2 QUALIFICATIONS: Required Education or formal training: Bachelor s degree in computer science, informatics, biomedical sciences, epidemiology, biostatistics, business or other relevant field of study. Experience: Three years professional experience with data analysis and reporting including two within a healthcare setting. Post-graduate education/training in a relevant field may count towards experience. Knowledge, Skills and Abilities: Communications o Excellent skills in communication; able to listen and understand what both medical subject matter experts and end-users need and how they intend to use data and reports; ability to explain analytics to those same people in a way that gets the point across. Effective, clear and accurate writing, data graphing and data visualization. Technical o Thorough knowledge of computer systems and the ability to effectively utilize software applications including spreadsheet development, database development and management, report writers, visualization and presentation software. o Intermediate programing and computational skills. o Working knowledge of relational database models, proficiency in writing SQL queries, and proficiency with analytical/statistical modeling tools. Clinical o Familiarity with the business of healthcare, from both a clinical operations and a financial perspective. o Familiarity with electronic health records and healthcare administrative/claims data. Quality Improvement o Familiarity with standard approaches to quality improvement (Six Sigma, Lean, PDSA) and the specific types of visualizations and other analyses common with these methodologies. o Skilled in team and group facilitation. Analytical o Ability to think critically and analytically; exceptional problem solving skills. o Data-centered; passionate about evidence-based problem solving. o Conversant in statistical concepts.

Ability to leverage care management information systems and databases in creative ways to support population health improvement.

4 REPORTS TO: Medical Director GENERAL PURPOSE: The Colorado Medicaid Accountable Care Collaborative (MACC) program was created in 2011 to improve the health of Medicaid enrollees while containing health care costs using a framework of Regional Collaborative Care Organizations (RCCOs), Primary Care Medical Providers (PCMPs) and a Statewide Data and Analytics Contractor (SDAC). Data is used to identify populations with more complex medical needs, and to track RCCO and PCMP performance on key performance measures, initially targeting emergency room visits, hospital readmissions and high-cost imaging. The Northern Larimer County MACC, is a partnership between Rocky Mountain Health Plan (the RCCO), primary care practices, the local mental health center, and the local hospital, with a focus on hot-spotting the highest need Medicaid enrollees and providing them with community-based case management services in coordination with their primary care providers. The position is being created to provide strategic and tactical support to the primary care practices and the MACC care management team to meet current key performance measures while broadening the scope of population health management in anticipation of Colorado s ACC s Phase II iteration. GENERAL DUTIES: The Analyst will help clinicians make and execute rational decisions about the care of their Medicaid patient panel by mining, analyzing, and creating intelligible, meaningful and actionable reports across multiple sources of data. This position works with MACC and practice leadership, quality improvement and care teams to identify the data and analysis needed to plan, execute and monitor initiatives to improve quality and reduce cost of care, and is accountable for increasing their customer's understanding and use of data. SPECIFIC DUTIES: 1 1) Acquire a clear understanding of the goals and needs of each primary care practice regarding population health management of their Medicaid-insured panel. Elicit and document what each of the analytics stakeholders will require and develops approaches to assure their needs are met. 2) Develop, document and implement an overall analytics strategy for each primary care practice in consultation with MACC and practice leadership, IT and data analytics managers and other resources. The analytics strategy will identify scope, vision, goals, current targets, specific improvement approaches, key stakeholders and users of analytics, analytics use cases, potential sources of data, data models, an analysis of data gaps, and existing systems for data integration, existing analytics tools, and what analytics support is available. Update analytics strategies as requirements or environments change. 3) Use practice-specific population reports from the Regional Care Collaborative Organization (Rocky Mountain Health Plan) to create useful and actionable information for clinicians and care managers. 1 Will depend on data and analysis infrastructure and needs of each primary care practice.

5 4) Unlock, understand, and use what is available in practice EMRs for supporting selected population health improvement interventions. 5) Aggregate patient data from multiple sources (e.g., practice EMRs, hospital discharge data, claims data, patient reported data) to create patient registries. 6) Utilize evidence-based algorithms and data mining to identify patients within provider panels who require particular services (e.g., sick enough to need care management support, needs reminders for preventive care, not meeting management goals, failed to receive follow-up, might benefit from risk reduction, etc.) and update frequently. 7) Stratify and prioritize patients based on risk evaluation algorithms. Create analytical capabilities and reports that allow care teams to drill down to the data needed about cohorts and individual patients. 8) Design reports and dashboards specific to quality improvement approaches used by practices (e.g., Lean, Six Sigma, and PDSA) to support MACC QI initiatives. 9) Meet periodically with MACC Oversight Committee, MACC Administrative Team, and practice data analytics and quality improvement teams to present reports, elicit data analytics needs and update analytics strategy. 10) Create data analytics reports and dashboards to monitor improvement initiatives and evaluate performance, processes and outcome objectives of the Northern Larimer County Medicaid Accountable Collaborative.
