Customer Work Flow Order Procedure You must register for an account before you can purchase FES products (or apply to be a test center). FES Administrators will review and process all account requests within two business days. You will receive a notification once your account has been approved. Once approved, you may browse and purchase products from the FES store, as well as complete an application to become a test center. Voucher Management If a purchase includes test vouchers, you may manage your vouchers via the website. Once the order is fully processed you may assign vouchers and print a.pdf of the voucher document. Test takers are allowed to purchase three re-test vouchers before the purchase of a new voucher is required. Test Centers Programs can request to become a test center. FES Store Administrators will review and must approve all test center requests before the user may proceed with any Test Center Activity. Section 1: Product Orders (Submission, Processing, Fulfillment) 1.1 Account Registration If you are new to the system you will be required to register a new Customer Account before proceeding to the ordering process. A valid email address must be entered as the user name. How to Register an Account 1.1.1 Go to Store Address: www.surgicalfundamentals.org - 2
1.1.2 Select Login from the top menu. 1.1.3 Select the Continue button in the I am a new customer box. 1.1.4 You must complete all required information. - 3
1.1.5 After providing all required information, and reviewing and agreeing to the site Terms and Conditions select Register to complete the registration. 1.1.6 An Account Created Confirmation will appear on screen. 1.1.7 Please review the What happens next text. Requests will be processed within the next two business days. A confirmation message will be sent to you via the email address you provided. 1.2 Submitting an Order (Purchasing Available Products) Once your account is active you may return to the store and purchase products accordingly. - 4
How to Submit an Order 1.2.1 Go to Store Address: www.surgicalfundamentals.org. 1.2.2 When the store s Product Catalog appears on screen, select the Login link from the top menu. 1.2.3 When the Account Login window appears on screen, enter your account information in the I am a returning customer box and select Login. 1.2.4 From the My Orders page, you can view all products available for purchase by selecting Products from the top menu. - 5
1.2.5 From the Product Catalog page, you can select items you wish to purchase by selecting Add to Cart. If you ve received a Coupon Code enter it before selecting Add to Cart 1.2.6 When the Shopping Cart window appears, you have the option to Update, Continue Shopping or Check Out. - 6
1.2.7 When you are ready, select Checkout. 1.2.8 When the Checkout: Shipping window appears on screen, you have the option to select the shipping address already on file for the master account or enter a new address. 1.2.9 If you wish to use the main shipping address on file select Continue from the Address Book box. 1.2.10 If you select to enter a new address, complete all of the required information in the New Address box and select Submit. The address will then be saved under Address Book. You will then select the address from the options saved and select Continue. - 7
1.2.11 Once the proper shipping information has been entered and the Checkout: Payment window appears on screen, you may select your preferred payment option. If you select Purchase Order you will be required to upload the actual purchase order before the order can be fully processed. Presently, only.doc,.docx and.pdf file types are supported. 1.2.12 After selecting your payment option, you will need to select the billing address. You will have the option to select the address already on file for the master account or enter a new address. 1.2.13 If you wish to use the main billing address on file, select Continue from the Payment/Address box. - 8
1.2.14 If you wish to enter a new address, complete all of the relevant information in the New Address box and select Submit. The address will then be saved under Address Book, and can then select the address from the options saved and select Continue. 1.2.15 Once the proper payment information has been entered, confirm your Order Details and Shipping/Billing Information when the Checkout: Review and Submit window appears on screen. 1.2.16 Once the order information has been confirmed, scroll down the page to complete the payment process. The payment section will vary depending on your selected payment option. - 9
1.2.16.1 If the Credit Card payment option was selected, you will need to fill in the proper credit card info and select Submit Order. 1.2.16.2 If the Purchase Order payment option was selected, you will be able to upload a copy of the purchase order and then select Submit Order. You may proceed with the order without uploading the purchase order but the order will be placed on hold and will not be fully processed until the Fundamentals Staff has received the purchase order. - 10
1.2.17 Once the order has been submitted, the Order Details confirmation window will appear on screen. 1.2.18 You may also view your order details at any time by logging in and selecting My Orders from the My Account menu on the right side of the screen. If you need to provide proof of payment for reimbursement, you may print your My Order status page or Invoices at any time. Re-Test Vouchers must be purchased by the account holder who purchased the original voucher. If you are attempting to purchase a Re-Test voucher, you must select the initial voucher number/test Taker. If the original test voucher is not present the customer should contact the Fundamentals Store Admin at info@fesprogram.org for assistance. - 11
1.3 Viewing Fulfillment Letters How to View Fulfillment Letters 1.3.1 Log into the store account as previously described. 1.3.2 Select your My Order page to see your order(s) listed. Then if multiple orders are listed, select the order you wish to view. 1.3.3 The Order Details page will appear on screen. Select View Fulfillment Letter in the top right corner. - 12
1.3.4 When the.pdf of the Fulfillment Letter appears in a new window, customers may print the letter from here or save a copy to their local hard drive. The letter can be generated from your account at any time. You may need to adjust your printer settings to print in the appropriate orientation. Section 2: Customer Voucher Management Once your order has been fully processed, you may log into your account and perform the following functions: o View number of available vouchers o Assign test taker to a particular voucher o Generate Testing Certificate o View/Print Invoice and/or Receipt 2.1 Viewing of Available Vouchers How to View Available Vouchers 2.1.1 Log into the store account as previously described. - 13
2.1.2 Select My Voucher Packages from the My Account menu. 2.1.3 The My Vouchers window will appear on screen displaying all of the vouchers that are associated with a voucher and indicate how many of the associated vouchers are available or have been assigned in addition to the expiration date. 2.1.4 If you don t see the package you are looking for you may change the display range by selecting from the available options in the drop down menu. - 14
You can view Assigned or Unassigned Vouchers by selecting the appropriate option from the My Account menu. 2.2 Assigning Vouchers How to Assign a Test Voucher & Generate a Test Voucher 2.2.1 Log into the store account as previously described. 2.2.2 Select My Unassigned Vouchers from the My Account menu. - 15
2.2.3 When the My Unassigned Vouchers window appears on screen, select the Voucher Package for the voucher you wish to assign. 2.2.4 Review the Voucher Assignment window when it appears on screen and take note of the warning label in red font. If there is more than one voucher to assign, you will have to fill out each one individually. If you try to fill in all the fields it will only save one record at a time. Once a voucher has been assigned to a test taker by the program it can t be changed or reassigned. If the originally assigned test taker does not redeem the voucher it will expire and go unused. 2.2.5 Fill in all of the required information (First Name, Last Name, and Email) and select Assign. - 16
2.2.6 If you are sure you wish to assign the voucher, select OK to proceed when the onscreen confirmation appears 2.2.7 The information just entered for the test taker will appear at the bottom when Voucher has been Assigned confirmation will appears. 2.2.8 Click on the Voucher No. link to view the Test Voucher. 2.2.9 A.pdf of the assigned Test Voucher will appear in a new window. - 17
Test Takers must bring this voucher with them to the test center. They will not be able to test otherwise. If needed, test vouchers can be reprinted at any time. You may need to adjust your printer settings to print in the appropriate orientation. Section 3: Test Center Management 3.1 Test Center Requests How to submit a Test Center Request 3.1.1 Log into the FES Store Account: www.surgicalfundamentals.org 3.1.2 Select Test Center Request from the My Account menu. - 18
3.1.3 Complete all of the required information and review and agree to the Test Center Policies and Select Submit. 3.1.4 Your application was received if the Submission Confirmation appears onscreen. - 19
