PR795-735-14-A001 Page 4 of 19 General Equipment Preventive Maintenance Guidelines Preventive Maintenance services at 201 Granby Street for NRHA. 1. Provide four (4) inspections per year; quarterly to check, lubricate, and adjust equipment listed in Attachment D. 2. Provide, as needed, filter changes. Filters provided by NRHA. 3. Evaporator and condenser coils shall be cleaned, as needed, once every other year. 4. Replace belts as needed. Belts shall be provided by Contractor as a part of the preventative maintenance program and cost thereof. 5. Change associated equipment from heating to cooling in the spring and from cooling to heating in the fall as directed by the Senior Building Maintenance Superintendent. 6. Every odd numbered year, tear down the boilers for insurance inspection. After inspection, place boilers back into serviceable condition, ready for heating season. Boilers shall be restored completely to manufacturer s specifications, new gaskets shall be provided and installed and any missing or unserviceable fasteners shall be replaced b. Within ten days of completion of the initial service of the HVAC equipment for NRHA and quarterly thereafter, Contractor shall submit to NRHA a written report. This report shall identify the building name or number, the date(s) service was performed, and the following additional information on each piece of HVAC equipment: 1) A general statement as to the condition of the equipment. 2) Specific recommendations for any major repair or modification which will enhance the operation of this equipment to include an estimate of the number of man-hours and cost to accomplish the work recommended. c. The approved PM schedule shall be posted on a check chart maintained for that purpose in the Senior Building Maintenance Superintendent s office. In addition to the maintenance schedule, this chart shall show: 1) Date scheduled PM was last performed. 2) Uncorrected deficiencies, reasons for deferring and any other information NRHA may need to know in the REMARKS column. d. PM service listed in the approved PM schedule shall be performed as part of the fixed cost for PM service. IV-3. On-Call Repairs a. In the event of a system failure, which may not constitute an emergency, the Contractor shall be on site to perform on-call repair service within two (2) hours of notification as provided by NRHA.
PR795-735-14-A001 Page 5 of 19 failure to properly perform preventive maintenance or repair services. In this case, such repairs shall be made at no cost to NRHA. c. On-call repairs authorized by NRHA which may be requested to be performed after normal work hours shall be paid at the overtime hourly rates described in Attachment C, Pricing Schedule. If regular time work must be carried over and the Contractor wishes to continue to work beyond NRHA s normal working hours, authorization for overtime work must be obtained from NRHA before proceeding. d. NRHA reserves the right to competitively bid replacement parts or source directly from the Contractor. e. Any repair and/or replacement part performed/installed by another Contractor shall remain the responsibility of the repairing or installing Contractor until the end of the OEM Warranty for the repaired or installed equipment or component. At the end of that period the Contractor under this service contract shall assume responsibility for preventative maintenance and repair service of that piece of equipment or component. IV-4. Emergency Repairs a. The Contractor shall be on site to perform emergency repairs within one (1) hour of receiving a call from NRHA. b. All emergency repair work shall be performed on a time and material basis unless it is determined that the cause of the failure or malfunction was due to the Contractor s failure to properly perform preventive maintenance or repair services. In this case, such repairs shall be made at no cost to NRHA. c. Emergency repairs authorized by NRHA which occur after normal work hours shall be paid at the overtime hourly rates described in Attachment C, Pricing Schedule. If regular time work must be carried over and the contractor wishes to continue to work beyond NRHA s normal working hours, authorization for overtime work must be obtained from NRHA before proceeding. IV-5. Work Hours a. Normal work hours for NRHA are from 8:00 a.m. to 5:00 p.m., Monday through Friday, except holidays. All preventive maintenance service, on-call and emergency repair services, whenever possible, shall be performed during normal working hours. IV-6. Contractor s General Procedures a. The Contractor s employees shall comply with the following procedures: 1) Check in and out with NRHA s designated representative during normal work hours. If work is to be performed on a time and material basis, NRHA s representative shall record the date(s) and time(s) the Contractor s employees arrive and leave the building. Lunch breaks and other extended periods of time away from the job site shall be accounted for in the log. There will be no truck charge or trip charges under this contract.
