The care team is directed by our physicians that are certified in Physical Medicine and Rehab. The team also consists of specialized rehab nurses, physical, occupational, respiratory and speech therapists. Gateway has over 14 years of experience caring for patients recovering from strokes, amputations, traumatic brain injuries and trauma. 5 Out-Patient Rehabilitation We know you have a distinct story and Gateway Rehabilitation Hospital is here to help. Whether you were injured in an accident, had a medical disabling event or just want to maintain your health, you want a provider who will help you achieve your goals. Rehabilitation is a partnership and we will work with you and your physician to customize a plan that enables you to achieve your personal goals. Our licensed/certified therapists are experienced in helping patients relearn skills lost through injury, illness or debilitation. Physical and Occupational Therapy involve the evaluation and treatment of physical disabilities. Speech Therapy involves the evaluation of communication, swallowing, and/or cognitive impairments. We offer flexible hours based on your physician s prescription and therapeutic interventions including: Physical Therapy, Occupational Therapy and Speech Therapy Aquatic therapy in our heated pool Wheelchair Assessments Workers Comp/Workplace Injury Fit Club/Functional Fitness Program Free Monthly Health Assessments We will assess several health & fitness components and provide recomendations To schedule an appointant call Lower & Upper Body Strength Flexibility Blood Pressure Endurance Blood Glucose Agility

6 Preparing you for your stay at Gateway Rehabilitation Hospital Your journey to recovery starts with an evaluation by our team members after you are admitted. You will help us to develop the goals which will guide you through your stay. Your active participation is crucial to recovery and we encourage you to work hard to achieve your goals. Our objective is to give you an excellent experience and to optimize your recovery so that you can get better quicker. Your admitting hospital will coordinate your transportation options to Gateway. What to bring: - All patients dress for therapy so bring comfortable clothes and shoes - Please bring your insurance cards and photo ID - Please bring a list of your home medications Visiting hours are from 7:00 am to 8:30 pm daily. We encourage and welcome visitors. All meals are served in the dining room. Family and visitors are welcome. Visitors may purchase a ticket at the front desk to join you for a meal. We encourage families to stop in for a tour to learn more about the Gateway difference.

7 Wellness Club We offer a free Wellness Club which provides you with resources, education and support to help you achieve your optimal health. The Gateway Wellness Club provides monthly programs and Fuctional Fitness Classes to help you live well and thrive, despite disability or injury. We also offer Stroke support groups and many other resources to help you on your recovery journey. Gateway s commitment to you goes beyond rehabilitation to promote ongoing health and wellness.
