There are currently five levels of assessment that are possible within each department. These levels are related to development of the department assessment data gathering techniques and use of the information to fine-tune courses as necessary. Activity Level: Level I: The department assessment processes have been detailed and developed for use by faculty. Level II: Data collection has been implemented. Level III: Faculty, instructional leaders, and deans have analyzed the data. Level IV: Faculty, instructional leaders, and deans have used the data to improve student academic achievement. Level V: Data has been used to improve the assessment process. STUDENT ASSESSMENT FOR COSMETOLOGY Fall 2008 1. Syllabus to keep on track of the chapters each week and be able assign what reading is to be done -- week by week. We have a New Students/1 st Semester and Returning Students Syllabus. 2. NPC Cosmetology School Rules/Monitors, Station Top This is the responsibility of each student. Students must practice and comply with the school s dress code and school rules, be prepared for class, and be attentive in class. Students are directed to learn the information taught and to study the materials provided by the school. Student will be written up for disobeying any of the rules. Faculty monitor each student each day to keep students on task. 3. Clinic Requirements for 2 nd Semester This is a form that the student will sign once they have seen a demonstration and have practiced the procedure on another student. 4. Visual Aid and Demonstration Activity Record This is a record of each demo that is given out on the clinic floor both am and pm. 5. Demonstration Sign-Off Sheet During the first 300 hours the instructor s will demonstrate all procedures on this list and sign-off as the student observes each demo that way there is a record to make sure each student has seen the complete list of demos and then the student will practice the demo. 6. MSDS Data Sheet This is a How to Understand Material Safety data sheet. This is a way of understanding how to read each sheet including those on Chemical Identification, Hazardous Ingredients, and Physical Data. This is a must when working with chemicals. 7. Blood Spill Procedure IF a blood spill should occur the students must know what steps must be followed. 8. NIC Health and Safety Standards Blood Spill Procedure. This is the candidate injury or client, model if they are injured. This form also covers Wet Disinfection Standard, Dry Storage Standard, and Hand Washing. 9. Whirlpool Footspas Notice to Licensees. This is letting us know what are our responsibility and standard of cleaning pedicure foot spas, and whirlpool foot baths. These are given to us by the State Board Cosmetology. This form is critical to follow proper health and safety procedures to protect the well being of you and your clients. 10. Confidential record of Student Counseling
A record of counseling during their school training. 11. 300 hour test practical (hands on) procedures the student must perform on manikins or on each other prior to getting out on the clinic floor to perform these procedures on actual clients. When the student has completed 300 hours they will complete each task while being timed and having each task signed off by the Instructor. That way we know what the student can complete the task in a safe and timely manner. 12. Test Written 300 Hour Exam This is a multiple choice 100 questions to evaluate the students retention on the first 300 hours of theory. 13. Salon Project Grading Sheet This assignment helps students learn what it takes to plan an opening of a salon, from a floor plan to service prices per client as well as planning financing or loan with the bank. This is given in two parts part 1. Business Plan 2. the rest of the project. 14. Pink Sheet of the Month This form is a record of what the student is working on when they are not working on clients. The student must complete task each day and have signed by an Instructor who assesses the work and the improvement each time it is repeated. If the student is working on clients they need to keep a record of clients of each task in the same way. 15. Cosmetology Requirements This is a total of all the pink sheets added up showing what the student has completed at the end of 1600 hours. These are the minimal requirements by the board of Cosmetology. 16. Semester Exam 300 questions on what students have in theory for the semester. This is a multiple choice or selection test. This is to help evaluate students retention of the textbook information. Basically it is a collection of questions reviewing the weekly quizzes the students have during the semester. Standard Theory Workbook Test Four tests are given during the students training on subjects such as Anatomy, Cosmetics, and on all chapters in the text book and the work books. This is to help evaluate students retention of theory. Some tests have 50 questions, some have 100 questions. All are multiple choice or selection tests. 17. 1400 Hour Test When a student has completed 1400 hours of instruction they are given their 1400 hour exam. This is a way to make sure they know and understand their theory. This is a method of evaluating student s retention.
18. 1600 Hour Test This test is given at the end of a student training. It is a comprehensive multiple choice test. This test covers all of the chapters, laws, and rules. This test has 300 questions. 19. State Candidate Information Bulletin This form lets students know about their exams, questions, and special needs for their State Board Exams. The bulletin also includes information about Infection Protection Product, Examination Safety, and Scoring Information. 20. National Candidate Information Bulletin Each candidate will be given a practical and written exam information bulletin for each test. a. Cosmetology Practical Cosmetology Written b. Instructor s Practical Instructor s Written All of these bulletins contains important information regarding the (NIC) National Cosmetology Practical, Written Examination and administration. All students please refer to your state guidelines. 21. Americans with Disabilities Act Policy Any candidate with disabilities will need to apply for accommodations. Completion of this form constitutes a request for modifications and/or special accommodations. 22. Northland Pioneer College Chemical Awareness Program This form is for all students enrolled in a course part time or full time. All staff employed on a full-time or part-time basis by NPC College receive a copy of this standard of conduct policy relating to chemical awareness hand out. Hand Out 23. Infection Protection Standard of Excellence This booklet is teaching the student how to performed services safely in salons and school and to educate and enforce cosmetology laws once licensed. This booklet also teaches the most updated measures to prevent the spread of disease. 24. Infection Protection Questions This is a true or false test there are 137 questions, some multiple choice. This is to evaluate student s retention.
25. Hair Cutting Basic This is a great visual aid when teaching Basic Lines in haircutting. It also shows sections of the head, nap lines, side and nape lines, block cut method. This booklet is a great visual. 26. Head Form This will let the student draw the lines used in hair cutting, when styling the hair parting used for guidelines. 27. How to Give a Permanent Wave This is a procedure of permanent wave, test curl, and blocking & wrapping, parting sections. 28. Face ad Scalp Massage This is definitions for face and scalp manipulations used in massage. The procedure for facial and scalp treatment. 29. State Board Curls This form shows base, curl stems of pin curls, and all other types of curls we work with, including Flat curl, standup skip waves pin curls, reverse curls. As a student of cosmetology they will need to learn all kinds along with parting. 30. Hair Coloring Application This form shows where to apply color. There are a lot of applications used in chemical services. The student will need to know where to apply when to apply what size of sections to take. Where the heat zone is and where the hair is porous. 31. 4 Quadrant Application Test This form is a way of testing the student on application of a virgin tint, retouch, relaxer virgin, relaxer re-touch. 32. Thermal Curling Method for Long Hair or Short to Medium Hair Thermal curling, which requires no setting creams or lotions may be used to great advantage in a quick style just by using your thermal iron. And know your curling iron manipulations. 33. Pedicure Procedure The need for pedicures is a major service. Students learn the procedure for pedicures, how to protect a client from diseases, and other safety measures. The goal is to prevent injury to the client and the student. The procedure also shows foot massage and the reflex reaction to a stimulus of the foot to a nerve. 34. Manicure Procedure This is another procedure that in the salon is a very high demand. The students need to learn all kinds of manicures from the water to a hot oil, and spa manicure, as well as how to protect a client from diseases and other safety procedures. The student also learns how to massage arm and hand. The Hot Oil Manicure is on the back side.
35. Hand & Arm Massage This is the basic hand and arm massage and manipulations. This is teaching students to keep contact, and keep consistency in the massage to promote total relaxation in your client mind and body. Again students learn safety at all times. 36. Foot Reflexology Students learn how to massage over the foot with pressure, consistency over the nerves to promote relaxation during a massage. 37. Hand Reflexology Students learn how to do the manipulations over the hand nerves to promote health and relaxation during the massage. This is an extra high-end in manicuring and pedicures. We also teach safety what we can and what we can t work over. 38. The Creative Academy Student Handout This is a nail diagram Nail Tech and Cosmetology students working on the hand and fingers need to know all parts of the nail. This diagram helps to evaluate students retention of topic. 39. Teaming with Creativity This handout from Creativity (CND) is one of products used in salons and schools today. This helps to build the strongest possible enhancements that won t lift. 40. Tommy Taylor Nails Handout letting student know of the product. 41. Skin Conditions Handout showing conditions of the arms and hands. 42. Nail Demo and Work Checked All students have to see demos and then practice each task. All tasks need to be checked by an instructor who signs off on this sheet. 43. Nail Technology Testing Sheet This test that is timed at 150 hours and then again at 600 hours. Practical Test. 44. Safety and Health Protection on the Job This is a must have at all school and all salons for both employers and employees. Arizona law also requires that each employee comply with Safety and Health Standards and Regulations. Orders issued under the act are and applicable to one s conduct.
