This plan may be based n the use f an evidence based practice, r may be the result f a lcal innvative, rganic apprach t service delivery. Our cnceptualizatin f Evidence Based Practice is that is ccurs n a cntinuum, as described belw: Innvative Practices are cmprised f practice appraches r prgrams that exist because a cmmunity agency determines they are a gd service delivery strategy; hwever, this practice is in existence withut prir frmal evaluatin f effectiveness. Innvative practices are ften simply the activities an agency cnducts as a part f regular service delivery, r a prgram r practice that the agency wants t ffer but has nt had adequate funding t implement r sustain, but that is nt a tested prgram; Evidence Based Practices are thse practice appraches r prgrams that have evidence f their effectiveness fr the target ppulatin. These are prgrams fr which there is a rigrus evaluatin plan in place that has shwn that the prgram wrks.
