3 Bakersfield Gas Control Technologies /13/2014 $ 7, Burns Pacific Construction /26/2014 $ 12, Campuswide repairs made to gas leaks discovered as a result of the Natural Gas Pipeline Leak Assessment Survey. Southern California Gas Priority One rated leaks of natural gas distribution lines were located at the main gas lines in front of the Bell Tower (#2) and in the street/walkway sections of Camarillo Street; both areas were excavated to locate the line and welded repairs to the 2 plastic lines and valves were made. Emergency repair of the broken domestic water line that feeds Ironwood Hall (#24) due to age; there is an existing storm drain line at the same location (right above the water line) that needs to be replaced in order to access the water main. Repaired storm drain in North Quad due to a storm drain main line collapse in need of repair to prevent flooding prior to storms. Work included labor and materials to remove corroded material and replace with new pipe and valves. Leaking hot water underground pipes between North and South Gyms caused ground saturation and no heat or hot water. Work included labor and materials to remove corroded material and replace with new pipe and valves. Replaced HVAC system, duct work and added backdraft damper at president's residence. Existing equipment and ducting was aged and corroded. Temperatures exceeded 100+ degrees in Fresno at time of failure. Leaking hot water underground pipes outside of Kremen Education Building caused ground saturation and no heat or hot water. Work included labor and material to remove corroded material and replace with new pipe and valves. Main Leak on Earl Warren Drive. On Friday 8/15/14, an underground 4" irrigation main was found to be leaking. On Monday, 8/18/14, repair work began and was completed on Wednesday 8/20/14. Work required immediate attention as it affected the entire irrigation system on the West side of campus and dorm residents directly adjacent to Earl Warren Drive were scheduled to start arriving on Friday 8/22/14. One month rental of condensate air cooled pumps to 28 classrooms. PPFM's normal procedure of providing box fans to classrooms that do not have air conditioning was not effective this summer due to record heat waves and high humidity. Due to abnormal temperature and humidity, instructors were not able to conduct class proceedings. 2/3/2015 Page 1

Roofing contractor took care of roof and remediation, while emergency contractor response dried/cleaned and replaced drywall as necessary. Chiller lost all refrigerant. Repaired leaks and provided refrigerant for York Chiller at the Library (#508). Repaired swimming pool (#100) plaster, striped and reinstalled plaster in approximately 6x10 area, work done underwater. Emergency repair necessary to keep pool open for classes. In January 2014, the university experienced 2 simultaneous Central Plant heating hot water hydronic pipe breaks during the coldest days of the year. The pipe breaks resulted in the loss of HVAC heat in 6 major campus buildings. Both pipe breaks were on Etiwanda Avenue. Northridge CA Rasmussen /2/2014 $ 14, Street repair due to central pipe repairs above. Pomona General Pump Company /28/2014 $ 5, Emergency services required to repair reclaim water pump station near Custodial Services (#48). This was the only pump in service and it failed from a motor short. If no reclaimed water is available in the reservior, there is no supply to: agricultural crops, livestock and campus land irrigation. Immediate remedial measures were required to protect property and/or the public's health, safety and welfare. The Trenchless Company /4/2014 $ 44, Service to repair drain line in food service area of the University Union Building due to main kitchen pipe collapse. Service included dry down and clean up of area. Repair leaks in main pool heater; required new burners and gasket. Pool must be heated for academic classes. Main pool heater sprang a secondary leak immediate repairs not performed as facilities had a PE Boiler Retrofit project in progress which could comprise the repair. Facilities rented a portable pool heater to use from April-June during the retrofit project. Upon completion, heater was repaired. Pool must be heated for academic classes. Student Housing at Arrowhead Village (#4) Services to students with disabilities required ADA compliant access to building #4 for wheelchair bound student residing in apartment during summer classes; installation of activation bars on building entries for egress and ingress into the building, exit doors by bedroom, laundry room and elevator lobby. Removed and replaced failed 3-ton HVAC equipment Building Temporary Classroom 302; Health and Safety for lab animals; temperature critical for academic program. Replaced hotwater at Arrowhead Village (#4) circulation line/valve; leak underground required water removal and excavation to locate leak. Replace broken sensors in emergency generator fuel tank at University Hall (#26) for appropriate ventilation; removed existing tank and installed new tank with new wire and piping to accept new tank installation. 2/3/2015 Page 2
