1 UW Health Pharmacy Resident Biographies PGY1 Pharmacy Residents Amanda Condon Pharmacy School: University of Arizona Residency Project: Implementation of clinical decision support for HIT testing in adult and pediatric patients Career Interests: Academia, clinical research, solid organ transplant, infectious diseases, professional leadership Hobbies: Spearfishing, hiking, biking, yoga, cooking, and cheese curds Why UW? While it is well known that UW has a strong residency program, what drew me here was the connection I had with the preceptors and pharmacy leadership on my interview day. I distinctly remember not wanting to leave my interview because everyone was fun, genuine, and innovators within their field of interest. This theme carried throughout interview season as I met other UWHC alumni (and my coresident, Eileen!) across the country. When you join UW, you join a family that will support you, challenge you, but also knows how to let loose with you. I cannot imagine myself anywhere else! Emily Dworkin Pharmacy School: University of Iowa Residency Project: Oral chemotherapy patient outreach standardization: interprofessional redesign to maximize pharmacist efficiency and improve patient safety Career Interests: Oncology/hematology, critical care Hobbies: Staying active outdoors, traveling, running, spending time with my family and friends, and cheering on the Iowa Hawkeyes and my favorite Chicago teams, the Blackhawks, the Bears and the Cubs. Why UW? The people! The pharmacy residency program at UW is outstanding, but it is the people here that make the difference. Initially, when I was applying for residencies, I was focused on how challenging programs were and what experiences and opportunities were offered. After my interview at UW, I realized how important it is to be surrounded by preceptors who will challenge you but also support you. Every day I am surrounded by people who are genuinely invested in my training and my success as a pharmacist and as a person. They also have very good cheese.

2 Jenny Koehl Pharmacy School: University of California, San Francisco Residency Project: Pediatric acute kidney injury prevention tool implementation Career Interests: Pediatrics, Critical Care, Emergency Medicine Hobbies: Soul cycling, barre, wine tasting, listening to live music, eating Jimmy Johns Why UW? I chose UW Health for my residency training because of the institutional mindset that emphasizes clinical risk taking and inquisition. Also, the overwhelming sense of family and mentorship made me feel comfortable expressing and highlighting the areas of my practice that I wanted to further develop during this year. This strong foundation supports an amazing assortment of clinical, academic, and administrative rotations that are enhanced by a challenging staffing model. And as an added bonus the cheese curds that ranked #1 are sold a mile from my house. Luiza Kerstenetzky Residency Project: UHC multicenter project: impact of discharge medication reconciliation Career Interests: Emergency medicine and ambulatory care with a transitions of care focus Hobbies: Dancing and traveling Why UW? I worked at UW as an intern during pharmacy school. When it came to applying to residencies, I considered many places but found it hard to leave a hospital that offers such a strong residency program with lots of opportunities. I also was excited to continue learning from/with the inspiring pharmacists that had helped shaped me into the young pharmacist I am today. I am passionate about transitions of care and the integrated pharmacy practice model has been a good fit for my interests.

3 Eileen Shannon Residency Project: Venous thromboembolism prophylaxis quality initiative: electronic documentation tool and electronic reporting of CMS measures Career Interests: Critical care and emergency medicine Hobbies: Running, biking, visiting my two adorable nieces, and exploring Madison s fantastic ice cream, beer, and cheese curd scene. Why UW? My top three reasons to choose UW: First, the University of Wisconsin hospital has the most progressive pharmacy model that both enhances patient care and safety while providing a challenging and collaborative practice environment. I wouldn t want to be trained anywhere else! Second, being part of the Wisconsin pharmacy family gives you a huge network of support and contacts all over the country. And lastly, our preceptors are brilliant, dedicated, and extremely fun. Erin McCreary Pharmacy School: Auburn University Residency Project: High-dose methotrexate process optimization through prior-to-admission urine alkalinization and supportive care standardization Career Interests: Infectious Diseases, Oncology, Academia Hobbies: Spending time on the lake, trying new restaurants, venturing to local festivals/fairs, burning old school mix CDs, hosting board game nights, watching football, and attempting to turn Pinterest dreams into a reality Why UW? The people of UWHC are passionate and innovative in every aspect of pharmacy practice, and they are especially dedicated to fostering the personal and professional growth of residents. They embrace the wealth of opportunities the institution has to offer, including ample clinical rotations, a culture of mentorship, a multidisciplinary approach to education and patient care, a diverse patient population, a commitment to leadership, and a strong sense of community. From my first introduction to the program at Midyear, I knew this was an institution where I would be intensely challenged yet overwhelmingly supported, and where I would develop into a knowledgeable and considerate practitioner -- the ideal environment for residency.

4 Junior Administrative Residents Nicholas Capote Pharmacy School: University of Tennessee Health Science Center Residency Project: Contract pharmacy evaluation and implementation following integration with a 340B eligible hospital Career Interests: Professional Organizations (Pharmacy Society of Wisconsin, University Health-System Consortium, American Pharmacists Association, American Society of Health-System Pharmacists), decision support tools and data manipulation, contract negotiation, technology in health care, payment and reimbursement, ambulatory pharmacy business development, 340B Drug Program, teaching and mentorship Hobbies: Outdoor team sports, camping, hiking, trying new food and drink Why UW? It was an absolute fit. I also realized I would be farthest from my comfort zone in the UW program and that was exactly what I wanted when searching for a quality residency. Additionally, the legacy stemming from this program is second to none. The career potential is limitless after receiving the type of training UW Health and the administrative program have to offer. The pharmacists here are qualified, fun to be around, and take the growth of their residents seriously. Wisconsin has some pretty delicious cheese curds, too! Ryan Craynon Pharmacy School: University of Cincinnati Residency Project: Evaluation, consolidation, and optimization of a comprehensive prior authorization service across a multi-hospital academic center Career Interests: Pharmacy administration within a large healthsystem. Specifically, my current interests are in operations, informatics, and supply chain. Hobbies: Staying active Running and golfing Why UW? First and foremost, I wanted to be a part of an innovative pharmacy department that plays an integral role within a healthsystem and UW fits the bill. The UW management team and preceptors are of the highest quality with a track record of producing leaders that are shaping the profession of pharmacy today. Residents are tasked with meaningful projects to complete with the chance to impact the UW health-system on many levels. Overall, UW provides a resident with the necessary resources, family atmosphere, and opportunities to excel as a pharmacy resident.

5 Dan Schenkat Residency Project: Impact of a multidisciplinary quality improvement initiative on STAT medication turnaround time Career Interests: Healthcare informatics, medication safety, and leadership development Hobbies: Playing the drums, triathlons, backpacking, road-tripping to see friends Why UW? I chose UW for its reputation of consistently producing great pharmacy leaders, the integrated clinical rotation/administration rotation/master s degree residency format, and the strong emphasis that is placed on enhancing the medication use system. Jim Langley Pharmacy School: Southern Illinois University Edwardsville Residency Project: Parenteral nutrition quality, safety and efficiency performance improvement project Career Interests: Shifting the paradigm of pharmacy practice Hobbies: Ice hockey, soccer, reading, hunting/fishing, recreational sports Why UW? 6 Senior Administrative Residents Rachael Fleagle Pharmacy School: University of Cincinnati Residency Project: Evaluation of a Community Tech-Check-Tech Program as a Strategy for Practice Advancement Career Interests: Pharmacy Administration! Hobbies: Tennis, travel, huge Big Ten sports fan (Boiler up!) Why UW? The UW HSPA program provides us with the opportunity to truly get our hands dirty through first-hand experience with project management from start to finish. The opportunity to work with leaders in the profession on a daily basis as well as the amazing network of alumni makes UW an outstanding program! Heather Helsel Pharmacy School: Ohio Northern University Residency Project: Insight through analytics: Engaging prescribers to use clinical data to minimize medication utilization variation Career Interests: Inpatient operations, clinical management, medication safety and data analytics Hobbies: Hiking, camping, running anything outdoors related. In addition, I enjoy my Mom s baking, spending time with my family and exploring Madison with my fiancé. Why UW? The tradition of UW program is why I applied for the health-system pharmacy administrative residency. For over 50 years, UW has been on the forefront of pharmacy practice and has developed exceptional leaders. Program directors, preceptors, and pharmacists are all dedicated to making each resident successful.

7 Tim Hinkley Pharmacy School: University of Michigan Residency Project: Pharmacy technician workforce development, engagement and retention improvement through FOCUS-PDCA Career Interests: Administration/Management (Ambulatory, 340B, Specialty, Supply Chain, Inpatient Operations) Hobbies: Spending time with my wife watching college sports (Go Blue!), trying new restaurants, traveling and going to movies Why UW? It was an amazing opportunity to lead and interact with great pharmacy leaders & mentors in the profession, partake in a multitude of different learning and networking experiences, lead major projects that have large impacts on the department, ability to complete coursework that supplements the administrative residency very well and the amazing pharmacy (administrative residency) history. Jerame Hill Pharmacy School: Samford University Residency Project: Implementation of a multi-site inventory management system at an academic medical center Career Interests: Pharmacy operations, supply chain, and specialty pharmacy Hobbies: Football and watching movies Why UW? The UW HSPA program has the reputation for producing leaders in our profession. The experiences offered in this program are unmatched and allow residents the opportunity to lead major projects that are essential to the mission of the department. As a 2 nd year administrative resident you are truly part of the management team and able to make decisions that affect the department. Finally, the UW network is extensive with past graduates in many areas of health care.

8 PGY1 Community Practice Resident Morgan Meinert Pharmacy School: University of Iowa Residency Project: Development of an electronic heart failure management tool for use by community pharmacists to optimize medication use and improve patient follow-up Career Interests: Primary care clinical pharmacist Hobbies: Watching Iowa Hawkeye sports, snowboarding, scrapbooking, dancing Why UW? I chose UW because it's an academic medical center that promotes continuous learning and allows for teaching opportunities. I also like how progressive the state of Wisconsin is in pharmacy practice and am excited to be a part of the development of pharmacy practice as a whole. Additionally, the staff and preceptors here are genuinely interested in my growth not only as a pharmacist but as a person. PGY1/PGY2 Specialty Pharmacy Resident Chris Hulstein PGY1 Residency Project: Development and evaluation of specialty pharmacy metrics to define and measure pharmacy resource needs and performance Career Interests: Clinical pharmacy, specialty pharmacy management, specialty pharmacy consulting, expansion of pharmacy services in specialty ambulatory clinics Hobbies: I enjoy staying active by heading to the gym, wakeboarding, hiking, and am just getting into cycling. I love cooking, and I read whenever I can. Why UW? Throughout pharmacy school, I completed a few rotations at UW Hospital and knew first-hand the excellence this pharmacy department practices. The culture around pharmacy practice in the state of Wisconsin is phenomenal, and the amount of responsibility pharmacists have at UW Hospital is unmatched. I enjoyed working with my preceptors during my APPE year and loved living in Madison I knew I had to stay put for another couple of years.

9 PGY1/PGY2 Medication Systems & Operations Residents Ashlin Samuel PGY1 Pharmacy School: Albany College of Pharmacy & Health Sciences Residency Project: Non-sterile compounding USP Chapter 795 compliance in the inpatient and ambulatory setting Career Interests: My professional interests include: pharmacy operations, medication use systems, pharmacy practice leadership, performance improvement, medication safety, transitions in care, as well as geriatrics, pain management and psychiatric pharmacotherapy. Hobbies: I am an avid sports fan! I enjoy being active, baking, continuously learning, and spending quality time with my friends and family. Why UW? UW's Department of Pharmacy has been a leader in advancing pharmacy practice and the hybrid drug distribution model and opportunities of a large academic health system were very appealing to me. I was excited by the opportunity to become a resident in a comprehensive program that offered a focus in developing clinical pharmacotherapy skills through clinical patient care rotations and also transition to a more pharmacy operations focused role through administrative and MSO specialty rotations. Sarah Emanuele PGY2 Residency Project: Auto verification of procedural medication orders Career Interests: After residency I will look for a career focused on implementing, redesigning and improving operational systems. I am interested in increasing patient safety and facilitating the progression of pharmacy practice through the expansion and integration of new technology support systems. This residency builds skills in multidisciplinary collaboration, team building and project management. I would like to continue to strengthen these skills throughout my career with a long term goal to provide consulting services to various pharmacy departments and enterprises. Hobbies: In my leisure time I like cooking, listening to music and reading. During the four months of warm weather we get in Wisconsin I enjoy hiking and gardening. Why UW? There are many reasons to choose a leadership or clinically focused residency at UWHC. The residency programs have a longstanding history of success and influence on pharmacy practice nationwide. I was specifically drawn to this unique residency in medication systems and operations by how clinical practice, pharmacy operations (highlighting the use of automation and technology) and leadership are tightly integrated into this well-developed program.

10 PGY2 Specialty Residents Kristina Bennwitz Ambulatory Care PGY1: Community University of Wisconsin Hospital & Clinics Residency Project: Pharmacist transitions of care: measurement of prevented medication related acute care episodes and optimization of services Career Interests: Ambulatory Care, Specialty Pharmacy, Transitions of Care Hobbies: Cheering on the Badgers/Brewers/Packers, running and Wisconsin festivals in the summer Why UW? The UW pharmacy department is at the forefront of innovation in pharmacy and advancing patient care. Residents have opportunities to develop and implement practice change. Additionally, residents are involved with all aspects of pharmacy practice including medication use systems, supply chain, revenue, and direct patient care in the acute and ambulatory care setting. Jenny Dela-Pena Infectious Diseases PGY1: Detroit Medical Center Pharmacy School: University of Michigan Residency Project: Engaging pharmacists in improving antimicrobial stewardship: implementation of an alert-based clinical decision support system Career Interests: Antimicrobial stewardship, academia Hobbies: Yoga, reading, Michigan football, dancing, traveling and looking for new adventures! 11 Lauren McGinty Oncology PGY1: Tufts Medical Center Pharmacy School: Northeastern University Residency Project: Implementation and evaluation of a pharmacistled immunosuppressive dosing delegation protocol in an academic cancer center Career Interests: Hematologic malignancies, bone marrow transplant, and pediatric hematology/oncology Hobbies: Swimming, playing tennis, exploring Madison, and spending time with family and friends Why UW? After meeting the UW residents and preceptors, I immediately knew this was where I wanted to come for my PGY-2. The pharmacists here are passionate about advancing the profession and providing every patient with the best possible care. UW offers an outstanding variety of inpatient and outpatient oncology rotations and an amazing support system of preceptors and coresidents. It is the perfect environment for residency training, and I am looking forward to a challenging and exciting year! 12 Brooke Hofmeyer Solid Organ Transplant PGY1: Iowa City VA Pharmacy School: University of Iowa Residency Project: TBD Career Interests: Transplant Pharmacist, including a clinic and a research practice Hobbies: Swing and ballroom dancing, running, hiking with my dogs Why UW? UW Health has a challenging residency program and one of the largest transplant centers in the country. I felt as though everyone who interviewed me already knew something important about me, which made me feel welcomed. I know there are resources at UW Health to help me achieve my goals and I will be able to carry my experiences at UW Health with me always. Dan Ruhland Informatics PGY1: Froedtert Hospital Residency Project: TBD Career Interests: Informatics Hobbies: Baseball, softball, golf Why UW? The University of Wisconsin Hospital pharmacy department has been a leader in the national landscape of the pharmacy profession and the pharmacy residents have played an integral role in the advancement of the department. Personally, being interested in informatics, UW is a unique organization in that we were one of the early implementers of the Epic electronic health record and a number of the Epic foundation system applications were developed at UW. The ability to learn the field of pharmacy informatics among some of the innovators in the field within such an influential department is a great experience.
