I have been appointed Assistant Clinical Instructor at the School of Pharmacy and will be precepting student pharmacists during their 3rd and 4th professional years. My current practice interests include geriatrics, transitions of care, ambulatory care, and academia. I am currently a member of the American Society of Health-System Pharmacists (ASHP) and American Society of Consultant Pharmacists (ASCP). During my free time, I enjoy being outdoors, figure skating, traveling, swimming, cooking, trying new foods, and spending time with my friends and family. Katrina Keith, Pharm.D. - PGY1 Pharmacy Practice [email protected] I received my Bachelor of Science in 2009 at Texas A&M University- College Station in Biochemistry and Genetics. Currently interested in pursuing a PGY2 in Critical Care after completion of PGY1 at Hendrick Medical Center. My professional interests are critical care, acute care, and pediatrics. I am currently a member of American Society of Health System Pharmacists, Phi Delta Chi, and American Pharmacists Association. I worked as a Microbiologist in Austin for a Tb laboratory prior to attending pharmacy school. My hobbies include exercise, outdoor activities including hiking and running, and cooking. Kristy Walker, Pharm.D. - PGY1 Pharmacy Practice [email protected] Kristy Walker is a PGY1 resident at the TTUHSC and Hendrick Medical Center. Dr. Walker is also appointed as an Assistant Clinical Instructor at TTUHSC SOP and will be precepting student pharmacists during their 3rd and 4th professional years. She received her Bachelor of Science in biomedical sciences from the Texas A&M University in College Station, Texas. She then received her Doctor of Pharmacy degree in 2015 from Texas A&M University Health Science Center Irma Lerma Rangel College of Pharmacy in Kingsville, Texas. Her practice interests include mental health, infectious disease, substance abuse, and pain management. Currently, she is a member of American Society of Health-System Pharmacists, College of Psychiatric and Neurologic Pharmacists, and Rho Chi. During her free time, she enjoys experimenting and trying new foods, exercising and learning about nutrition, watching football, playing the viola, and spending time with family and friends. Following completion of the pharmacy practice residency, she plans to pursue a PGY-2 specialty residency in mental health.

She is also appointed Assistant Clinical Instructor at the School of Pharmacy and will be precepting student pharmacists during their 3 rd and 4 th professional years. She received her Doctor of Pharmacy degree from TTUHSC School of Pharmacy in Lubbock, Texas in Dr. Patterson s current practice interests include critical care, infectious diseases, and academia. Keith Chow, Pharm.D. - PGY1 Pharmacotherapy [email protected] Dr. Chow is a 1st year Pharmacotherapy Specialty Resident at Texas Tech University Health Sciences Center School of Pharmacy and Northwest Texas Hospital in Amarillo. Dr. Kaczmarski received her Doctor of Pharmacy degree Magna Cum Laude from St. Louis College of Pharmacy in May Additionally, Dr. Kaczmarski completed her ASHP accredited PGY1 residency with the University of Wisconsin School of Pharmacy Community Pharmacy Residency Program from July 2014 to June 2015, where her primary practice site was the Stockbridge-Munsee Health and Wellness Center a tribal primary care clinic with outpatient pharmacy in rural Wisconsin. Dr. Kaczmarski also holds certifications as a certified asthma educator (AE-C) and as a certified tobacco treatment specialist (CTTS). Her practice areas of interest include mental health, smoking cessation, asthma/copd, and cardiology. Dr. Kelly completed her PGY-1 pharmacy practice residency at the VA North Texas Health Care System in July Katherine is a native Texan raised in Colleyville, TX. Coming from a family of Aggies, Katherine graduated magna cum laude in December 2009 with her Bachelor of Arts degree in chemistry from Texas A&M University. Dr. Kelly graduated with high honors with her Doctor of Pharmacy degree from the University of Texas At Austin and the University of Texas At Austin Health Science Center at San Antonio in May of Dr. Kelly is also appointed as an Assistant Clinical Instructor at the School of Pharmacy and will be precepting Texas Tech pharmacy students. Dr. Kelly is a member of the Hematology/Oncology Pharmacy Association and Rho Chi Honor Society. In her free time, she enjoys spending time with her husband, family, and friends, exercising, and watching Aggie football.

She received her Doctor of Pharmacy degree from the University of Houston in May 2013, and subsequently completed a PGY1 Pharmacy Practice Residency at Methodist Hospital in San Antonio, Texas. During her free time, she enjoys the traveling, trying new eateries and Netflix marathons. Whitney Zentgraf, Pharm.D. - PGY2 Geriatric Pharmacy [email protected] Whitney Zentgraf is a PGY-2 Geriatric Pharmacy Resident at Texas Tech University Health Sciences Center School of Pharmacy (TTUHSC SOP) and VA North Texas Healthcare System in Dallas, Texas. Dr. Zentgraf received her Doctor of Pharmacy degree from Virginia Commonwealth University School of Pharmacy in 2014 in Richmond, Virginia and completed her PGY-1 Pharmacy Practice Residency at Campbell University College of Pharmacy & Health Sciences and East Carolina University Family Medicine in Greenville, North Carolina. Dr. Zentgraf is appointed as an Assistant Clinical Instructor at TTUHSC SOP and will precept pharmacy students in their third and fourth professional year. Her practice interests include geriatric primary care, falls prevention, geriatric psychiatry, and polypharmacy. Dr. Zentgraf is a member of several professional organizations including the American Society of Consultant Pharmacists, American College of Clinical Pharmacy, American Society of Health-System Pharmacists, and Kappa Epsilon. As this is her first time in Dallas, she loves to explore her new Texan surroundings with friends, swim, bike, run, read, and travel. She received her Doctor of Pharmacy from Texas Tech University Health Sciences Center School of Pharmacy. Her practice interests include ambulatory care, internal medicine, and oncology. Her practice interests include psychiatry, substance abuse, and ambulatory care. During her free time, Dr. Duquette enjoys traveling, shopping, baking, reading, and puppies.
6 Catlin Grisham-Takac, Pharm.D. - PGY1 Pharmacy Practice [email protected] Catlin Grisham-Takac is currently a PGY-1 Pharmacy Practice resident at the VA North Texas Healthcare System. She grew up in the not so tiny town of Buda, TX and earned her B.S of Animal Science at Texas A&M University in College Station in Dr. Grisham-Takac then received her Doctor of Pharmacy in May 2015 from Texas Tech University Health Science Center School of Pharmacy (TTUHSC-SOP). Her practice areas of interest include ambulatory care, particularly in diabetes management, mental health and geriatrics. Dr. Grisham-Takac is a member of American College of Clinical Pharmacists, American Society of Health System Pharmacists and Kappa Psi Pharmaceutical Fraternity. In her free time, she enjoys singing karaoke, playing the trumpet, horseback riding, catching a Bob Schneider concert and watching Aggie football. Nwachukwu, Pharm.D., MPH - PGY2 Pharmacotherapy [email protected] Dr. Linda Nwachukwu is a 2nd year Pharmacotherapy Specialty Resident at Texas Tech University Health Sciences Center School of Pharmacy and University Medical Center in Lubbock. Dr. Nwachukwu is also appointed as an Assistant Clinical Instructor at the School of Pharmacy and will be precepting student pharmacist during their 3rd and 4th professional years. Dr. Nwachukwu holds a Doctor of Pharmacy from Howard University College of Pharmacy, and a Master of Public Health with a Biostatistics and Epidemiology concentration from Drexel University School of Public Health. Her practice interests are multifaceted including but not limited to internal medicine, academia, critical care, cardiology, and infectious diseases. Dr. Samuel is currently the secretary/treasurer of the Lubbock Area Society of Health System Pharmacist and is also a member of Texas Pharmacy Association (TPA), and American College of Clinical Pharmacy (ACCP). Dr. Samuel enjoys volunteering at the Lubbock Impact Clinic where she collaborates with medical staff to provide care for uninsured patients.
