JOB DESCRIPTION Title of Post: Support Worker, Mental Health Grade/ Band: Band 3 Directorate: Reports to: Accountable to: Initial Location: Hours: Adult Services, Mental Health Programme Team Leader Community Mental Health Services Sector Manager Initially based in the Home Treatment House, Conlig (the Post holder may be required to work in any location throughout the Trust) 37.5 hours per week, including shift work and weekends Job Purpose The Support Worker, Mental Health will work as a member of a multi disciplinary team as part of a range of Mental Health services, which include Mental Health Outpatients, Mental Health Day Hospitals, Mental Health Inpatients, Home Treatment, Community Mental Health Teams and Assessment Centres. They will work within a matrix organisation promoting effective inter-disciplinary and interagency relationships and acting as a link between service users, professionals and other services to ensure continuity of care. The purpose of the Home Treatment House is to provide an alternative to inpatient admission or facilitate early discharge from hospital. The service seeks to enable service users to receive treatment and care in an environment offering support and encouragement but promoting ownership and independence. The Home Treatment House will maintain users links with the Community Services, Carers, Friends and Family, therefore minimising the disruption of their lives and the lives of those caring for them. The post-holder will work as part of a team to ensure a healthy and normal lifestyle for people with a mental illness, providing a therapeutic environment underpinned by Recovery Model approaches. The successful applicant will be required to work in any part of the South Eastern Health and Social Care Trust area within any mental health team or service as needs dictate. Mental Health services are flexible and responsive requiring staff to work day and night shifts over a seven day period.

Main Responsibilities Clinical/Professional/Specific Managerial Responsibilities To support clients in different settings, within the Home Treatment House, in their own homes and during organised outings. To provide clients, where appropriate, with assistance and guidance with areas of personal care. To assist and support client s in daily living skills, within the Home Treatment House, ie, household skills, individual / group activity and during organised therapeutic activity in the wider community. To support clients in the ordering and collecting of their prescriptions and with medication regimes. To advise and support clients in the maintenance of good standards of home care and budgeting skills, and advice in the management and security of their homes. To accompany and support clients whilst shopping or engaged in other community activities, as necessary. To facilitate effective communication with clients, families and team members regarding the planning, support and development of each support package. To support directly an individual or small group in activities located within the Home Treatment House or within the local community. To assist clients overcome problems they might experience whilst participating in community activities, and to assist clients in developing an awareness of their locality / local community. To enable clients to increase their capacity for independence and independent decision making. To report on any observed changes in mood or behaviour, health care needs or mental health care needs To notify the manager or their nominee of any change in the client s circumstances and maintain all records as instructed by the manager, and report any untoward incident or complaint, or abuse of a vulnerable adult. To observe all health and safety regulations across the service, ensure correct use of electrical equipment, ensure fire procedures are adhered to, and report all faults to the appropriate authority.

To attend staff meetings, training, supervision and appraisal sessions as required. To liaise with the manager on all leave entitlements and adhere to the policies and procedures of the Trust. Records Management All employees of the Trust are legally responsible for all records held, created or used as part of their business within the Trust including patient/client, corporate and administrative records whether paper based or electronic and also including emails. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environmental Information Regulations 2004 and the Data Protection Act 1998. Employees are required to be conversant with the Trust s policy and procedures on records management and to seek advice if in doubt. Environmental Cleaning Strategy The Trust s Environmental Cleaning Strategy, recognises the key principle that Cleanliness Matters. It is everyone s responsibility, not just the cleaners. Whilst there are staff employed by the Trust who are responsible for cleaning services, all staff employed by the Trust have a responsibility to ensure a clean, comfortable and safe environment for patients, clients, residents, visitors, staff and members of the general public. Infection Prevention & Control All Staff should co-operate fully with regard to Trust policies and procedures relating to infection prevention and control. All staff should be aware of the Trust s Infection Prevention & Control strategy and their local reduction of infection plan and know their role, in keeping with key principle Infection Prevention and Control is everyone s business. Staff, in delivery of all care must:- -Wash their hands thoroughly between each patient contact. - Be compliant with Standard Infection Control Precautions, Hand Hygiene and decontamination and other relevant infection prevention and control measures. - Be aware of the Infection Control guidance within the Trust s Infection Control Manual and ensure they obtain mandatory Infection prevention control training or other specific infection control related training as required.

Hygiene & Prevention of Infection You will at all times be required to observe hygiene/infection prevention & control standards and notices in operation throughout the Trust. Hygiene and control of - infection are key elements in delivery of safe, quality services and you will, in certain jobs, be required to undertake an appropriate training course at the expense of the Trust. Personal Public involvement Staff members are expected to involve patients, clients and their families in developing, planning and delivering our services in a meaningful and effective way, as part of the Trust s Personal Public Involvement (PPI) Strategy. Location Please note that it is a standard condition that all Trust staff may be required to work at any location within the Trust s area, as needs of the service demand.
