2 Welcome to Your Benefit Plan Open Enrollment for 2016! This informational notice is being provided to encourage you to review, evaluate, and make necessary changes to your benefit elections from October 26 November 13, Any changes you wish to make will be effective January 1, A comprehensive overview which includes all benefit plan rates for 2016 is available onlineat hr.tufts.edu/benefits. Tufts University is pleased to offer our employees a comprehensive benefits package that supports you and your family s physical, mental, and financial health. We have had great success with the new array of health plans offered last year and will be continuing those options with no changes to plan design. Carefully assess the benefit options and costs before making decisions for yourself and your eligible family members. You may make benefit changes online via Employee Self Service (eserve) at hr.tufts.edu/eserve/. Please note that all of your benefit plan elections continue from year to year, except the Flexible Spending Accounts which require an active annual election. WHAT is changing for 2016? Health Plan costs for 2016 are increasing by 5.5%. In comparison, our health plans are performing better than the national average health cost increase of 6.5%. Qualified Domestic Partner coverage for Health, Dental, Vision, and Dependent Life Insurance will now include both same sex and opposite sex domestic partners. Dental benefits will continue to be provided through Delta Dental; there is no increase in cost for employees. Coverage for services provided at the Tufts University School of Dental Medicine Clinic has been enhanced. Supplemental Life insurance rates for employees will be decreasing; some age brackets will see reductions as much as 10%. Long Term Disability insurance rates for employees will be decreasing by 5% and the maximum monthly benefit amount will increase from $10,000 to $12,000/month. Health Advocate is a new benefit resource available at no cost to you as of October 26. See the informational section for details in this brochure.

All three options use the same broad PPO network and all three plans cover the same array of medical and pharmacy services. Also, all three plans cover Preventive Care at 100% with no deductibles or copayments. 4 Oral Health is Important Tufts University offers a comprehensive dental plan for you and your family members through the Delta Dental Premier USA Plan, which includes the Tufts University School of Dental Medicine Clinic. The table below compares the coverage through the Tufts University School of Dental Medicine Clinic versus a Delta Dental Premier PPO Dentist. To confirm if your dentist is in the network, please visit Delta Dental s website at or call (800) Dental plan costs are not changing for Cost Per Pay Period: Employee Semi monthly: $6.36 individual / $22.78 two person / $27.26 family Employee Weekly: $2.94 individual / $10.51 two person / $12.58 family *The School of Dental Medicine Clinic is a teaching facility with dentistsin training working with experienced dentists. 5 Your Health is Important: Wellness Center and Health Coaching Services Tufts University is committed to supporting the health and well being of our faculty and staff. We view wellness as a core component of the employee benefits package. The Wellness Center, located on the Medford campus, is operated by Marathon Health, one of the nation s leading providers of worksite healthcare. Services are offered to employees, their spouses or qualified domestic partners and include care for common illnesses, routine physical exams, and onsite lab for services such as blood draws and testing for strep or flu. In addition, the Wellness Center Health Coaching services are provided on all three campuses. These services address lifestyle issues such as stress, nutrition, smoking, and physical activity. Additionally, the Wellness Center also provides disease management coaching for chronic conditions such as diabetes, high blood pressure, high cholesterol, and asthma. Wellness Center services are provided at no cost to you. You have the option of receiving care at the Wellness Center, continuing to see your primary care provider, or combining both in the manner that is most convenient and effective for you. Employee Assistance Program (EAP) and On site Service Tufts University provides an Employee Assistance Program for all employees and their families. The purpose of the EAP is to offer help to those experiencing life changes and personal stress such as relationships, caring for an elder parent, financial difficulties, and problems caused by alcohol or drug abuse. For the convenience of our employees, there are licensed counselors available on all three campuses throughout the week. Please call the EAP at (800) for assistance or to schedule an appointment. Need Assistance with health or health insurance issues? NEW! Health Advocate is Available 24/7/365 Health Advocate, a new and important benefit resource, will be available to our employees and their eligible family members. Health Advocate is designed to help handle healthcare and insurance related issues by cutting through the red tape and barriers that so often create frustration and problems. With Health Advocate, you will have access to a Personal Health Advocate, typically a registered nurse, supported by medical directors and benefits and claims specialists. You will have the confidence in knowing that the entire Health Advocate team is working on your behalf to help you and represent your needs.
