2 This brochure provides a general overview of the services Pathways, CareNZ and Workwise offer in the Wairarapa region. Mobile community support services. Residential services. 3 Support that s right for you Sometimes life presents challenges that are out of the ordinary. You may have been feeling down for a really long time, or you may be struggling with the hold drugs, alcohol or gambling have on your life. It s important to know you have a lot of options available to you. You re not facing these challenges alone. We provide community-based mental health and addiction services in Wairarapa. We ll talk to you to work out what s going on in your life that s causing you concern. Then, we can offer you a range of options to help you get life back on track. Together we provide a wide range of communitybased mental health and addiction services, which are outlined in detail in this brochure. Every person is different, so if you visit our service we ll work with you to map out a plan that is just right for you. It doesn t matter to us whether you have issues with substance misuse, or issues with your mental health, or both either way, you re in the right place. Who we are Mental health community support and addiction services in Wairarapa are delivered through a partnership between Pathways, CareNZ and Workwise. Pathways has been providing community-based mental health and wellness services throughout New Zealand for 25 years. Pathways is dedicated to doing whatever it takes to create mental health and wellbeing opportunities to enable people to live their dreams and flourish. CareNZ helps people and the people who love them who are struggling with alcohol and/or drug misuse problems to change their lives for the better. CareNZ is the delivery arm of the New Zealand Society on Alcohol and Drug Dependence (NSAD). Workwise help employers gain and keep valuable employees by supporting people with mental health and/or addiction issues to return to work and stay in work. 3
4 Mobile community support services Mobile support is available for people living in their own homes or with friends or family/whanau. We can visit you at your house or meet you somewhere else in the community. We offer two levels of mobile support in the Wairarapa region. 1. Mobile community support. 2. Enhanced mobile community support. When you re using our mobile services we ll talk with you regularly about how much support you need. Some people are seen just once a week, others may need a higher level of support and are seen every day. It s about finding what works best for you. What is offered? Pathways Wairarapa Creating mental health and wellbeing opportunities that enable people to live their dreams and flourish. If you re using our mobile services you ll have a dedicated key worker who you ll work with most. This person will: provide flexible support to suit your needs support you to identify your goals and help you to achieve them as you journey towards recovery provide support around practical daily living support you with your accommodation needs support you to find a job or access training if you want it, get involved in the community and choose a healthy lifestyle. Where is this service offered? Pathways Wairarapa offers mobile support services to people living in the greater Wairarapa region from its base in Masterton. How is this service accessed? Referrals are made through community mental health teams at the Wairarapa District Health Board. You can also contact Pathways directly or your GP may refer you. 4
5 Residential services For some people supported accommodation is an important step towards living independently. Our residential services support you to work towards moving into your own home and living well in the community. What is offered? We offer a transitional residential service in the Wairarapa region offering various levels of support. All services have staff on site 24 hours a day, 7 days a week. While staying at a residential service you will have a dedicated key worker who will work with you to develop a goal plan. Support will vary depending on what you need but may include: help in achieving your goals support around practical daily living such as shopping, cooking or household chores budgeting advice and support self-management of medicine support to find and get a job accessing community services, groups and training choosing a healthy lifestyle such as stopping smoking, exercising and eating well. The length of time people use residential services varies depending on what they need. Support is also available to help people find a new home when they are ready to leave our residential service. Where are these services offered? Pathways provides residential services in Masterton. How are these services accessed? Referrals are made through community mental health teams at the Wairarapa District Health Board. You can also contact Pathways directly or your GP may refer you. 5
6 Alcohol and drug misuse services Substance misuse counselling Creating a recovery from substance misuse can be a long process one that requires time, commitment, motivation and lots of support. Whenever possible, we try to involve your family or significant others, and you are encouraged to bring a support person. Our aim is to provide a safe, confidential environment and to support you in your recovery. Our staff are passionate about your recovery and offer substance misuse counselling which is a free and confidential service, available to anyone who has concerns about their own alcohol and/or drug misuse or the alcohol and/or drug misuse of a loved one. Counselling may include a focus on abstinence, harm reduction or relapse prevention. Some of this counselling is one-on-one which means it s just you and your counsellor. These sessions give you the opportunity to talk to each other in a private and safe environment. We also offer education programmes around drugs and alcohol for school, tertiary institutions and community groups. Specialist supports If a person needs more intensive supports, we can refer through to inpatient alcohol and other drug residential services. People using our services may also work with a DHB psychiatrist. We are able to refer anyone using addiction services directly to the Hepatitis C clinic as required. Youth service Our youth team provides a professional and confidential service to the young people within our local community. We aim to reduce harm associated with alcohol and other drug misuse; and strive to assist youth with making positive choices by utilising strengths and building resilience. We provide treatment/intervention options to suit individual needs including individual, group and family work and are strong advocates for youth health and wellbeing. Where are these services offered? 6 CareNZ offers these services to the greater Wairarapa region from its base in Masterton. How are these services accessed? You don t need a referral to access these services. You can contact CareNZ directly, or your GP may refer you.
7 Residential managed withdrawal A short-stay option for people working through issues associated with detoxification from drugs or alcohol. It is staffed around the clock by support workers and a detox nurse. Opioid substitution treatment programme We offer an opioid substitution treatment programme (which includes methadone). Our opioid substitution team includes a doctor, nurses and counsellors, who work in partnership with clients to reduce the harm and dependence associated with opiate drug use. If you re ready for change, and in a place where you re starting to think opioid substitution treatment might be for you, come in to our Masterton clinic and meet with one of our case managers for an assessment. When opioid dependence has been established and you ve been accepted into the programme, you will have a comprehensive assessment. As no single treatment is effective for all individuals, your case manager will work with you to establish your personalised treatment plan, set goals and will monitor your progress throughout your treatment. You may be referred back to your GP for ongoing management. What is offered? Comprehensive assessment and treatment planning. Screening for blood borne viruses. Referral for Hepatitis C treatment. Withdrawal support and detox management. Referral to other services. Medical review. Case management. Counselling support. Safer injecting education. Where is this service offered? CareNZ offers its opioid substitution treatment services to the greater Wairarapa region from its base in Masterton. How is this service accessed? Referrals can be made through your existing healthcare provider or by contacting CareNZ directly for an appointment. 7
8 Intensive outpatient services CareNZ offers a group therapy programme called the Intensive Outpatient Programme (IOP). The programme runs for nine weeks. You will learn about substance misuse and ways of living free from drugs and alcohol. During the programme you will spend three days a week participating in group therapy and education as well as individual recovery planning. Some people find the thought of group therapy daunting but you shouldn t worry. Group therapy provides a supportive environment for you to work through your substance misuse issues with others who both understand what it is like to live with substance misuse and can share their experience. The real benefit of this programme is that you can share your experience with others and also learn skills to help you cope with some of life s everyday challenges. What is offered? CareNZ s treatment programme includes: education group therapy a discharge plan individual recovery planning. Topics covered in the programme include: the effects of alcohol or drugs relapse prevention techniques managing depression and anxiety self-care and lifestyle balance anger management and healthy communication. The programme is run in a safe and supportive environment by our team of experienced professionals. Where is this service offered? CareNZ offers intensive outpatient services to the greater Wairarapa region from its base in Masterton. How is this service accessed? 8 Referrals to this service are made through your existing counsellor or by contacting CareNZ directly for an appointment.
9 Problem gambling services Problem gambling counselling We offer counselling and support for people who are concerned about their own, or another person s, gambling. The service is free and confidential. CareNZ s gambling counsellors will listen and work with you and anyone else you choose to have involved to develop an appropriate recovery plan to support everyone through this difficult time. Where is this service offered? CareNZ offers its problem gambling service to the greater Wairarapa region through its base in Masterton and an additional outreach clinic in Featherston. The service is mobile and can also visit people in their homes, at schools or at other community agencies. How is this service accessed? Please contact CareNZ directly for an appointment. No referral from a healthcare practitioner is required. Gambling education programmes Our gambling public health work raises community awareness about problem gambling and its effects. Where is this service offered? CareNZ offers its gambling education programmes to schools, community groups, tertiary institutions and any other interested groups throughout the greater Wairarapa region. How is this service accessed? Please contact CareNZ directly to arrange a gambling presentation for your organisation. 9
10 Employment Services Having a paid meaningful job is often a key part of getting well and staying well. Workwise employment consultants Workwise employment consultants can help you get back in the workplace. Workwise is a unique employment agency for people who are facing personal or health challenges. Workwise consultants are incredibly passionate about helping people choose, get and keep a job. They ve helped many people return to work; and can do the same for you. How much employment support you want will be up to you. You may need help returning to an existing job or finding a new job. We can search for a job for you, you can search by yourself, or we can search together. What is offered? Employment consultants can support you to: explore your work passions and career dreams create employment goals, and a plan to achieve those goals work through any problems you see in returning to work plan your employment journey set up your CV and prepare for interviews job search maintain your employment manage your health while working. Pathways and Workwise provide employment services from our base in Masterton, and can work with you to find employment throughout the Wairarapa region. 10 How is this service accessed?
