Responsible for the day-to-day clinical management of BMS s School Based Health Programs in Baltimore City Public Schools. Will work in conjunction with BMS' Chief Medical Officer (CMO) to provide leadership and supervision over all clinical services. Medical Director will provide direct patient care services on a part time basis. Leadership opportunity for a Board Certified Internist, Family Medicine Physician, Pediatrician, or Med Peds Physician. Clinical and Administrative role. Director of Clinical Operations & Patient Services Administrative Building 3501 Sinclair Lane The Director of Clinical Operations and Patient Care Services is a registered nurse responsible for the organization s nursing and clinical support services. This position provides leadership and support to the clinical operations of Baltimore Medical System s health delivery centers. In addition, the Director of Clinical Operations Patient Care Services oversees the organization s occupational health, care management and infection control programs. BSN or MSN Master s degree in a healthcare or health policy field Five (5+) or more yoe in healthcare. Four (4+ ) or more years healthcare management Knowledge and experience with Joint Commission requirements, essential Experience in Performance Improvement Management and Quality, essential Patient Centered Medical Home recognition/accreditation experience, desired Knowledge of Federally Qualified Health center requirements desired Director of Facilities Administrative Building 3501 Sinclair Lane Responsible for the management of the organization s facility resources. Functions as a source of environmental health knowledge. Manages facility construction projects, including design, construction, development and installation of facilities and facility systems (HVAC, plumbing, wiring, etc.). Includes space planning including identification of space needs and capital requirements for construction and renovations. Supervises facility technical staff and couriers.
2 ADMINISTRATIVE Substance Abuse Counselor (2) Full Time Responsible for providing a wide range of counseling and casework services. Coordinates efforts with the health center staff. Designs treatment and rehabilitation program using knowledge of drug and alcohol dependency. Conducts intake interviews, clinical assessment, treatment/discharge planning, maintaining client records and linking clients to community resources. Bachelor s degree preferred, must be registered with the Maryland Substance Abuse Professional Certification Board. CSAC strongly preferred. Bilingual and prior experience managing behavioral health in a primary care setting highly desired. Community Outreach Worker (Bilingual: English/Spanish) Administrative Building 3501 Sinclair Lane Functions as a liaison between the patient and Baltimore Medical System s medical teams. Develops a virtual relationship with the patient and is accessible for patients who need to contact their medical team with questions related to their appointment with Baltimore Medical System or appointments with community providers/programs. Will work with the medical teams to increase access for patients with chronic disease or preventive care needs. Reviews chronic disease registries and calls patients to schedule appointments for medical care. Uses internal patient registries, MCO lists, and other means of identifying patients who need access to care at Baltimore Medical System. Communicates and meets with the medical teams to provide feedback to the teams regarding the outreach efforts. Maintains documentation on the success of their telephonic, electronic, or written outreach to patients. More than 3 years experience in a health clinic setting or other client service environment such as a community agency or a neighborhood organization. High School Diploma or equivalent. Interpreter/Care Coordinator Orleans Square 2323 Orleans Street (1) PRN Amharic (1) Part Time Spanish The Interpreters provide a number of services to promote full accessibility to BMS medical services for specified populations of non-english speaking patients & their families such as: interpreting, health care coordination, and health information. Interpreters function as cultural competency resources for both staff and patients and assist with outreach activities. High School Diploma or equivalent degree. More one (1+) year of previous related work experience, preferable in a health care environment. Medical Records Clerk Administrative Building 3501 Sinclair Lane Responsible for the maintenance and distribution of patient medical records: filing, scanning, and retrieving patient medical records in a timely manner, checking patient chart for accuracy and completeness, purging inactive medical records and filing/scanning loose materials. Maintains correspondence. Required: HSD & a Medical Terminology course. 1 year of experience in a medical setting.
3 ADMINISTRATIVE Referral Coordinator Orleans Square 2323 Orleans Street The Referral Coordinator is essential in the management of the patient referral process and is a key link between the clinician, referring specialist, and the patient. The Referral Coordinator must simultaneously maintain an efficient referral system, assist patients and clinicians on an individual basis and manage referrals on a systems level by monitoring referral reports and tracking referrals until close out. Referral Coordinators must have knowledge of the three factors by which all referrals are governed: payors and their insurance rules; providers and network participation, and primary care providers and their referring instructions. High School Diploma or equivalent. One year of experience consistently working with patient referrals required. Service Representative St. Agnes (1) Part Time 2 10 hr days 1 5 hr day The service representative is part of a cross-trained team that performs the following processes necessary for efficient operational function of a health center: pre-registration, registration, appointment confirmation, enrollment, scheduling, rescheduling, insurance verification, panel management, with medical record tasks such as record requests, printing Immunization records, copies of referrals, lab slips etc. Also assist patients by tasking messages through EHR to providers and clinical staff. The position is responsible for greeting customers and providing quality customer service in person and on the telephone. Receives co-payments and post payments for back balances to patient accounts and gives appropriate receipts to patients. Minimum 12 months of experience performing similar duties in a healthcare office.
4 Pediatrician P R O V I D E R S Medical Doctors Middlesex 1245 Eastern Blvd Baltimore, MD Full Time and Part Time positions available Board Certified Pediatrician needed for Full or Part Time positions. The physician will see a diverse patient population with a range of health conditions. Will provide preventive services for patients and also treat patients seeking immediate care for acute illnesses, injuries, or other medical issues. Family Nurse Practitioners and Physician Assistants For Immediate Care Services Saint Agnes Middlesex 1245 Eastern Blvd. Baltimore, MD PRN positions are available for mid level providers. 50 hours a month, minimum requirement. Mid Level Providers PRN Nurse Practitioners & Physician Assistants needed to work in our immediate care clinics. The Extended Services Clinician treats patients from pediatrics to geriatrics seeking immediate care for acute illnesses and minor injuries. A minimum of one year of experience after training is required. Must be comfortable treating patients of all ages.
5 Behavioral Health Integrationist (LCSW-C) Care Manager Substance Abuse Expansion Grant (RN or Social Worker) Saint Agnes SOCIAL WORKERS The behavioral health specialist, as part of the primary care treatment team, identifies, triages and manages patients with medical and behavioral health problems within the primary care setting. In addition, the behavioral health specialist provides skill training through psychoeducation and patient education strategies and develops specific behavioral change plans for patients and behavioral health protocols for target populations. Communication skills are required to persuade or gain cooperation and acceptance of ideas. Responsibility exists to ensure that internal customers are satisfied with service. Represents the company externally and provides information on a variety of programs/services. Licensed Clinical Social Worker (LCSW) certified by the Maryland State Board of Social Work Examiners. The Care Manager is responsible for the care coordination of a designated patient panel and acts as an advocate, educator and resource person for this population. Addresses the logistical, financial, emotional, educational and social barriers to care and compliance using the healthteam model of care. Facilitates & builds relationships with MCO s, hospitals, and other organizations to ensure coordinated care transitions of BMS patients. Leads efforts in building systems and workflows to facilitate care management. Requirements: Bachelor s degree in Nursing or Social Work, a minimum of 1 year of experience working in community-based care with adults including the provision of direct care coordination. Experience in Behavioral Health setting highly preferred. Strong advocacy skills and knowledge of community resources. Bilingual in Spanish highly desired.

Pediatric Charge Nurse Care Manager Substance Abuse Expansion Grant (RN or Social Worker) St. Agnes Under the supervision of the Clinical Manager, the Charge Nurse is responsible and accountable for leading and supervising clinical support staff in the respective department. Works collaboratively with clinicians, administrative staff, and the other center charge nurses to provide safe, high quality, effective, and efficient medical care. Provides direct nursing care as part of the departmental team including triage, patient education, and case management. BSN and a minimum of 2 years of Pediatric experience & supervisory experience. 7 CLINICAL Pharmacy Pharmacist All Locations PRN FLOAT Under supervision and direction of the Director of Pharmacy, provides clinical services and acts as a resource to physicians and nurses, and serves as a resource for clinical practice issues. Produces clinical recommendations and programs for the pharmacy. Assists in the implementation of therapeutic initiatives. Acts as a liaison between Pharmacy, Nursing, and Medical staff. Responsible for aiding and assisting the Director with patient care decisions and therapies. Reviews current clinical programs, staff, and patient education and safety programs, pharmacoeconomic programs, and overall process improvement. Able to access patient specific information and interpret this information to make clinical judgments. Coordinates all clinical pharmacy functions of the department and promotes the role of staff pharmacists in drug quality surveillance programs and disseminating safety, labeling, and quality information on marketed drug products. Pharmacy Courier All Locations PRN FLOAT The individual in this position is responsible for performing courier services primarily between Patient Residences and BMS Pharmacies. Additional courier duties between the Central Office, BMS Health Centers and any other locations, may be required. He/she is required to operate BMS s Company vehicle in a safe and courteous manner in order to pick up prescriptions, prescriptions transfers, interoffice mail, bank change, etc. and deliver them to designated patient residence, health centers, bank, and other facility. Pharmacy Technician All Locations PRN FLOAT Pharmacy Technician Belair-Edison 24 Hours SA 16 Hours FLOAT The Pharmacy Technicians are responsible for assisting with prescriptions by counting pills, measuring and labeling medications, verifying prescriptions from doctors, maintaining patient records, tracking insurance information, and assisting patients with insurance forms as needed. High School Diploma or equivalent. Must be a Registered Pharmacy Technician and must retain certification. Basic medical terminology preferred. 1-3years relevant work experience. High School Diploma or equivalent. Must be a Registered Pharmacy Technician and must retain certification. Basic medical terminology preferred. 1-3years relevant work experience. The Pharmacy Technician will float to other centers as needed. Pharmacy Technician (Bilingual: English/Spanish)
8 CLINICAL Medical Assistants Job Summary: Medical Assistants are responsible for completing the patient intake process, assisting clinicians in providing daily care, performing various medical tasks (vital signs, EKG s, etc), and documenting information in electronic & hard copy medical records while maintaining excellent standards of patient care. Immunization Specialist Belair-Edison Orleans Square 2323 Orleans Street Provides immunizations to the pediatric and adult patients. Managing vaccine inventory (par levels), reviewing expiration dates (and discarding expired vaccines according to the BMS policy), monitoring refrigerator/freezer temperatures, completing vaccine logs, obtaining second signatures on all vaccine orders administered. Obtains the consent for vaccines. Providing the patient/families education regarding vaccines via Vaccine Information Sheets. Documents the lot numbers, the administration details, and billing codes per BMS policy. 3+ years of experience working with patients in a Pediatric or Family Practice setting as a Medical Assistant. Bilingual preferred. CMA/RMA preferred. St. Agnes Medical Assistant Belair-Edison (4) Full Time Experience working with Adult, Pediatric, & OB patients. The MA might be required to float to other centers or departments as needed. Minimum 1 year working experience as a MA outside of an externship. Certification preferred. (6) Full-Time Experience working with Behavioral Health, Internal Medicine, Family Practice, OB-GYN, and/or Pediatric patients.
