(Home Page) (Online Application) INFORMATION REQUIRED FOR DESIGN APPROVAL OF HOISTING MACHINE (Please refer to DOCUMENTS REQUIRED BY CATEGORY FOR DESIGN APPROVAL PROCESS ) 1. DESIGN APPROVAL LETTER Letter of application should include the company letterhead (letter head of company) from the manufacturer, supplier or owner. Letters of application should contain the type of hoisting machine, the model (if applicable), manufacturer and country of manufacturer, the unit requested and the signature, name and designation. Title of the letter should include the application of Design Approval of Hoisting Machine. Example: Application for Design Approval for (2) units of Hoisting Machine 'Roller Coaster'. 2. DRAWING PLAN IN MALAY OR ENGLISH, A1 SIZED SHOW THE FOLLOWING ITEMS:- a) Detail assembly drawing with scale measures. b) Details regarding material specifications including the material size and quantity of materials used. c) Machine specification data such as type, safe working load (SWL), hoist manufacturer name, model hoist, lifting height, lifting speed, floor area, design code, power supply, control and other declaration in the drawing. d) Empty space for the departmental approval seal with a size of at least 12cm x 7cm on the right side of each design plan. e) Drawing plan shall be kept in the format *.pdf and A1 size only. 3. DESIGN CALCULATIONS Estimates the overall structural strength, stability, loads and other details in accordance with international standards. Safety factor. 4. CATALOG / TECHNICAL SPECIFICATIONS Catalogue issued by the manufacturer or the detailed technical specifications of the machinery. Examples that Safe Working Load (Safe Working Load, SWL), size, power, etc. 5. WORKLOAD CHART FOR MACHINERY Work load charts or working distance (working range) or a graph that shows Safe Working Load, (SWL) for the machine. Only for the chart issued by the Maker. Page 1 of 6

CERTIFICATION FROM THE MANUFACTURER / CERTIFICATE OF ORIGIN / MANUFACTURING LICENSE FROM STATE AUTHORITIES OF THE PRODUCER (MANUFACTURER LICENSE) - (FOR IMPORTED EQUIPMENT ONLY) Certificate from the manufacturer - confirmation letter from the manufacturer that the machines produced or a certificate or certification of inspection from the manufacturer (test certificate) Certificate of Origin Certificate issued by the country concerned to verify the machinery from the country. Manufacturing License from the producing countries (manufacturing license) example of China, the expressed Manufacturer Company Name, address and the types of machines can be built. 7. OPERATIONS, INSTALLATION AND MAINTENANCE MANUAL Manual should include how to operate, installation and maintenance of the whole machine. 8. TEST CERTIFICATE / SPECIFICATIONS / HYDRAULIC CILINDER / HOIST DETAILS (WINCH) ATAU/DAN SLEWING TABLE@TURNTABLE OR/AND LUFFING HOIST (WHICH RELATED) Hoist/winch the hoist / winch mounted on the machine structure for lifting. Slewing table@turntable mounted on structures such as tower crane machine for making round. Test Certificate Copies of test certificates containing the model and serial no. of hoist, slewing turntables and / or luffing hoist that shows the results Specifications Contains details of the hoist, slewing turntables and / or luffing hoist used to be installed on the crane structure. 9. TEST CERTIFICATE / SPECIFICATIONS / DETAILS OF WIRE ROPE AND/OR CHAIN Test Certificate - Copies of test certificates that contain the details of the wire rope / chain Wire ropes / Chain - need to put forward showing Size specification, a breaking load, type and so on 10. LIST OF SAFETY FEATURES/EQUIPMENTS Information about the function and position of the safety control devices (safety devices) on the machine. Please specify clearly. 11. IMPORT LICENCE (AP) FROM THE MINISTRY OF TRADE AND INDUSTRY (MITI) / CLEARANCE FORM - (FOR IMPORTED EQUIPMENT ONLY) Import Permit (AP) completed JK69 Form and approved by MITI - To be alone, please consult the websites of MITI to check machinery requiring / not requiring (AP) to be imported into Malaysia - For machines that require AP to be brought into Malaysia only (imported equipment) or / and Custom Clearance Form 1 showing the machine went to Malaysia and shows the machinery serial Form number. 12. TEST CERTIFICATE OF TWIST LOCK AND SPREADER Test certificates for each unit Twist Lock installed on machines Only for the machine that designed using Twist Lock and / or spreader, such as Rubber Tyred Gantry Crane and similar 13.
