Objective To provide a step-by-step overview of the registration process to begin training in residency programs to new Canadian and IMG trainees, Program Administrators, and Program Directors. Please note that the requirements indicated are the PGME mandatory requirements and that individual hospital sites have additional mandatory requirements (e.g. TB testing, modules). It is up to the resident to ensure they have also completed all mandatory hospital requirements. This overview accompanies your registration kit. Please have your registration email and supplemental information available.
Registration Process Trainee must complete all steps in order to be able to start training on July 1st: 1.1 Go to Registration website and enter username and password provided 2.1 Sign into Registration website 2.2 Submit eloa 3.1 Verify accuracy of information on registration form 3.2 Submit electronic registration form 4.1 Apply online on CPSO website 5.1 Apply online on CMPA website 6.1 Pay uottawa registration fees online 7.1 Submit immunization results and send to Office of Risk Management (ORM) 7.2 Submit proof of application for VSS and final results, once received, to ORM 8.1 Set up uottawa email account following instructions from MedTech 9.1 Complete e-learning Module: Preventing Mistreatment of Students
1. Access 1. Go to Registration email. 2. Click on Registration to access Registration website. 3. Copy and paste username and password.
2. eloa 1. From Postgraduate Trainee Portal, select Forms/Letter of Appointment(LOA). 2. Verify for accuracy and submit eloa (print if desired).
3. Electronic registration form (ereg) 1. From Postgraduate Trainee Portal, select Forms/Registration Form. 2. Verify for accuracy and submit ereg.
4. CPSO Refer to Supplemental Information in Registration email 1. Complete CPSO on-line application form. 2. Clinical Fellows: Renew educational license or apply for an Independent Educational License.
5. CMPA Refer to Supplemental Information in Registration email 1. Complete CMPA on-line application form; note that your CPSO number can be provided later when available. 2. Complete debit authorization.
6. uottawa Registration fee Refer to Supplemental Information in Registration email 1. Pay on-line at financial institution using account number (student #). 2. International bank-to-bank transfer (see slide Where to get help ).
7.1 Immunization Refer to Supplemental Information in Registration email 1. Click on documentation and select Program Requirements and deadlines/postgraduate Medical Education. 2. Complete the Clinical Placement Requirements Record.
7.2 VSS Refer to Supplemental Information in Registration email 1. From Program Requirements and deadlines/pgme, complete Police Record Check/VSS. 2. Send proof of application and results to ORM. 3. Trainees living outside Canada: VSS must be obtained before leaving home country.
8. Activate/configure/forward uottawa email account Refer to Supplemental Information in Registration email 1. Instructions including username and password to be sent from the Faculty of Medicine Information Management Service (Medtech). 2. Access though uozone/infoweb account.
9. e-learning Module: Preventing the Mistreatment of Students Refer to Supplemental Information in Registration email Wait until you receive an email from Medtech informing you of your username and password. 1. To access the module, please follow this link: https://learn.med.uottawa.ca/courses/preventing-themistreatment-of-students/ 2. Click Log in and take this Course 3. On the login page, please choose I m affiliated with the Faculty of Medicine 4. Enter your Faculty of Medicine / One45 account information and click Sign in. You will be taken to the module where you will be able to start it or continue it where you left off last time.

Important Reminder You must complete all the mandatory requirements in order to start training on July 1 st. Delays in training cause disruptions in scheduling and ultimately affect patient care.
