Governance structure 1. Governance structure 1.1 Governance diagram The governance structure for the CCHP Program is depicted in the diagram below. Note This diagram would be replicated for each project in its respective Implementation Plan, clearly stating the specific Business Unit groups. Governance structure 1.2 Roles and responsibilities 1.2.1 Program Owner The Program Owner is the champion of the CCHP Program. He or she promotes the benefits of the program to the community, government and key stakeholders, and may be viewed as the public face of the program. The Program Owner has delegated authority of the Steering Committee to assist with program management issues that arise outside the formal business of the Steering Committee. Program Owner Deputy Secretary, Community Health Services 1.2.2 Project Owners As CCHP is a program of projects, there will be multiple Project Owners (ie one for each project established within the program). Project Owners within the CCHP Program are responsible for managing the delivered project outputs, and are held accountable by the Program Owner for the achievement of the project s outcomes. Project Owners are responsible for assisting the CCHP Team from a service management perspective to enable the delivery of the projects outputs. Project Owners Director, Primary Health Services Director, Mental Health Services Director, Statewide Specialist Services 1.2.3 Steering Committee The Steering Committee supports the Program Owner and each Project Owner with the delivery of the outcomes for each project. The Steering Committee is responsible for policy and resourcing decisions essential for the delivery of program and project outputs, and the attainment of project outcomes. It is also responsible for ensuring appropriate management of the program and its projects as outlined in the Business Plan and each project s Implementation Plan. Governance structure Members Program Owner (Chair) Project Owners Deputy Secretary, Shared Services Director, Information Services Acute Health Services representative External DHHS representative Program Manager (ex officio) 1.2.4 Program Manager The Program Manager is contracted to the Steering Committee to deliver the defined program level and project outputs that are fit for purpose within the constraints of time and cost. He or she is responsible for organising the program into projects (and sub projects), the management of the program s resources, the development of project plans, resolving planning and implementation issues, and monitoring progress and budgets. 1.2.5 Implementation Management Groups These groups will provide collaborative support to the Project Owner and Program Manager in monitoring and resolving service/business unit specific and operational issues occurring as a result of the implementation of the CCHP Program. These groups will also review recommendations and/or advice as requested by the Reference Group(s). Issues and/or recommendations may be relayed to the Steering Committee for resolution and/or endorsement through the Project Owner or Program Manager. It is envisaged that the Project Owners and their respective business unit/service/regional managers (as appropriate) would be members of this group. Separate Terms of Reference will be developed for each Implementation Management Group. Implementation Management Groups Primary Health Services Mental Health Services Statewide Specialist Services 1.2.6 Clinical Reference Groups These groups provide expert advice to that project s respective Implementation Management Group, and the CCHP Team, on matters relating to the service and clinical functions of the service/business unit. Governance structure It is envisaged that projects may require multiple reference groups that may represent specific service streams (eg Palliative Care within Primary Health Services). Separate Terms of Reference will be developed for each Clinical Reference Group. Clinical Reference Groups Primary Health Services Mental Health Services Statewide Specialist Services 1.2.7 CCHP Team The CCHP Team is led by the Program Manager, working for the successful delivery of program and each project s outputs. The CCHP Team will comprise permanent and fixed term staff from the various DHHS groups/business units, as well as sub contractors and/or consultants; all required enabling the delivery of the defined outputs. The CCHP Team will be structured into multiple project teams, each led by a project leader responsible to the Program Manager. 1.2.8 Quality Review Consultant The Quality Review Consultant independently advises the Program Owner and Steering Committee on the selection, application and effectiveness of project management processes and standards. 1.2.9 Key Contractor XYZ Pty Ltd is the vendor of the xyz application.
