Anaheim Ballet School s Summer Intensive 2016: Summer Dance Celebration Space is limited: In order to assure your place in the session, please complete these forms, print them out and return to us at your earliest convenience. Application Form (along with video submission) is due by March 26, 2016, but please submit at your earliest convenience. If accepted, please remit a minimum $200 non-refundable deposit by April 9 th to secure your placement in the program. The deposit will be applied toward your program fees. Payments may be made toward fees starting from the time of acceptance, however total balance is due no later than June 1 st. 2016 Four-Week Intensive Dates: June 27 th July 23 rd Ages: Ages 10-20, intermediate/advanced Tuition +Performance Fee: $1,650 Housing: $1,350-2,300 (see below) Orientation: June 27 th, 10:00 am orientation followed by day of classes Arrival/Check in: June 26 th Departure/Check out: July 24 th Tuition includes: all dance classes, lectures, master classes, rehearsals, performance, and supervision. Housing includes: meals, room, laundry, transportation, and supervision. *Please Note: The class schedule for June 27 th will be emailed to students/parents during the week prior. The class schedule for the full four-week program will be distributed at the June 27 th 10:00 am orientation. Audition Information Sunday February 21 st, 2016 Registration: 11:30 am Class: 12:00 noon 1:30 pm at Anaheim Ballet School 280 E. Lincoln Ave. Anaheim, CA 92805 Ages: 10-20, intermediate/advanced (girls, please bring pointe shoes) Fee: $20 for non-ab School students Please bring a dance photo in first arabesque and a short write-up of What dance means to me You are welcome to fill out and bring page 3 of the attached application Please pre-register for the class by calling 714-520-0904 or email info@anaheimballet.org Video Audition: If you are unable to attend the audition class, you may submit a video along with page 3 of the application as your audition. Video auditions should include barre work, center adagio, pirouettes, small jumps, grand allegro, and ladies please include pointe work. The video should not exceed ten minutes. You may either mail a DVD to our studio, or email a link (via YouTube or Vimeo) to an online video, but please include a completed copy of page 3 of the application in your package or email. In-person Audition: If you are local, but are unable to attend the scheduled audition, please contact ABS to make arrangements to attend a class taught by an AB director. Student Information Travel Anaheim Ballet is easily accessible from John Wayne Airport (SNA). Please attempt to arrange arrivals and departures from SNA. If unable to secure flights to/from SNA, flights should be arranged from Long Beach Airport (LGB), Ontario Airport (ONT) or from the Los Angeles International Airport (LAX). Please note on your application if transportation to and from the airport will be necessary. Housing Option 1 ($2,300) - Chaperoned Extended Stay Suite/Residential Housing: Students will be lodged in an extended stay suite at a Marriott hotel property in the Disneyland Resort Area (just three miles from AB), 1 of 5

featuring fully equipped kitchens and separate living area from sleeping areas. Each suite will house 2-3 students as well as an adult chaperone. All meals as well as transportation to and from Anaheim Ballet will be provided. Additional amenities include complimentary high-speed internet access, full breakfast buffet seven days a week, an on-site laundry facility, as well as outdoor pools and fitness center. Additionally, limited Residential Housing with students and adult chaperone may be available, please note if preferred. Option 2 ($1,350) Housing with AB Family (Limited Availability): Students will stay with a host family whose student is also participating in the Summer Dance Celebration program. Host family will provide meals and transportation. Note: If you would like information on arranging your own accommodations, contact VisitAnaheim.org Location Anaheim Ballet is located in an internationally favored vacation area that has many lovely accommodations. Lodging at local hotels provides easy access to the Anaheim Ballet studios. Parents wishing to join their children may request information for lodging by visiting www.visitanaheim.org. Anaheim Ballet is located near the Disneyland Resort, Anaheim Ice Hockey Rink, and Anaheim Packing District. Field Trips Optional group activities will include a trip to Disneyland The Happiest Place on Earth for an additional $89. Students under 13 will have a chaperone with them at all times at Disney. Additional no or low-cost social events may be available, please plan ahead accordingly (for example, beach bonfire or pool party). Supervision Supervision will include attention to ensuring that each participant receives the most from their experience by forming new friendships and gaining positive feelings of accomplishment and productivity. Dress Code Ballet Ladies: Black, pink, white leotards; pink tights; pink ballet shoes; pink pointe shoes; black, pink or white chiffon skirts are optional. Hair must be secured in ballet-style. Free style dressing is allowed during rehearsal periods at the discretion of the instructor. No jewelry may be worn in class. Gentlemen: White or black snug fitting T-shirts, black tights, black or white ballet shoes with coordinating socks. Free style dressing is allowed during rehearsal at the discretion of the instructor. Jazz, Hip Hop, Tap, Character Dance, Modern, Musical Theater & Performance Free style choice of leotards, tights, and T-shirts (no logos please) for contemporary dance styles. Bright plain colored T-shirts for performance. Dancers will also need black jazz shoes, sneakers, character shoes, tap shoes (character shoes may be used in place of tap shoes), black jazz pants, black class skirt for ladies, and a pair of denim/jean pants. Stage Make-up Neutral foundation (base), neutral powder (finish), earth-toned dark brown eye shadow, black eye liner, black mascara, earth-toned natural powder blush, earth-toned natural lipstick, earth-toned natural lip liner, white eye shadow, light brown eyebrow pencil (unless very dark brows), gray/brown contour powder. Brushes: large powder, medium blush, small eye shadow assortment. Street Wear & Incidentals Daily casual wear for to/from AB, warm jacket (evenings get cool), optional bathing suit. Students may find it helpful to bring funds for occasional movie outings or similar events, and for any personal purchases. Media Release I understand that photographers, videographers and/or television crews may sometimes be present photographing or filming Anaheim Ballet classes, rehearsals, coaching sessions or presentations. I give my permission for resulting photographs and/or television/film footage that may include myself/my child to be used for promotional purposes on television, newspapers, programs, magazines, or any other media.
