1 Denise Brooks-Williams President & CEO Henry Ford Wyandotte Hospital A native of Michigan, Denise Brooks Williams received her bachelors and masters degrees in health services administration from the University of Michigan before starting her career as a management fellow at Mercy Hospital in Detroit. She has been active in numerous professional groups including serving as president of the National Association of Health Services Executives (NAHSE), one of the premier minority health professional organizations in the United States. In 2013, Brooks-Williams became president and CEO of Henry Ford Wyandotte Hospital. This 401-bed acute care hospital serves the downriver region and surrounding communities of southeast Michigan with approximately 2,400 employees, 600 medical staff members, including Henry Ford Health Center-Brownstown outpatient facility and physician practices. Brooks-Williams is active in several community- and charitable-based organizations. She is a past chair of the United Way Community Services--Neighborhood and Family Multi-Services Allocation Committee and a past member of the Community Impact Cabinet, the Lighthouse of Oakland County board, and secretary of the board of Ennis Center for Children. She currently serves on the University of Michigan Health Management and Policy Alumni Association, the Southern Wayne County Regional Chamber Executive Committee, the Midwest Chapter of the American Health Care Executives (MCACHE), and serves as a Board member for Playworks. Brooks-Williams is actively engaged in the promotion of growth and diversity in the health care profession. In addition to her past service with the NAHSE, she is a past board member of the Institute of Diversity and currently serves as Delegate to The Region 5 Policy Board of the American Hospital Association s Section for Health Care Systems. In 2013, Brooks-Williams was appointed by Governor Rick Snyder (MI) to the Certificate of Need Commission. Among Brooks-Williams honors are receiving the University of Michigan Five under Ten alumni award for civic and professional service. At its 2004 educational conference, NAHSE presented her the Young Health Care Executive of the Year Award. She was included in the Who s Who in Black Detroit magazine, and awarded the Early Careerist Award from the Michigan Chapter of the American College of Healthcare Executives (ACHE). The Michigan Chronicle selected her as a winner
2 of the 2010 Women of Excellence Award. In April 2010, she was named one of the 'Top 25 Minority Executives in Health Care' by Modern Healthcare magazine. Kristen Adams Chasteen, MD Dr. Kristen Adams Chasteen is a senior staff physician at Henry Ford Hospital in the section of palliative medicine and the associate program director for the Henry Ford Hospital/Wayne State University hospice and palliative medicine fellowship, positions she has held since 2012 when she joined Henry Ford Health System. She spends her clinical time doing palliative medicine consultation in the hospital and in the outpatient clinic. One of Dr. Chasteen s main interests is improving the way that we teach clinicians to communicate with patients about end-of-life care. As part of those efforts, Dr. Chasteen led the revision of our existing Advance Directive booklet to enhance its accessibility to all patients, including those with low health literacy. Dr. Chasteen also is one of the leaders of the Caring Conversations Program at Henry Ford, which is a comprehensive program to teach communication skills to clinicians of all levels using a combination of simulation workshops and bed-side teaching. She is also a member of the Henry Ford Bioethics Committee. Dr. Chasteen is a graduate of Williams College and the University of Michigan Medical School. Her residency in Internal Medicine and her fellowship in Hospice and Palliative Medicine were both at University of California San Francisco. She is board-certified in Internal Medicine and Palliative Medicine.
3 Jennifer Reynolds Jennifer Reynolds is a highly motivated Registered Nurse with over ten years of critical care experience. She transitioned into a managerial role four years ago and is currently the assistant manager of the Intensive and intermediate Care units here at Wyandotte Hospital. She is very passionate about improving practice surrounding care of the chronically ill as they progress to end of life, particularly in the Intensive Care setting. Jennifer firmly believes in the importance of a dignified, patient and family centered approach at any level within the health care spectrum. She led a Quality Caring Rounds pilot in order to educate members of the healthcare team on evidencebased goals of care and interprofessional relationships. She is a certified Advanced Care Planning Facilitator, the Organ Donation Liaison for Gift of Life, and a Trauma Champion. In 2009, she was named Nurse of the Year. That same year, she was a recipient of the Star of Excellence Award at Henry Ford Wyandotte Hospital for providing compassionate care. A highlight in Jennifer s career came when she spent some time in Dublin, Ireland, shadowing the palliative care team at the Mater Misericordiae University Hospital and St. Francis Hospice in Raheny. She observed their beliefs surrounding end of life issues, the politics of healthcare, and their journey toward implementing palliative care into their hospital culture. Jennifer decided to go back to school after seeing the chronically ill suffering, not understanding the progression of their disease, and witnessing their moral distress. She is enrolled in the Doctor of Nursing Practice program at Wayne State University and will be graduating this summer. At that time she will be eligible for licensure as an Acute Care Nurse Practitioner (ACNP) with a doctoral degree in Nursing Practice (DNP).
4 Rev. Jim Kraft For the last fifteen years Rev. Jim Kraft has worked as both a hospice and hospital chaplain. He now serves as Director of Advance Care Planning and Collaborative Services at Henry Ford West Bloomfield and Macomb Hospitals where he working to integrate Advance Care Planning throughout the continuum of care: inpatient, ambulatory and community settings. Before transitioning into the chaplaincy, Jim served in pastoral and mission roles in the Midwest and abroad. He and his wife Celeste have travelled extensively throughout the U.S. and have a rich cross cultural history serving in over 15 countries on five continents. Jim holds a B.A. in Missiology and a Masters of Theology from South Africa Theological Seminary. He is a Certified Advance Care Planner and Certified Advance Care Facilitator Trainer in both First and Last Steps Planning. Jim and Celeste have been married for 28 years and have four sons - Simeon, Judah, Benjamin, and Elijah.
5 James B. Tubbs, PhD James B. Tubbs is Professor of Ethics and Religion and Chair of the Department of Religious Studies at the University of Detroit Mercy, where he teaches courses in Health Care Ethics, Christian Ethics, Ethics and Public Policy, and Social Justice. He is also an Adjunct Professor in the Oakland University - William Beaumont School of Medicine. A graduate of Hampden-Sydney College in Virginia, Dr. Tubbs earned his MA and PhD degrees from the University of Virginia and has been on the faculty of the University of Detroit Mercy since His publications include journal articles, book reviews, case studies, book chapters, and two books: Christian Theology and Medical Ethics: Four Contemporary Approaches (1996) and A Handbook of Bioethics Terms (2009). He also serves on UDM s Human Subjects Research Review Board, two hospital Ethics Committees, and chairs the Ethics Advisory Committee of the Presbyterian Villages of Michigan.
6 Kimberlydawn Wisdom, MD, MS Senior Vice President of Community Health & Equity Chief Wellness & Diversity Officer Henry Ford Health System Michigan s First Surgeon General Kimberlydawn Wisdom, MD, MS is the Senior Vice President of Community Health & Equity and Chief Wellness and Diversity Officer at Henry Ford Health System. She is a board-certified Emergency Medicine physician, the Chair of the Gail and Lois Warden Endowment on Multicultural Health, and Michigan s and the nation s First State-level Surgeon General. In 2012 she was appointed by President Obama to serve on the Advisory Group on Prevention, Health Promotion and Integrative and Public Health. Since 1987 she has been on the faculty of University of Michigan (UM) Medical School s Department of Medical Education and adjunct professor in the UM School of Public Health. Dr. Wisdom focuses on health disparities/health care equity, infant mortality/maternal and child health, chronic disease, unintended pregnancy, physical inactivity, unhealthy eating habits, and tobacco use. She has worked collaboratively with school districts, faith-based organizations and the business community. Dr. Wisdom provides strong leadership in community benefit/population health, and improving the health of those disproportionately affected by poor health outcomes. She founded the awardwinning African American Initiative for Male Health Improvement (AIM HI) and most recently, the Women Inspired Neighborhood (WIN) Network which aims to improve access to healthcare and reduce infant mortality in neighborhoods in Detroit. Since 2008, she has chaired the Detroit Infant Mortality Reduction Task Force. In 2007, she founded a youth leadership development effort Generation With Promise (GWP) designed to equip youth to drive policy, environmental and behavioral change in their school and community. GWP youth were featured on the cover of Modern Healthcare in June Dr. Wisdom is the recipient of numerous awards, has authored several peer-reviewed publications and book chapters and appeared on national television, including ABC s Nightline, and has presented to audiences across the country and internationally.
