The vision and model for school nursing An opportunity for school nurses to re-claim their role as leaders and delivers of public health to school aged children Safeguarding Community Universal Services Universal Plus Universal Partnership Plus describes a range of health services (including GP and community services) for children and young people and their families. School nurses will be involved in developing and providing these and making sure you know about them. s from your school nurse team provides the Healthy Child Programme to ensure a healthy start for every child this includes promoting good health for example through education and health checks, protecting health eg by immunisations and identifying problems early provides a swift response from your school nurse service when you need specific expert help which might be identified through a health check or through providing accessible services where you can go with concerns. This could include managing long-term health issues and additional health needs, reassurance about a health worry, advice on sexual health, and support for emotional and mental health wellbeing. Plus delivers ongoing support by your school nursing team as part of a range of local services working together and with you/your family to deal with more complex problems over a longer period of time

What did children, young people and parents tell us? Children Young people Parents School nurses need to be more visible in schools. School nurses need to be visible and well known to pupils. School nurses need to use technology such as texting and emailing. School nurses need to be more visible in schools and to parents. Pupils should be introduced to their school nurse. Pupils should be told when the school nurse is available to see them. Pupils should be told where the school nurse can be found School nurses need to offer early help to support young people. School nursing services need to offer choice to young people in order to ensure that services are accessible and confidential. Young people want to be able to offer their views about the services they receive Parents should be introduced to the school nurse and the service available. Parents need to be advised directly about the service through letters, leaflets and clear information. Parents need to be told where the school nurse can be found and provided with contact details. Be in synch with the youth..

We work both in and out of School to provide support around Keeping Healthy Immunisations Emotional Health Weight Management Sexual Health Drugs and Alcohol Smoking More Support for those who need it most A Confidential Service This means that you can discuss personal information in confidence. We will not discuss your personal information with anyone, else without your permission. This includes parents and teachers. We would only pass on your information in order to protect you or someone else from serious harm. Whenever possible we would discuss this with you first. See a school nurse before it gets worse

A PARTNERSHIP PATHWAY.What is a pathway? Guidance to support community nurses to work more collaboratively with other key professionals and partners to deliver improved quality of life and health outcomes for both the carer and the people they care for. including: AHP s, GPs, practice nurses and third sector agencies. A structured approach to addressing the common issues identified by community nurses and their partner professionals to support the needs of carers To enhance and maintain quality of life and health outcomes; including options for service delivery which are detailed together with a comprehensive timeline. Delivery of the pathway requires the skills, knowledge and leadership of District Nurses, effective partnership working with partner organisations and stakeholders whilst respecting carers as expert partners in the care (to include the prevention of cared for persons needing acute hospital or residential care). Builds on good practice and provides a systematic solution-focused approach for practice development. Focus on addressing the support required, for young carers whilst recognising that each carer will have unique needs and needs that change due to personal circumstances

Workforce issues, lack of training opportunities: Addressed through joint training, e-learning and shared learning resources Communication systems; fragmented within health and partner organisations: Addressed through shared learning and dissemination of best practice to develop seamless sign posting between the professions. Standardisation of procedures: Transfer and sharing of records between health and social services Service fragmentation and variation: Addressed through clear identification of the different yet complimentary contributions of professionals and partner organisations throughout the transition timeline. Evidence base is limited and often under-utilised: Addressed through the identification and promotion of supporting policy and evidence.

Call to action. Support when we need it most to avoid crisis Do you have good links with the school nursing services? Do you understand the local service offer? Do you know how to advise young people (and parents how to access the service? Are you agreeing local packages of support including the school nursing service? Are you sign posting young people to their local school nursing service? Do you receive regular updates on your local service? Do you provide information for the local school nursing service about what you can offer?
