased, folded or otherwise damaged or be altered in any way to give it an unfair advantage in the drawings. 3. There will be two (2) drawings total, one held on each of the following dates at Lottery headquarters: November 13, 2015 and May 20, 2016. (Dates are subject to change at the discretion of the Lottery Executive Director). 4. Eligible entries must be received by 4:00pm EST on 11/10/2015 or 5/17/2016. Tickets received after these deadlines will be entered into the next available drawing; tickets received after 4:00pm on May 17, 2016 will be set aside for destruction. 5. Entries are good for one drawing only. Tickets not drawn for a prize at any drawing will be removed for destruction prior to any subsequent drawing. 6. Fifteen (15) eligible tickets (defined in (2) above) will be drawn at the drawing on 11/13/2015 and shall be awarded a 2016 Bridgestone NHL Winter Classic Trip comprising: a. 3 day/2 night New Year s trip to Boston & Gillette Stadium b. Private New Year s Eve dinner with hockey legends c. New Year s Eve party with hockey legends d. Chalk Talk pre-game event with a hockey legend e. Winter Classic T-shirt and hat for winners and guests f. Two (2) tickets to the Winter Classic g. $250 travel allowance h. $300 spending money i. Federal taxes paid (25%) j. Total value of prize is $9,067.00 7. Fifteen (15) eligible tickets (defined in (2) above) will be drawn at the drawing on 5/20/16 and shall be awarded a 2016 Boston Bruins Suite Night comprising: a. Private pre-game winners dinner with hockey legend b. Chalk Talk pre-game event with a hockey legend c. Suite tickets to Bruins game for two (2) including food and non-alcoholic beverage with hockey legend d. Pre-game tour of TD Garden e. Bruins merchandise for winners and guests f. $200 travel allowance and $50 Garden Gold Card g. Federal Taxes paid (25%) h. Total value of prize is $1,976.00 Page 1 of 5
8. At the conclusion of each drawing described in (3) above five (5) alternate tickets will be drawn. 9. No person may win more than one (1) prize per drawing date. In the event that two or more tickets are drawn with the same entrant s name, only the first ticket drawn will be eligible to win a prize; the subsequent ticket(s) will be discarded and another will be drawn in its place. 10. Federal taxes estimated at 25% of the value of each package that is valued at $600 or greater will be borne by the New Hampshire Lottery Commission, whose agents will send winners IRS Forms 1099 in January of the year following the respective usage of the awarded prize. Any other taxes or fees will be responsibility of the prize winners. 11. The prizes will be as described above and there will be no cash option for any prize won. 12. Game tickets will be for games determined by the Boston Bruins and NHL only and cannot be exchanged for different games. 13. Winners will be notified by mail, email, or telephone, using the contact information on the drawn tickets. Prizes will be fulfilled by MDI Entertainment. Prizes sent to winners from MDI Entertainment may use certified mail and/or Federal Express with a signature required. 14. Winners will be required to complete a New Hampshire Lottery Commission Prize Claim Form as well as provide copy of photo identification and proof of social security number and return same within 30 (thirty) days of the forms being mailed from the Lottery. Failure to comply will result in the prize package being forfeited and the prize will be awarded to the first available alternate winner. 15. The NH Lottery reserves the right to amend the drawing rules as necessary without prior notice no less than 10 (ten) days before any drawing. 16. The NH Lottery offers no warrantees or guarantees of product quality and that availability and quality of the prize and its conformance to advertised representations is the sole responsibility of the promotional partner. 17. The NH Lottery shall not be responsible for any prizes lost, damaged or stolen during shipment, pickup or use. 18. The NH Lottery shall not be responsible for injury or loss of life resulting from any prize awarded. 19. CHILD SUPPORT INTERCEPT Pursuant to RSA 284:21-v, the New Hampshire Lottery Commission is required to review all lottery prize claims subject to federal tax reporting with a child support database provided by the Department of Health and Human Services (DHHS). In the event that a winner of any prize package described above owes child support, the winner shall have 21 days from the draw date to settle the total arrearage due as determined by DHHS to the DHHS, after which settlement the winner will be awarded the prize. Failure to settle any arrearage shall result in that winner s prize package being forfeit and the package will be issued to the first available alternate winner. The original winner(s) owing Child Support shall receive no other prize or compensation from the New Hampshire Lottery Commission relating to this game. 20. Abstract from CHAPTER SW 6000 RULES OF SECOND CHANCE DRAWINGS Sw 6001.01 Definitions PART Sw 6001 SECOND CHANCE DRAWINGS The following definitions shall apply for the second chance drawing rules: Page 2 of 5
(a) Eligible means as authorized by the New Hampshire Lottery Commission (NHLC). (b) Instant scratch game means a lottery ticket sold by a NHLC licensed retailer that requires the player to remove a covering to reveal the play symbols to determine if the ticket is a prize winner in accordance with game instructions. (c) Player means an individual who participates in a drawing by submitting a designated entry form, ticket, or identifying number(s) from an eligible ticket. Sw 6001.02 Valid Entry Characteristics. All entries for a second chance drawing shall meet at least one of the following characteristics as designated by the NHLC: (a) An instant game ticket shall meet the requirements set forth in Sw 1002.01 and Sw 1002.02; Sw 6001.03 Invalid Ticket or Form Any ticket which does not meet the drawing entry requirements designated by the NHLC shall be considered invalid for the drawing and shall not be eligible for any prize from that drawing. Sw 6001.04 Drawing Entry Deadlines. An entry ticket shall be received by the deadline date and time as designated by the NHLC. An entry ticket received after the drawing s deadline shall be considered invalid and ineligible for any prize from that drawing. Sw 6001.05 Drawing Procedures. (a) Drawings shall be conducted in a public accessible place. (b) Drawings shall be video recorded and the recording shall be securely stored for one year from the date of drawing. (c) Prior to the start of a drawing the following information shall be announced by the draw supervisor: (1) The name of the lottery personnel assisting with the drawing; (2) Date and time of the announcement; and (3) A summary of the prize to be awarded during the drawing. (d) The following procedures shall be used in the event that the drawing entries are physically selected from a container: (1) If possible, the container shall be rotated at least 3 times before selecting an entry; (2) A member of the public shall select an entry or entries from the container; Page 3 of 5
(3) In the event that a member of the public is not available, a lottery staff person who is not assisting with the drawing shall select an entry or entries; (4) Any person selecting an entry shall first complete an attendance sheet listing their name and address, and shall announce their name for video recording purposes; (5) Any person selecting an entry shall first be checked by lottery staff to ensure that there is nothing held in their hands and that their shirt sleeves, if applicable, do not extend below their elbow; (6) Any person selecting an entry shall have the front and back of their hands video recorded immediately prior to them entering the container to select an entry; (7) The drawing supervisor shall announce the prize to be awarded prior to the selection of an entry; (8) Any person selecting an entry shall not look in to the container when selecting an entry and shall, instead, look at a spot designated by lottery staff; (9) Each selected entry shall be immediately given to the lottery draw supervisor who will review the entry to ensure its validity; (10) The draw supervisor shall announce the following information for each valid selected entry: a. The prize awarded; and b. The name and address listed on the entry; (11) The draw supervisor shall announce the following information for each invalid selected entry: a. The reason why the entry is considered invalid; and b. The name and address listed on the entry; (12) In the event an entry is considered invalid, the prize shall be awarded to the next selected valid entry; (13) In the event that 2 or more entries are selected at the same time, the draw supervisor shall: a. Count and announce the number of multiple entries selected; b. Place all multiple entries back in to the container; c. If possible, rotate the container at least 3 times; and d. Have another entry selected; (14) Following the selection of the final valid entry, 5 additional entries shall be selected and designated as alternate entries to be used in the event an entry or entries are later determined to be invalid. (15) If needed alternate entries shall be used in the order selected; (16) If a selected entry is determined by the NHLC to be ineligible to receive a prize, the next available alternate entry shall be awarded the prize which would have been awarded to the ineligible entry; and (17) The draw supervisor shall announce the completion of the drawing following the selection of the final valid entry.
