This booklet contains a benefit eligibility summary followed by an overview of the benefits for CHI Mercy Medical Center employees. 3 Eligibility Employee eligibility may vary, based on the benefit. Note: All physicians are eligible to participate regardless of pay. n CHI 457(b) Plan An employee is eligible after 12 months of service if at least 1,250 hours are worked during that 12-month period. n Family and Medical Leave Act (FMLA) two-week pay period) or a part-time employee (regularly scheduled to work at least 40 hours per two-week pay period), begins to accrue paid time off immediately. 4 Health and Welfare Plans CHI Medical Plan Comprehensive medical and prescription drug coverage through a Blue Cross Blue Shield PPO plan with an extensive network of physicians, hospitals and pharmacies across the country. Three benefit options are available to meet employees specific needs. CHI may also contribute to the health savings account at its discretion. ** If an employee visits a CHI facility, the CHI facility coinsurance will apply to any services billed as a facility charge. The annual deductible does not apply to CHI facility coinsurance, except under the HD/HSA plan. *** Employees who fill a brand-name prescription when there is a generic equivalent available will pay the brand-name prescription coinsurance plus the difference between the generic and brand-name amount. Note: AD means after deductible. In-network benefit levels and costs are shown. Out-of-network services and prescriptions are covered at the applicable out-of-network benefit levels and costs. Preventive care is covered 100 percent. For most other services (including office visits), the Plan will pay a percentage of the cost. The CHI Medical Plan defines: n Deductible: the amount an employee pays for covered services before the Catholic Health Initiatives Medical Plan benefits subject to coinsurance begin. n Copay: a specified dollar amount an employee pays each time covered services subject to copays are received (i.e., ER visits). n Coinsurance: the percentage of an eligible charge an employee is required to pay when covered services subject to coinsurance are received. 5 Health and Welfare Plans Continued Healthy Spirit Wellness Program CHI supports the health and well-being of employees and their families through the Healthy Spirit Wellness Program. The Healthy Spirit Wellness Program, offered through our partner, Preventure, is a free, voluntary program that offers activities and tools to help employees (and their spouse or domestic partner, if enrolled in the CHI Medical Plan) reach their health goals and enjoy a personalized approach to maintaining or improving their well-being. The features offered include lifestyle coaching, disease/condition management, wellness workshops, health trackers and logs including activity tracking devices, fitness club discounts, challenges and more! Aside from enjoying the benefits of a healthy lifestyle, employees and their spouse or domestic partner enrolled in the CHI Medical Plan can also earn an incentive for participation in the Healthy Spirit Wellness Program. Progress toward earning the wellness incentive can also be tracked through Preventure. Lifestyle coaches are available to help employees manage their health in the areas of weight management, smoking cessation, dealing with stress, controlling health risks and more. Employees may work with a lifestyle coach to discuss their current habits, setting goals and boosting their motivation. The disease/condition management program helps employees, and their dependents who are enrolled in the CHI Medical Plan, to manage chronic conditions and improve their overall health and quality of life. The chronic conditions addressed through the program include asthma, COPD (emphysema or chronic bronchitis), cardiac conditions, depression, diabetes and musculoskeletal. Employees have access to a registered nurse who has extensive experience with the chronic condition. In partnership with the employee s physician, the nurse consults with the participant over the phone to assess the employee s health, review care, discuss other medical concerns and develop a holistic plan for improving the employee s overall well-being. These free, voluntary offerings are completely confidential and provide in-depth and personalized support to help employees enrolled in the CHI Medical Plan make the best possible health decisions. Note: Federal regulations prohibit employees not enrolled in their employer s health plan from participating in wellness activities that are clinical in nature, such as onsite biometrics screenings and lifestyle coaching/condition management services. Employees who are not enrolled in the CHI Medical Plan will no longer be able to participate in wellness activities provided through our CHI wellness partner, Preventure, or earn a wellness incentive tied to these activities. Because CHI is committed to building healthier communities and providing you with tools and resources to help you live a healthy life, you can still participate in local activities like wellness challenges, lunch and learns and fitness classes, as well as access tools and resources on the My Healthy Spirit website. CHI Dental Plan MetLife administers a comprehensive dental plan with open access to any dentist and orthodontist across the country. There are two options available to meet employees needs, which pay 100 percent for preventive and diagnostic services. Both options provide an orthodontic benefit and include a $50 individual/$150 family deductible. The PPO option includes a $1,500 annual maximum and an enhanced benefit is available if the employee visits a dentist in the MetLife network. The Core option includes a $1,000 annual maximum. Additional information about the Dental Plan, including a provider directory, is located on the MetLife website at Employees with access to HR/Payroll Connection may find more detailed information by logging into HR/Payroll Connection, clicking on the My Handbook tab, selecting Benefits & Time Off, selecting CHI Health Plans and selecting Dental Plan. 6 Health and Welfare Plans Continued Vision Plan A vision plan is provided to ensure optimal eye health. The vision plan is administered by EyeMed. An enhanced benefit is available if employees and their dependents receive care from a provider within the EyeMed network. When visiting an EyeMed network provider, you ll receive: Vision Plan Coverage Benefit Exam $15 copay; limited to one exam every 12 months Contact Lens Fit and Follow-up Up to $40 Lenses (single, lined bifocal and lined trifocal) $15 copay; limited to once every 12 months Frames $120 allowance, plus 20% discount available for out-of-pocket expenses; limited to once every 24 months Contact Lenses (allowance covers materials only) $105 allowance; conventional or disposable contact lenses in lieu of glasses; limited to once every 12 months Additional information about the Vision Plan, including a provider directory, is located on the EyeMed website at then choose SELECT from the provider locator drop-down box. You can also call EyeMed at CHI Flexible Spending Account Two flexible spending account options are offered to enable employees to use tax-free dollars to pay for out-of-pocket health care and dependent care expenses for themselves and individuals who qualify as dependents based on Internal Revenue Service requirements. Employees can deposit up to $2,500 in the health care flexible spending account and up to $5,000 in the dependent care flexible spending account. If you enroll in the HD/HSA medical plan option, you may enroll in a limited health care flexible spending account to help pay for eligible dental and vision expenses. Note: If you make $115,000 or more in base pay, the amount you can contribute to a dependent day care flexible spending account is limited to $3,000 to ensure your contributions remain tax free. Employee Basic Life and Accidental Death and Dismemberment (AD&D) Insurance Business Travel Accident Insurance Basic life and AD&D insurance protects an employee s loved ones from loss of income in the event of an employee s death. n Basic life and AD&D insurance is provided at two times an employee s base pay or $750,000, whichever is less. The combined plan maximum for basic and supplemental life is five times base pay or $750,000, whichever is less. n Business Travel Accident insurance is provided at three times an employee s base pay or $750,000, whichever is less. Basic life and AD&D insurance and Business Travel Accident insurance are available at no cost to the employee. Employee Supplemental Life and Accidental Death and Dismemberment (AD&D) Insurance Spouse and Child Life Insurance Employee supplemental life and AD&D insurance is available for purchase in increments of $10,000. The combined plan maximum for basic and supplemental life is five times an employee s base pay or $750,000, whichever is less. Life insurance for your spouse is available for purchase in increments of $10,000 up to $200,000. Life insurance for your child(ren) is available for purchase in increments of $2,000 up to $20,000. 7 Disability Plans Short-term Disability Short-term disability provides employees with replacement income for up to 26 weeks if sickness or injury prevents them from working their regularly scheduled hours. Short-term disability benefits begin after a seven consecutive day elimination period, up to 40 hours. Employees will then receive 60 percent of their base pay for up to 25 weeks. Long-term Disability The long-term disability plan provides a monthly benefit to full-time employees if they are continually disabled due to illness or injury beyond 26 weeks. The monthly payments are equal to 60 percent of an employee s base pay, up to a maximum of $10,000 per month. Note: The plan will not pay benefits for a pre-existing condition for which you received medical treatment within the three months prior to your long-term disability coverage becoming effective or, if it causes a disability, within 12 months after the effective date of your long-term disability coverage. Paid Time Off Paid Time Off (PTO) Paid time off is available to provide employees with time away from work for rest, relaxation and leisure, as well as to cope with short-term illness, to care for dependents or to fulfill other personal commitments. Below are the new hire PTO accrual levels based on 80 hours per pay period. Employee Status Hours Earned (annually) Days per Year Carryover Maximum Non-exempt % Exempt % Directors and Senior Leaders % PTO hours are accrued based on the hours worked in a pay period. The above table highlights PTO accrual levels for new employees. Tenured employees can check with their local Human Resources office for the complete PTO accrual schedule. Once an employee accrues the maximum amount of PTO, the employee will stop accruing PTO until the employee takes time off. Holidays Paid time off on designated holidays is provided to employees for a time of celebration or relaxation. 8 Retirement Program CHI 401(k) Retirement Savings Plan The CHI 401(k) Retirement Savings Plan enables employees to save for retirement now and provides a choice of: paying taxes now and withdrawing account balance later tax free, or deferring taxes until later when you withdraw your account balance at retirement. Employees can make pre-tax and Roth after-tax contributions to the plan, up to the annual IRS maximum. Employees who do not elect a contribution rate when first eligible will be automatically enrolled in the plan at a 4% contribution rate. Employees may choose to opt out of the plan, increase or decrease their contributions at any time. In addition, after meeting the 1,000 hours paid eligibility requirement, with each subsequent payroll, CHI will match an employee s contributions 100% on the first 1% of eligible pay and 50% on the next 5% of eligible pay an employee contributes to the plan for a maximum matching contribution of 3.5% of eligible pay if the employee saves at least 6% of their pay. CHI will also make an annual employer contribution equal to 2.5% of an employee s eligible compensation but no lower than a minimum contribution of $1,000, regardless of the contribution amount made by the employee. To receive the annual employer contribution, an employee must be eligible for the annual contribution portion of the plan, be paid for 1,000 hours in the calendar year and be employed on the last day of the year (December 31st). The 1,000 hour requirement has to be met each year a contribution is received. An employee with three years of service would be considered vested in the CHI 401(k) Plan for both Match and the Annual Employer Contribution. Employees with less than three years of service will become 100% vested in the employer contributions after three years of vesting service have been earned. An employee is always 100% vested in their own contributions. CHI 457(b) Plan The CHI 457(b) Plan is a tax-deferred savings plan that offers employees the opportunity to contribute additional salary deferrals over the above current contributions to the CHI 401(k) Retirement Savings Plan. It is recommended that employees invest in the CHI 457(b) Plan only after they contribute the maximum possible in the CHI 401(k) Retirement Savings Plan. It s important to understand that a 457(b) plan is a non-qualified deferred compensation plan under IRS regulations, meaning that employee contributions made to this plan are assets of the corporation and therefore subject to the risk of claims by creditors of the corporation in the case of any insolvency. Additional Benefit Options Tuition Assistance Tuition assistance is designed to encourage and assist employees to pursue educational objectives that will aid the employee in their current position or a future position. n Full-time employees have a maximum annual reimbursement of $1,500. n Part-time employees have a maximum annual reimbursement of $750. Employees who receive this benefit are obligated to maintain employment with CHI Mercy Medical Center for the following 12 months. Adoption Assistance Adoption assistance is designed to provide reimbursement to employees for some of the financial obligations related to adopting a child. n Full-time employees are eligible to receive up to $2,000 for each child being adopted. n Part-time employees are eligible to receive up to $1,000 for each child being adopted.
