1 Dulag (5/7/2014) Meeting with assistant major Nov 6 (2 days before the typhoon) Dulag was hit by a tornado. Nov 430am- wind started from NW 700am Winds stops 730am dark clouds were visible, winds from the South. 3 big waves were observed (threes story high waves) No surge was observed at the building locations. A1 Market Center Building use: retail/ merchandise Years of construction: 4 years old Roofing: metal truss/frame, metal sheet ( mm). 12, 14 and 16 mm bars for bracing the roof frame. Level of repair: no repair Height of water mark: 0ft Structure type: Reinforced concrete frame, 13.4ft span, beam depth= 16in. -Roof: metal frame damaged, metal sheet is gone. Fastener connections for the metal sheets tier-out. -North side of the building experience damage first. A2 Dulag Central School Building use: Education/school Years of construction: 1906 ( under the Americans ) Roofing: wood truss/frame, metal sheet ( mm). Level of repair: no repair Structure type: Reinforced concrete frame.

2 Exterior Walls: Unreinforced concrete block (concrete filled all cells) Floor: Wood deck in the classroom Windows: wood -Roof: wood frame damaged, the majority of the metal sheet is gone. -Concrete column cracks close to the roof. Remarks: Roof debris fell over the floor deck, and collapsed the floor in the classrooms. The school was used as an evacuation center during the storm (2,000 refugees). After failure of the roof the refugees hid under the floor deck. A3 Tower (Globe Network Company) Years of construction: 2004 Height of water mark: 0ft Structure type: Metal truss (constant cross section members) -The tower fell at 10am towards the North. The failure occurred about mid-height. -North side of the building experience damage first. A4 Civic center Building use: Public Assembly Years of construction: 2000 (the inside structure built before 1999 and used for graduations) Roofing: arch type metal truss/frame, metal sheet ( mm). Level of repair: not clear if roof has been repaired. Structure type: Reinforced concrete frame. 36 and 15 feet spans in the shorts and long direction, respectively. Floor: Concrete

3 -Roof: metal frame shows minor damage, large opened area in the metal sheet. 4:30 am winds started from the SE and blew-out the North side roofing metal sheet. Tolosa (5/7/2014) Meeting with major 4am- storm surge 6am- wind 11am- wind stop B1 Old Municipal Building Building use: office Years of construction: 1970 s ( Marco s time ) Roofing: wood truss/frame, metal sheet ( mm). Rebars extended out of the columns to connect to the wood frame. Structure type: Reinforced concrete frame. Exterior Walls: concrete block Floor: Wood deck in the classroom Partitioning: concrete blocks Windows: Glass w/ metal frame Ceiling: gypsum boards -Roof: the metal sheet is completely gone. Remarks: The municipality staff had to occupy the new building because of the extent of damage the old one experienced. B2 Daniel Z. Romualdez Elementary School Building use: education/ school Level of repair: Originally the roof was Spanish ceramic tiles, then reroof with metal sheets.

4 Roofing: combined metal and wood truss/frame, metal sheet ( mm). Structure type: Reinforced concrete frame. Two story building. 39.5in. by 10in. columns. Exterior Walls: concrete block Partitioning: wood walls Windows: Glass. -Window damage: glass is gone. B3 Mr. and Mrs. Pomposo Santillano Memorial Sport Center Building use: Sports Years of construction: 2009 Level of repair: None Roofing: metal truss/frame, metal sheet ( mm). Structure type: Reinforced concrete frame. Floor: concrete deck -Roof: the metal truss is completely damaged and unstable (should be removed ASAP). The metal sheet is completely gone. -Visible damage to the concrete frame (probably caused by coconut trees debris). Remarks: According to the interviews, the water level got to the top of the beam, 13.6 feet. B4 Residential structure across the street from B3 Building use: residential Level of repair: none. Roofing: metal truss/frame, metal sheet ( mm). Structure type: Reinforced concrete frame.

5 Exterior Walls: concrete block Windows: Glass w/ metal frame. Ceiling: gypsum board -Window damage: glass is gone. -Evidence of water debris inside the electric breaker box (water level?) - Across the street (same side as B3), there is a church used as an evacuation center in which were many deaths, water level got up to 4 9. Tanauan (5/7/2014-5/8/2014) Meeting with major Most of the damage was caused by the storm surge. In the municipal building the surge heigh was 10 feet. 6,350 deaths out of a 53,000 population. 5am Wind started 630am Surge water came very fast and stayed for an hour. 700am experienced the strongest winds Debris was observed up to 2KM from the beach. 14 meter diameter circular wind direction. Localized damage from wind. No lightning was observed. Poor visibility, everything look white because of the water. C1 New Leyte Edible Oil MFG., Corporation Several structures. We were not allowed to take pictures. 40 Year old steel frame building. 14 feet high storm surge at green building Storage tank overturn, does not have connections at the base, just gravity. The tank was 1/3 full during the Typhoon. Dimensions: 44feet diameter by 54 feet high.

6 C2 Barangay Hall Building use: Public Assembly Level of repair: None Years of construction: 2010 Phase 1 and 2012 Phase 2 Roofing: metal truss/frame, metal sheet ( mm). Structure type: Reinforced concrete frame. Columns 12 x 12.5 Partitioning: reinforced concrete blocks (serve as shear walls). Windows: Gone Doors: Gone Ceiling: wood (or gypsum board). No damage in the north side. Remarks: Lateral E-W direction movement of the building was experienced during the Typhoon. Water surge got up to halfway of the second floor. Shear of coconut tree about 8 meters high. Weichang s last terrain photo is toward North direction. C3 DPWH Conference Center Building use: Public Assembly Roofing: metal truss/frame, metal sheet (0.14mm?). 40 overhang of metal truss. Self tapping screws attachment, 16 on center. 6 wide steel beam. Structure type: Reinforced concrete frame. Building footprint is 100ft x 60ft (100ft by 51ft if not take porch into account). Rectangular columns are 17 x 20. Circular columns are 16 diameter. Exterior Walls: Concrete blocks. -Windows: damaged by rock debris? -RC Frame: Cracks in the infill below beam. Remarks: This building is currently being used for coconut lumber storage to be distributed for restoration efforts. Self tapping screw sample was taken by the team.

7 C4 Public Market Building use: retail/ merchandise Roofing: metal truss/frame, metal sheet ( mm). Rebar extents out of the wall/column to connect the roof frame. Metal acoustic grid for ceiling. Exterior walls and partitioning: 6 thick RC Structure type: Single story structure. -Roof: metal frame gone. Remarks: South side inspection of the building was done by team Master, Pang, Kennedy and Rullan. A portion of the building was shielded by a higher structure East side. Crack on the wall may be because of shifting of the building because of wind (one side in compression and the other in tension). Palo (5/8/2014) Meeting with major (she used to be the Governor of Leyte Province) Water didn t reach the municipal building, which is 1 km from the coast line. Wind damage was perceived in the municipal building. The church across the street (in the square) was converted into a hospital for five days. The strongest winds were perceived from 5am-8am, and then they suddenly stopped. Major damages are to roofing and vegetation. Almost 100% of the buildings suffered roof damage. DPWH structural review of the ongoing reconstruction. Discussion about the wind speed design from 175 kph to 250 kph. Remarks about building unreasonable structures for citizens with fewer resources. D1 Sacred Heart Seminary Building use: education/ school Year of construction: 1944 Roofing: wood truss/frame. Structure type: Reinforced concrete frame. Three story building. 12 square columns. Partitioning: plywood walls, non-structural Windows: Glass with wood frame.

8 Floor: Wood deck, joists are x 7.5, 12 on center. -Roof: completely gone, probably removed to place the new steel roof truss/frame. D2 Palo Convention Center Building use: Public Assembly Year of Construction: 2013 (Never inaugurated, construction was almost complete before typhoon) Roofing: metal truss/frame, metal sheet. Structure type: Reinforced concrete frame. 2.5ft column depth under floor, no indication of column foundation. 25MM rebars in columns. Exterior Walls: Reinforced concrete blocks. -Roof: steel frame completely collapsed. -RC Frame: Erosion under east wall caused loss of bearing capacity ultimately producing the wall collapsed (the wall collapsed away from the building, towards east). Tacloban (5/8/2014-5/11/2014) E2 Residence Building Water level up to the first floor roof. Exterior Walls: concrete blocks. -Metal sheet roofing in the first floor was not damaged during the typhoon. -Metal sheet roofing in the second floor was damaged during the typhoon and replaced. E3 Airport Control Tower The control room is the last floor in the tower (fifth floor). The top of the floor is about 70meters high. Only the south glass window survived. The rest of the windows glass broke during the typhoon (a sample of the glass was taken). The window tempered glasses are 0.5 thick, 66 by 75. The windows are inclined about 20 degrees from a vertical axis.

9 Leyte Park Resort Hotel B1 004 Sports Complex This is building 32 in the figure above. The building is completely collapsed and is not safe to walk in the structure. Roofing: metal truss/frame, asphalt shingles. Structure type: Reinforced concrete frame. Exterior Walls: Reinforced concrete blocks. B1 005 Convention Center This is building 31 in the figure above. -Waves crashed at the top of the second floor, which is 18ft high. Structure type: Reinforced concrete frame. Exterior Walls: Reinforced concrete walls. East wall collapsed. Concrete walking side rails are 34 high x 5 diameter. They are spaced at 5 3. Roof has been replaced.

10 B Restaurant This is building 26 in the figure above. 1 story building, slab on grade. The building is completely gone. The slab remains. B These are buildings in the figure above. The water level goes up to the top of the roof truss. New coconut lumber. Reconstructed roof, metal sheet (old metal truss/frame). B Spa This is building 41 in the figure above. B Zipline This is building 42 in the figure above. -Small concrete bunker. The rails in the roof fell over because of the wind loads. The rails were rusted out at their bases. B Power House/ Engineering Office This is building 21 in the figure above. Structure type: Reinforced concrete frame. Roofing: timber truss/frame, concrete tiles over metal sheets. Siding: wood. B Club 65 hundred This is building 33 in the figure above. Roofing: metal truss/frame, metal sheets. Tap screws in C sections. Roofing damage in the back structure (east side). B Formosa Tea House This is building 36 in the figure above. Roofing: timber truss/frame, fiberglass shingles (20-30years old). Roofing has been replaced by new metal shingles. 6 fastener spacing in the new roof. In the old roof are 4 nails per shingle. Extensive loss of shingles in the centers and corners. B Radio Station This is building 37 in the figure above.

11 Roof has been repaired. Same construction as in convention center (building 31). There is no access to the second floor roof. Roofing: timber truss/frame, metal sheet. B2 001 University of Philippines Level of Repair: Medium - Full Roofing: metal truss/frame, metal sheet. Windows: jalousie windows Entrance point small building: damaged to column produced by a wood debris. B2 002 University of Philippines Library Building use: Education/school Years of construction: 2012 Roofing: metal truss/frame. 2 long fasteners, 16 on centers. Structure type: 3 story building, reinforced concrete frame. 16ft bay width. Windows: Glass w/ aluminum C section frames -Roof: wind damage- windows failed and caused the internal pressure that produced the roof failure. Remarks: Water level almost reached the 2 nd floor B Gaisano Capital Central They did not let us in the building. Water level was about 3ft over the top of the stairs. B1 002 Leyte Provincial Capitol Meeting with Engineer 5-6am Strongest winds.

12 7-8am maximum water level. The engineering office was fully submerged. 3ft of water at the ground level. Debris got up to this location, which is about 100 meters from the shoreline. The only damaged observed in this building was to the windows. B2 001 Old University of Philippines Roofing: metal truss/frame, metal sheet (many corrosion). The C sections are 4 wide. Windows: jalousie windows Remarks: minor to none damage to this building because it is heavily shielded by the taller surrounding structures. Residential House at 3pm Water level got up above to 2 feet on the second floor. Floor: RC slab, ceramic tile on top. Roof damage because of wind. B Private Beach Resort Building use: 2 stories residential building. The structure next to this building is completely gone. Roofing: C channels w/ timber truss/frame, gypsum board with concrete tiles (the tile manufacturer is HARD ROCK ENT.) Structure type: Reinforced concrete frame square columns. Windows: Glass. B San Jose Central School Met with the school principal: Monina E Gabrino- We lost many friends but we gain new friend referring from the national help they have received after the event. 2 stories building. Structural type: RC Frame

13 Roofing: metal truss/frame. Water level up to almost the second floor. This building was used as an evacuation center and 10 children died. B62A- 001 Building across street from Astrodome 2 stories building. Water level got up to 114 inches above ground. Roofing: metal truss/frame, metal sheet. Tacloban Convention Center Astrodome Three stories round structure. There is no access to go up the structure. This building was used as an evacuation center. Structural type: RF frame w/ exterior reinforced concrete block walls Partition: Lightly reinforce concrete block walls. Many of these walls are breached. B Marasbaras National High School Building use: Education/school Level of repair: Several structures, some have been repaired. Roofing: metal truss/frame w, C sections, metal sheet ( mm). Structure type: Reinforced concrete frame. Exterior Walls: Reinforced concrete block Ceiling: gypsum board w/ wood grid Windows: glass w/ metal frame -Concrete block gable end wall appears to have fallen towards the outside of the building. Remarks: Brandon found water mark of 6-5 in a bathroom. Roofing companies: GalvaSteel and Apo Puyap. B Marasbaras Elementary School Fully repaired. No water mark or roof damage is noted.

14 GSIS Government Service Insurance Building 4 story RC frame building. Roof repaired: new metal sheets, the old truss/frame were not replaced because it did not suffered significant damage. Water level: 5-2 above grade level. Gaisano Super Metro Level of repair: none. Roofing: metal truss/frame, metal sheet ( mm). Structure type: Reinforced concrete frame. RC foundation. Exterior Walls: Reinforced concrete block Remarks: Building collapsed. Water level of 2. Flexure in columns, this is an indication of footings. The Church of Jesus Christ of Latter Days Level of repair: full. Saint Scholastica Private School Building use: Education/school Construction year: Level of repair: none. Roofing: metal truss/frame w, metal sheet ( mm). Structure type: 4 story building. Reinforced concrete frame. Partitions: Concrete block walls. Ceiling: metal grid Windows: glass w/ metal frame -Roof truss/frame collapsed in 4 th floor.

15 Remarks: Water level of 6-7. B Old Tacloban Public Cemetery Several 2 story RC frame structures. Roofing: timber truss/frame. The metal sheets are gone. Remarks: Water level of 3 B Infant College Building use: Education/school Level of repair: Several structures, some have major repairs. Roofing: Arch metal truss/frame w, metal sheet ( mm). Structure type: 2 story building. Reinforced concrete frame. Windows: glass (broken) -Roof metal sheets pulled out. The fasteners remained in place with no damage. Fasteners typ 10 on centers. Remarks: No flood/surge at this location. Mother Mary Hospital Fully repaired. Bethany Medical Arts Center They didn t let us in. 3 story building. Roof metal sheet replaced. The roof is not straight probably because they used the old truss/frame structure. B Tacloban Public Market Building use: retail/ merchandise Roofing: metal truss/frame w/ C sections, metal sheet ( mm). Structure type: Two story structure. RC frame.

16 -Roof: not damaged! Remarks: water reached the floor of the second level (to scale the height a picture was taken w/ a 70 measurement. B RC structure w/ a RC flat roof. According to the interview the water level at this location was ft. B Metal roof truss, sustained on pipe columns. All metal sheet roofing is gone. San Jose SJ-1 RC frame structure. 10 columns w/ 4 no. 3 rebars. Remarks: Only the floor and slab remains. Small slab under the main slab. San Jose SJ-2 Large RC residence owned by a Belgium family. Partitions: Concrete block walls, lightly reinforced. Windows: 10ft above ground and have no damage. Remarks: main core structure seems fine. According to the interview the water level was 12.5ft. San Jose SJ-3 Two story RC residence. Partitions: are gone Windows: 10ft above ground. Remarks: concrete damage from water, south side wall is damaged. Timber truss/frame roof is gone. San Jose SJ-4 Only the slab remains.

17 San Jose SJ-5 Only the slab remains (concrete block slab). Erosion under the slab. This property is protected by a 7 high sea wall. San Jose SJ-6 RC frame structure, roof is gone. The windows and the east wall are also gone. Roofing: Timber truss/frame. San Jose SJ-7 Roof has been replaced. 9ft water level. San Jose SJ-8 Warehouse building RC frame building. No connection between column and walls. 13ft on center columns. Roof: Metal truss/frame damaged. Remarks: South and North walls are leaning south. West and East walls are gone. It is likely that the roof truss is in compression because of the way the walls are leaning. San Jose SJ-9 RC frame building. Roof: timber truss/frame, metal sheets. Remarks: Beam collapsed. At least half of the roof frame is gone. South wall is also gone. Erosion under foundation at the south east corner.
