1 Pharmacy Practice Residency Preceptors Vincent A. Lacroce, Pharm.D, Vince has been on staff at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center since January of 1985 in various capacities. Currently, he is the Administrative Director of Pharmacy and has been involved in the residency program since its inception in Vince received a B.S. in Pharmacy from Temple University in 1979 and a Pharm.D. from the University of Maryland in Vince is an active member of the Pennsylvania Society of Health- System Pharmacists and ASHP. He received the Roche Preceptor of the Year Award from Temple University in Lisa A. Braccini-Barletta, MBA, R.Ph, is a 1986 graduate of the Philadelphia College of Pharmacy and Science. Lisa completed a General Practice Residency at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center in June She has held a variety of leadership positions at the Medical Center. In January 1992 she graduated with her Master in Business Administration from Lebanon Valley College and in October 2010, was named to her current position as Director of Pharmacy, Outpatient Services. Lisa's areas of responsibility include the Anticoagulation Clinics, the Chemotherapy and Infusion Room Pharmacy, the Investigational Drug Service, the Medication Management Clinic, the Prescription Assistance Program and the Retail Pharmacy operations including the Employee Prescription Drug Program. Lisa maintains several professional memberships and has held numerous leadership positions in several organizations. Dwayne Gallagher, Pharm.D, is a graduate of the Philadelphia College of Pharmacy. He completed an ASHP-accredited pharmacy practice residency at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center in 1998, and stayed on as a clinical specialist in adult critical care. In 2002, he transitioned into his current position of Pharmacy Informatics Specialist. His professional interests include meds integration, troubleshooting, writing queries, and teaching. Cynthia Halas, Pharm.D, received both a Bachelor of Science and Doctorate of Pharmacy degree from Duquesne University School of Pharmacy, Pittsburgh, PA. She completed an ASHP-accredited Hospital Practice Residency at Mercy Hospital, Pittsburgh, PA. Cindy is one of the adult critical care clinical specialists practicing in the Neuroscience Critical Care Unit. Her areas of interest are neurosurgery, neurology, and nutrition.
2 Michele Haydo, Pharm.D, BCACP, received a Doctor of Pharmacy degree from Duquesne University. She completed an ASHP-accredited Pharmacy Practice Residency at the Lebanon VA Medical Center. Currently, Michele works at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center as a clinical pharmacy specialist in the medical group involved with care management of heart failure patients. Frank Herrmann, R.Ph, is a 1994 graduate of the Philadelphia College of Pharmacy and Science. After participating in hospital pharmacy for several years, Frank transitioned to ambulatory care in Since that time, he has been a full-time pharmacist at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center in the Anticoagulation Clinic. Under his guidance, the Clinic has experienced significant growth with the current patient load of more than 1500 patients and a staff of 8. The Anticoagulation Clinic is a past recipient of the Bristol-Myers Squibb Heart First Clinic Recognition Award. Sallie Johnson, Pharm.D., BCPS-AQ Cardiology, graduated from the University of the Sciences in Philadelphia (USP) with a Doctor of Pharmacy degree. Following graduation, she completed an ASHP-accredited Pharmacy Practice Residency at Mercy Suburban Hospital in Norristown, Pennsylvania as well as a specialized residency in Cardiovascular Pharmacy Practice at USP, with practice sites at University of Pennsylvania Health- System and the Philadelphia VA Hospital. Sallie is currently a Clinical Pharmacy Specialist in Cardiology at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center. She is a member of the Southeastern chapter of the Pennsylvania Society of Health-System Pharmacists Association, where she is a Past President. Sallie serves as the Pharmacotherapy Editor for the American Heart Association Guidelines for CPR and ECC. Her primary interests include acute coronary syndrome, chronic heart failure and heparin-induced thrombocytopenia. Kimberly Keefer, Pharm.D, BCPS, graduated from Temple University School of Pharmacy in She completed a Pharmacy Practice Residency at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center in She currently works at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center as the Medical Intensive Care Unit Clinical Specialist. Dr. Keefer's practice interests in the ICU include: geriatrics, infectious disease and medication safety. Jihye Kim, Pharm.D, BCPS, received a Doctor of Pharmacy degree from the University of Wyoming School of Pharmacy. She completed an ASHPaccredited Pharmacy Practice Residency at the University of New Mexico Hospitals in Albuquerque, New Mexico and an Infectious Diseases Pharmacy Residency at the University of California Davis Medical Center in Sacramento, California Areas of interest include antimicrobial stewardship, microbial resistance mechanisms, and opportunistic infections in immunosuppressed patients.

Lay, MS, R.Ph, FASHP received a B.S. in Pharmacy from the University of Maryland in 1978 and an M.S. from the Ohio State University in Ron completed an ASHP accredited residency at the Ohio State University Hospital in Prior to joining the staff at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center, Ron worked as a Director of Pharmacy at Humana Hospital in Cypress, Florida and Good Samaritan Hospital in Lebanon, PA and as a Pharmacy supervisor at St. Joseph Hospital in Lancaster and the Pinnacle Health System in Harrisburg. Ron is an active member of the Pennsylvania Society of Health-System Pharmacists, where he held the position of President. He also participates in a number of community and church activities and was the recipient of the PSHP Community Service Award in Jennifer Morrow, Pharm.D, graduated from the University of Rhode Island in She completed her pharmacy practice residency at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center. Dr. Morrow currently works as a clinical pharmacist primarily in the Heart and Vascular Institute. Her areas of interest include critical care and cardiothoracic surgery. Kevin M. Mulieri, PharmD, BCPPS, is a graduate of the Ernest Mario School of Pharmacy at Rutgers, The State University of New Jersey. He completed residencies in Pharmacy Practice and Pediatric Pharmacy Practice at the Medical University of South Carolina. Kevin has been the Clinical Pharmacy Specialist in Pediatric Hematology/Oncology at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center since July of Kevin is an active member of the Children s Oncology Group where he serves on multiple committees. His primary interests include the care of pediatric patients with oncological disorders, pain management and palliative care. Ashley L. Quintili, PharmD, BCPS, received her Doctor of Pharmacy degree from the University of Pittsburgh School of Pharmacy, Pittsburgh, PA. She completed her Pharmacy Practice Residency and Critical Care Residency at Vanderbilt University, Nashville, TN. Ashley transitioned to Hershey from the University of Maryland Medical Center where she was the Neurocritical care clinical specialist and the Coordinator for Formulary Management. Currently, she is the Surgical/Anesthesia Clinical Pharmacy Specialist at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center where she also serves as the Assistant Residency Program Director for the PGY1 Pharmacy Practice Residency. Areas of interest include infectious disease, pain management, sepsis, and anticoagulation/reversal. Catherine E. Rejrat, PharmD, BCPS, received her Doctor of Pharmacy degree from the Philadelphia College of Pharmacy in Philadelphia, PA. She completed a Pharmacy Practice Residency at NorthShore University Health System Glenbrook Hospital in Glenview, Illinois and a Pediatric Pharmacy Residency at Helen DeVos Children s Hospital in Grand Rapids, Michigan. She is currently the Neonatal Intensive Care Unit Clinical Pharmacy Specialist at PSHMC.

She completed a Pharmacy Practice Residency at Detroit Receiving Hospital and University Health Center in Michigan followed by a Critical Care Specialty Residency at the University of Colorado School of Pharmacy and University of Colorado Hospital in Denver. She subsequently completed a Critical Care/Infectious Diseases Fellowship at the Department of Pharmacy Practice, University of Nebraska Medical Center in Omaha. She also completed her MBA in healthcare administration from the University of Scranton in Jill served as PGY2 Critical Care Residency Program Director, Associate Professor of Surgery, and critical care pharmacotherapy specialist at the University of Vermont College of Medicine and Medical Center in Burlington, Vermont from She subsequently served as PGY1 Residency Program Director and held various leadership positions at Lancaster General Health in Lancaster, PA for 8 years before transitioning to PSHMC. Jeffrey Sivik, PharmD, BCOP, graduated from the University of Pittsburgh with a Doctor of Pharmacy degree in He completed an ASHPaccredited Pharmacy Practice Residency at Johns Hopkins Hospital in Baltimore, MD. and completed an ASHP-accredited Oncology Specialty Residency at the H. Lee Moffitt Cancer Center & Research Institute in Tampa, FL. Jeff is currently a clinical pharmacy specialist in Adult Oncology at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center and is an instructor of pharmacology at Penn State College of Medicine. He is an active member of the Hematology/Oncology Pharmacy Association (HOPA) and the American Society of Clinical Oncology (ASCO). His primary interests focus on bone marrow transplantation/malignant hematology and the supportive care of these patients. Lindsay C. Trout, PharmD, BCPPS is a 2006 graduate of the Ernest Mario School of Pharmacy at Rutgers, The State University of New Jersey. She completed a Pharmacy Practice Residency at Temple University Hospital and a Pediatric Pharmacy Practice residency at University of the Sciences with a practice site at the Children s Regional Hospital at Cooper University Hospital. She is the solid organ transplant clinical pharmacy specialist at Pennsylvania State University Hospital, the Milton S. Hershey Medical Center. Her primary practice interests include: kidney, liver, and pancreas transplant and transplant related infections. Lisa Hanlon Wilhelm, BS Pharm, R.Ph, obtained her Pharmacy degree from the University of Pittsburgh in 1994 and completed the ASHP Executive Residency in Association Management in Lisa coordinates Medication Safety and Regulatory activities at PSHMC. She has previous clinical experience in Infectious Diseases, Home Health Care, and the Anticoagulation Clinic.
