Blanchard Ph.D. Executive Vice Chancellor This coded memorandum allows campuses to provide the annual reporting required by the 2015 Student Health Services Systemwide Audit Report The audit report required: For the Division of Academic and Student Affairs to inform campuses each fall of changes to policies related to health-care services provided in association with academic degree, certificate, and credential programs; and For the campuses to investigate and certify compliance with policies and, if applicable, report timelines for achieving compliance by spring term of the same academic year. A reporting and certification form is included below, for your convenience. Attached to this memorandum are relevant policies: AA Clarifications to Executive Order 943 Policy on University Health Services and Executive Order 943 Policy on University Health Services. There have been no updates subsequent to the issuance of the July 7, 2015 AA coded memorandum. We are asking that by March 7, 2016, campuses please review health services and send to the [email protected] certification of campus compliance or (as applicable) certification of compliance efforts that will be concluded by the end of spring 2016: To complete your report, please return the first two pages of this memorandum, indicating: I certify that at all health-care services provided in (campus) association with academic degree, certificate, and credential programs comply with current California State University policies; or I certify that at all health-care services provided in (campus) association with academic degree, certificate, and credential programs except those listed in an attachment comply with current California State University policies.
