Silver Plan AGREEMENT Priority Emergency Plumbing / Heating Repair Service: All service labor will be billed at our prevailing labor rate with no discount. Gold Plan-5 All labor will be billed at our prevailing labor rate less 5%, (see terms and conditions) All parts will be billed with a 5% discount, (see terms and conditions). Plus Water Conditioner Equipment Check-up Add an Annual check-up of your Water Conditioner Equipment. o No Culligan equipment please. 1
2 Terms and Conditions Regular hours of service: 8:00 am to 4:30 pm Monday thru Friday billed at prevailing service rate. o After 4:30 pm Daily, Saturday or Sunday any service call will be billed at one and a half times the prevailing service rate. o All Holidays, any service call will be billed at two times the prevailing service rate. Service plan applies to all plumbing & heating work. o CompleteHeat* service tune-up add-on of your oil fired boiler / furnace or gas fired boiler / furnace or gas fired space heater can be added to your service agreement and will be billed at the add-on price. (See page 4 of service agreement) o Additional oil or gas boiler / furnace units or oil fired water heaters or gas fired space heaters can be added to your service agreement and will be billed at the add-on price. (See page 4 of service agreement) Service plan becomes effective upon payment of service plan in full and approved by SMITH. o Repair of existing problems over and above normal cleaning of a boiler / furnace will be billed at customers expense. (eg: replacement of a zone valve, thermostat, circulator pump). An emergency is defined as No Heat, No Water or any other service threatening harm to people or property. Service plan is effective for one calendar year. (January 1 st to December 31 st ) o Pro-rated Service Plans are available for late enrollees on the 1 st year only, but will not be less than 75% of the plan chosen. o Add-on s are not Pro-rated. Discounts: o On labor, only applies to work done on a Time and material basis and does not apply to written proposals. o Labor time is calculated portal to portal, travel time, to and from the job will be included in the labor charge. o On parts, only applies to work done on a Time and material basis and does not apply to written proposals. o Service Plan discounts can not be combined with any other discounts or promotions that we may offer. Your annual CompleteHeat* tune-up, if you elect to have one, should be scheduled between March 1 st and August 30 th. Your annual CompleteCool** tune-up should be scheduled between April 15 th and June 30 th. Your annual Water Conditioner Equipment*** check-up will be scheduled between April 15 th and June 30 th. Service plans cover single-family residential customers only. o Commercial accounts or multi-family rental unit with standard Residential boilers or furnaces can extend coverage by choosing the Commercial Plan and then adding the cost of the additional heating or cooling units. o Commercial accounts with large Commercial boilers, furnaces or air conditioning equipment must call for special Service Plan quotes. Note: Customers who elect not to purchase a service plan: Will be billed at our prevailing service labor rate with no discount. Will be billed for all parts with no additional discount. Will not receive the (Smith s Exclusive CompleteHeat* Service Tune-up), but most of our Add-on services can be purchased separately at the prevailing rate. (call office for pricing) Will not receive priority dispatch. 2

Smith s Exclusive CompleteHeat* Service Tune-up Light cleaning of your oil or gas boiler / furnace, including heat exchanger, to remove old soot and carbon deposits. Replace oil pump strainer, nozzle and oil filter element. Replace air filter. Lubricate blower motor or burner motor and circulators. Lubricate blower wheels and bearings. Adjust main pilot burners for efficiency and fuel economy. Inspect flue pipe and chimney for blockage. Cleaning of electrodes, air tube, fan and fan housing. Test aquastat limit, fill valve, gas valve. Complete inspection of ignition components, thermocouple, cad cell, controls, thermostats, transformers and oil piping in basement. Smith s Exclusive CompleteCool** Service tune-up (also pertains to ductless A/C equipment, see separate add-ons) Check your condenser coil and evaporator coil. Clean evaporator drain to remove old deposits. Adjust main thermostat for comfort and economy. Check condenser fan motor, start and run capacitors, contactor, fuses, relays, compressor, line-set, valves, transformer, fan motor & wheel and wiring. Check refrigerant charge. Check refrigerant line insulation. Replace air filter. Smith s Exclusive Water Conditioner Equipment*** Check-up Check injectors & backwash screens. Check timer/clock. Check drain line for obstructions. Check salt level. Check brine draw & refill. Check level of media. Water sample analysis tested by Master Water Conditioner Corp. Determine corrective measures if required based on new water analysis. 3
