2 Welcome Mary Immaculate Health/Care Services (MIHCS) is known throughout the Merrimack Valley as a quality provider of healthcare and housing services for well and sick older adults. The continuum of care offered at Mary Immaculate Health/Care Services is designed to meet the broad social and health care needs of older adults. Our expansive facilities include housing and supportive services for older adults including apartment living, three adult day health centers, a certified assisted living facility, and chair car transportation services. The Mary Immaculate campus also includes skilled nursing, dementia care and a short term rehabilitative program. Mary Immaculate was established in 1868 by the Sisters of Charity of Montreal, the Grey Nuns. Originally founded to care for the orphans of the mills of Greater Lawrence, over the years, MI came to serve an increasing number of elders who needed medical care and support. This began a new focus that would lead to the development of one of New England s largest and most advanced non-profit healthcare facilities dedicated to the care of older adults. The spirit of the Grey Nuns continues to live on our campus. 3 Dementia Care Dementia impacts many families. The National Alzheimer s Association estimates that, today: MI Nursing/Restorative Center (MINRC) is a 231-bed skilled nursing facility devoted to the care of frail older adults licensed by the Commonwealth of Massachusetts. At MINRC, we provide: Skilled Skilled nursing services Dementia care Nursing Restorative and rehabilitative care Therapeutic nutrition and dining services A wide range of therapeutic and recreational activities Religious/spiritual services provided by our Spiritual Care Department The MI Nursing/Restorative Center houses six distinct nursing units, with two units located on each of three resident floors. Each nursing unit serves approximately 41 residents mainly in semi-private, two-bed rooms. Our experts in geriatric healthcare work closely and collaboratively with each resident and their family members, along with physicians and community providers to design a personal care plan for each individual. 5.2 million Americans are living with Alzheimer s disease. Alzheimer s disease is the sixth-leading cause of death in the United States. It is the only disease among the top ten causes of death in the United States that cannot be prevented, cured or even slowed. In 2013, Alzheimer s disease and related dementias cost the nation $220 billion. This number is expected to rise to $1.2 trillion by The MI Nursing/Restorative Center includes a nursing unit dedicated to the care of those challenged by Alzheimer s disease and other dementias. The staff on this secure unit is specially trained in dementia care and cognitive impairment. Activities are tailored to individual needs, and the environment is supportive of residents and their families. Our clinical excellence is recognized by successful surveys by the Department of Public Health and top rating by the Centers for Medicare and Medicaid Services (CMS).

4 Remarkable Recoveries Short Term Rehabilitation Remarkable Recoveries is a dedicated short term rehabilitation unit devoted to the needs of patients requiring short term therapies and care as they recover from illness, accident or surgery. This newly renovated unit features large, private rooms with private bathroom, cable flat screen television and telephone. We specialize in treating the unique needs of people who are in transition between the hospital and home. Our interdisciplinary approach combines short term, aggressive rehabilitation and nursing care designed to return our patients home, usually within 21 days. Remarkable Recoveries features a supportive therapeutic environment. Our aggressive, interdisciplinary rehabilitation services and alternative therapies/modalities are designed to promote healing and enhance the quality of life for our short term rehabilitation residents. Our Interdisciplinary Services Core therapeutic services are offered up to seven days a week and can include: Physical Therapy Exercise and soft tissue mobilization in a fully equipped rehabilitation gym to restore muscle tone and strength, to improve safety and to increase mobility Pain Management Wound Care Falls and Safety Management Nu-Step and Omni Cycle Therapy Modalities for pain control, muscle stimulation and strengthening Ultrasound Community Reintegration Speech Therapy Speech and swallowing evaluation Strengthening swallowing mechanism, compensatory strategies Feeding and oral motor skills Improving written and verbal communication, cognitive abilities, and executive function. Occupational Therapy Improving safety by re-learning basic ADL skills (Activities of Daily Living) Modalities for pain control and stroke recovery Community Reintegration Incontinence Management Cognitive Screening for Safety Additional Services Nutrition Management: Dietician/minimize weight loss/skin integrity Weekly, Comprehensive, Team Care Plan Meetings 24 Hour Care by specially trained post-acute care staff Alternative Therapies
