Frequently Asked Questions Background Information What is UC Living Well? UC Living Well is a system-wide wellness initiative coordinated by the UC Office of the President. The goal of UC Living Well is to encourage all members of the UC community to lead and maintain a healthy lifestyle and to access the wellness activities and programs offered by UC campuses and medical centers. Who is Optum? Optum is a health and well-being company that designs and operates programs to help individuals get and stay healthy. Serving nearly 60 million people, Optum is one of the nation s largest health and well-being companies. Our team of health coaches, nutritionists, physical trainers, wellness consultants, online health experts and other professionals are here to provide you with guidance and support, tailored to your individual situation, to help you reach your health goals. Why should I participate? Making healthy choices is a personal decision that can have many positive impacts on your life. Whether you want to fit into your favorite pair of jeans again, or just be more active and eat healthier, manage your stress or finally give up a bad habit, we have a program that can help. We provide you with options so you can tailor your experience. Plus, participation can not only benefit your total health and wellness, but it can earn you a $75 prepaid Visa Rewards Card. Who is eligible to participate in the UC Living Well programs from Optum? Everyone is welcome to participate! Employees and retirees are welcome and encouraged to participate in UC Living Well programs. Spouses or domestic partners are welcome to participate in coaching programs online or on the phone but aren t eligible to receive the $75 gift card. Rewards What do I get for completing all of the activities? If you earn 7,500 reward points you ll earn a $75 prepaid Visa Rewards Card that can be used anywhere Visa is accepted. Programs and activities must be completed by Dec. 15 to earn the reward, but we suggest starting early to ensure you have enough time to finish Why is an incentive being offered? Healthy employees contribute to a healthy workplace. Your daily decisions can help you lead a healthier, happier, more fulfilling life. UC Living Well and Optum can help you reach goals to achieve that quality of life. How do I track my reward progress? No matter where you are on the Wellness Website, you can monitor your total rewards progress just look for the My Rewards point meters. For a complete picture of the healthy activities you ve completed, go to I Get > My Rewards > Earn Rewards. A check mark in the status column lets you know you ve completed the necessary requirements for that specific activity. If I don t see my incentives reflected on the Wellness Website, what should I do? If you are eligible to earn an incentive and completed an activity, but don t see the points earned on I Get > My Rewards > Earn Rewards, please note that some activities (such as telephone coaching) may take a few days to display your points. If you have questions, you may also email customer support by clicking on the link at the bottom of each page on the Wellness Website. We apologize for the inconvenience and appreciate your patience as your coaching progress is recorded in our systems. How do I redeem my points to receive the gift card? Once you have completed enough activities to earn 7,500 points, go to I Get > My Rewards. From the left-hand navigation, click My Rewards to open the menu and select Redeem Rewards to order your gift card. Select the $75 Visa Rewards Card, select Add to Cart and complete your order. When will I receive my Visa prepaid gift card? Your gift card order is processed and mailed within 10 days from the date you place your order. Please make sure your delivery information is correct to ensure your gift card arrives quickly. page 2
Wellness Activities How do I get started? Log on to the Wellness Website at uclivingwell.ucop.edu. The first time you use the Wellness Website, you ll need to register. You ll be asked to use the social security number of the UC employee who owns the benefit plan. This private number is only used to verify your identity when registering (or if you need to reset your password). Our identification process allows us to securely match you with your private health information. Your privacy and security are a top priority. We recognize that your personal information is sensitive. We take every precaution to ensure use of your information is secure and protected. If you have questions or concerns while registering, please email customer support by clicking on Customer Support Email on the Wellness Website registration page. If you prefer to talk to someone by phone, please call 1-877-818-5826. What if get an error message when registering for the Wellness Website? Check to ensure you entered all of your information accurately. If you have two first names, try using only the first one. If you re still having problems, use the link at the bottom of the page to email customer support or call us at 1-877-818-5826. What is the Total Health Profile? It s best to start your healthy journey by completing the Total Health Profile. This simple questionnaire takes only 15 minutes to complete and provides a personalized Total Health Profile report. It includes your Health Age, information about changes you could make to reduce your risks, helpful tips and actions you can take right away to improve. Your individual responses and results are kept strictly confidential and are never shared with anyone outside of Optum. Where do I find Total Health Profile? To complete your Total Health Profile and earn 5,000 points, go to uclivingwell.ucop.edu and log on to the Wellness Website. You ll find the Total Health Profile under I Am. page 3
How do I complete Online Coaching? Log on to the Optum Wellness Website at uclivingwell.ucop.edu. Once you ve entered the website, you ll find Online Wellness Coaching resources in the I DO section. Then, follow these easy steps: Step 1: Go to I Do to add an online program topic. If you ve completed the Total Health Profile, good job! Topics may already be added to I Do based on your conditions, risks and interests. To add more topics, simply click Add Program at the bottom of the left-hand column in I Do and pick one that appeals to you, or is perhaps recommended by a coach. Feel free to add as many topics as you wish to your I Do action plan. Step 2: Take action. Online Coaching includes articles, slideshows, videos, activity trackers, calculators and more to help support and guide you through your health journey. Start by reading the overview of the program you would like to join, and then click on an article or tracker to begin. When you re done, be sure to click the I have completed this button so your progress is recorded. Note: Some trackers do not have the I have completed this button; completion of the tracker itself is all that s needed to get credit. To check your progress, Progress Bars are visible from the homepage, the I Do landing page and the landing page of each Online Program topic. Step 3: Earn your Incentive. Complete a minimum of 12 Activities such as articles, tools and videos. Also, complete a minimum of 5 entries into any of the trackers associated with that topic for a minimum of five weeks from the day you added the topic to I Do. The first entry each week counts toward your progress. Entries do not need to be made in consecutive weeks, and if you miss a week, you have a 1-week grace period. How do I work with a wellness coach over the phone? Simply call to speak with a wellness coach at 1-855-688-9775. Or, you can enroll online and schedule a time for a wellness coach to call you. What can a wellness coach do for me? Working with a wellness coach makes wellness simple. You ll be assigned a wellness coach who will tailor the program to your individual needs. Coaching programs are available for the following wellness topics: Healthy Weight QuitPower tobacco cessation Nutrition Exercise Diabetes Health Heart Health Stress Management A Wellness Coaching program usually takes between two and three months to complete. To earn points for enrolling, you must schedule and complete your first call with the coach. To earn points for completing the program, you will work with your coach to set personalized milestones that you can meet. Once you and your coach have worked on and achieved those milestones, your coach will record your completion to earn the additional points. You are also welcome to continue working with your coach to reach new milestones. page 4
How I participate in an on-campus UC Living Well activity to earn points? Your UC campus or medical center offers a variety of health and wellness activities. Find out more about your specific campus at to http://uclivingwell.ucop.edu/campus_prog.html.then, watch for more information throughout the year to learn about the available activities you can participate in to earn points. After participating in an on-campus UC Living Well activity (or designated alternative), log in to the Wellness Website. Go to the I Do section and click on the On-Campus Activities program. From the left column, select UC Living Well Activity #1 (for your first activity) or UC Living Well Activity #2 (for your second activity) and click the I have completed this button. Be sure to keep a record of your participation for verification purposes if needed. Additional Questions Who can see my health care information? Your health care information can only be seen by you. Employee- (and spouse-/domestic partner-) specific information is not available to the University of California. UC Living Well and Optum programs are in full compliance with the privacy requirements under the Health Insurance Portability and Accountability Act (HIPAA). Is this program mandatory? No, participation is completely voluntary. We understand that making healthy choices is a personal decision that only you can make. These resources are here to make it easier for you to achieve your health goals. The rewards of a healthier life are clear: feeling better, performing better, preventing disease. We hope you participate not only for yourself, but for those you love. Participation in the Total Health Profile is strictly voluntary. Any health information collected as part of the assessment will be kept confidential in accordance with applicable law or your health plan s privacy policy and will only be used for health and wellness recommendations. The Wellness Coach program is administered by Optum on behalf of the University of California. The information provided through the Wellness Coach program is for informational purposes only. Wellness coaches and other representatives cannot diagnose problems or recommend specific treatment and are not a substitute for your doctor s care. The telephonic service should not be used for emergency or urgent care situations. In an emergency, call 911 or go to the nearest emergency room. Your personal health information is kept private in accordance with your plan s privacy policy. The Wellness Coach program is not an insurance program and may be discontinued at any time.
