Worker & Student Injury Reporting System Tools and Tips FREQUENTLY ASKED QUESTIONS/ROADBLOCKS TO REPORTING Issue 1: Answer 2: Issue 2: Answer 2: Issue 3: Answer 3: I get an error message when I try to log into WSIRS You have to be an active employee in order to have access to WSIRS under your own log in credentials. If you ve recently returned from leave, maybe the appropriate HR action hasn t happened yet to place you back on active status. Because not all OHSU employees have log in access to the OHSU intranet, WSIRS was designed to allow another OHSU employee with intranet access to log into WSIRS and then step away to allow the injured employee to complete the report. I m trying to type in the gray boxes and it won t let me. See Page 3 of this document screen shot at the top of the page. Nothing happens when I click the Add Injured Party button The pop-up blocker of the web browser you are using is blocking pop-ups from this site. Look at the top of the screen where you see the menu items File, Edit, View, Favorites and Tools. Click on Tools and look for the menu item that relates to pop-up blocker. Turn off the pop-up blocker and click the Add Injured Party button again. If at any time you have questions, please call 4-2451 or 4-7189. Your Novell name and password are entered here. This is your Dashboard The Supervisor sections will only appear on your Dashboard if an injury has been reported previously indicating you as the supervisor of the injured party. To begin entry of a new injury, press the Start a New Report button. 2
Click Add Injured Party button to pre-fill general information about the injured party. Indicated the injured party s role to OHSU when the injury event occurred. We are unable to automatically populate general information for Students, Contractors and Other injured parties. Please follow the screen prompts provided if one of these groups is selected as the role of the injured party. There are three ways to identify an injured Employee, Graduate Student or Resident by Employee ID, Email Address or Social Security Number. You only need enter one of these pieces of information in the appropriate box and press the Search button. Basic information about the individual will appear. 3
Not the right person? Click Change Injured Party and reselect. Incorrect information? (i.e. wrong phone number or mail code.) Click Edit Injured Party to correct. Enter information requested. 4
Click Add Supervisor/Proctor button to pre-fill contact information for the injured party s supervisor. Enter email address for injured party s supervisor and press Search button. You will need to scroll down to the bottom of this Section to proceed. 5
Press the Next button to move to the next screen. Please provide requested information. You will need to scroll down to the bottom of this Section to proceed. 6
This is a continuation of the previous screen (bottom of Page 5). Please provide requested information and click Next to move to the next screen. These questions relate to blood and body fluid exposures/sharps injuries. These questions relate to patient assist/lifting injuries. Please provide requested information. You will need to scroll down to the bottom of this Section to proceed. 7
Based on the answer to the injured party having sought or intending to seek medication treatment, the inactive/grey questions which follow, become active and need to be completed. This block of questions may or may not be visible based on previous responses. Click Submit when finished. The system will automatically contact the injured party s supervisor for additional information. The individual that completed the injured party information may check the status of a report by entering the system at any time. The Supervisor sections will only appear on your Dashboard if an injury has been reported indicating you as the supervisor of the injured party. The three most common status indicators for this stage in the reporting process are: Referred (Supervisor) waiting for action by your supervisor Processing - under review by Risk Management Processing Complete at which time the injured party and their supervisor will receive an email with one or two attachments (based on the type of injury) with instructions on what needs to be done to finalize documentation of the event. 8
