1 To: All Project Vendors & Contractors RE: PROSPECT (Area/Block Well No. ) EMERGENCY RESPONSE/OPERATIONAL SAFETY PROCEDURES INTERFACE DOCUMENT This Interface Document is supplemental to an Apache /Drilling Contractor Contractor/Vendor Bridging Document executed between Apache and Drilling. The agreement has an effective date of DATE and specifies the expectations regarding safety and environmental management between Apache s Safety & Environmental Management System ( SEMS ) and Drilling Contractor safety and environmental policies and practices. Your company is defined by Apache as a service company ; meaning that you are a contractor or vendor other than Drilling who is retained by Apache and have personnel working aboard the semisubmersible drilling rig. Accordingly, you should by now have received and executed an Apache/Service Company Contractor/Vendor Bridging Document which shall govern the emergency response, operations, and safety procedures to be followed during the drilling and completion of the Prospect. The purpose of this Interface Document is to provide you and your personnel with an understanding of how the Apache (Lease Operator) and (Drilling Contractor) emergency response and operational safety policies and procedures mesh together and function. Please ensure that all supervisors receive copies and discuss the contents with their respective crews. The primary objective is that all work be performed safely, in compliance with regulatory requirements and in an environmentally responsible manner. This goal can only be achieved through the cooperation of all parties involved. The manuals and other documents referenced are available onboard the drilling rig, at the shorebase, and at Apache s and Rig Contractor offices. All supervisory personnel involved in the Prospect drilling and completion program are expected to obtain and review the relevant documents. 3 1. EMERGENCY PROCEDURES Apache and Drilling Contractor have adopted Emergency Procedures as contained in the following types of documents: A. Drilling Contractor Emergency Management Plan. B. Drilling Contractor Safety Policy Manual. C. Rig Name Operations Manual. D. Drilling Contractor Shipboard Oil Pollution Emergency Plan (to apply when not operating on Apache s lease). E. Apache s Regional Oil Spill Response Plan (to apply at all times when operating on Apache s lease). G. Apache s Emergency Evacuation Plan as filed with the U.S. Coast Guard. The above documents have been made available to personnel at Apache s and Drilling Contractor offices, the shorebase, and on board the drilling rig. It is the responsibility of all parties to be familiar with and to comply with the applicable emergency procedures. 2. SAFETY ORIENTATION / INDUCTION All personnel will receive a thorough safety orientation/induction upon initial arrival onboard the drilling rig. All personnel will be required to acknowledge their understanding of important safety information by signing the induction checklist. 3. MANAGEMENT OF CHANGE 3.1 Changes to well permits and regulatory approvals are the responsibility of Apache s Environmental & Safety Dept and/or regulatory consultant (J.Connor Consulting). No changes shall be implemented without authorization by the Apache s Well Site Supervisor acting in conjunction with Apache Management. 3.2 Changes to the mooring system, stack and riser design are the responsibility of Drilling Contractor Engineering Group, except that Apache s Drilling & Completion Department will provide the design of any facilities or equipment not part of the normal rig components and necessitated by the prospect drilling and completion program. 4. PERMIT TO WORK The Drilling Contractor Permit to Work system will be used for critical operations. 4.1 The Drilling Contractor Hot Work/Welding policy will be used throughout the Rig Operations Phase of the project. A permit to work will be completed for all hot work/welding. 4.2 The Drilling Contractor Confined Space Entry procedures will be used throughout the Rig Operations Phase of the project. A permit to work will be completed for all confined space work. 4.3 The Drilling Contractor Lockout/Tagout (Isolation) procedures will be used throughout the Rig Operations Phase of the project. A permit to work may be required, depending on the nature of the job, and shall be at the discretion of the Drilling Contractor OIM. Page D-29-B-3

4 5. SAFE WORKING PRACTICES 5.1 All personnel are to participate in an ongoing safety awareness program. The objective is to recognize unsafe acts and apply immediate corrective action. Apache s stop work policy empowers anyone to Stop Work and shut the job down until the project can proceed safely. 5.2 Applicable safe work practices as per the bridging agreements including Job Safety Analysis (JSA) on specific tasks will be implemented to reduce risks to as low as practical. 6. HANDOVER 6.1 Drilling Contractor senior supervisors, such as Rig Superintendents, Toolpushers, Drillers, Barge Captains, Mechanics and Electricians shall continue using the Drilling Contractor handover procedures. 6.2 Written handover notes are required when the Apache Well Site Supervisor and other Apache supervisory personnel go off duty or off of the rig. As much as practical the work schedules of senior Drilling Contractor and Apache personnel will be staggered to minimize potential handover problems. 7. WELL CONTROL 7.1 Well control equipment, such as BOP's, risers, and diverters will be tested according to BSEE requirements and conditions of approval of the drilling permit application. Drilling Contractor test procedures for well control equipment will be used. 7.2 Drilling Contractor Well Control Procedures will be applicable during the Rig Operations Phase of the project. 7.3 Personnel who are assigned well control duties must be current on well-control training per BSEE regulations and have documentation of such training in their possession or the documentation maintained at a central location on the rig. 8. TRANSFERING PERSONNEL/HEAVY LIFT PROCEDURES 81 All crane operations will be performed in accordance with Drilling Contractor safety policy on crane operations, and applicable government regulations. 8.2 A Permit to Work shall be required for all heavy lifts as determined by the Drilling Contractor OIM in consultation with the Apache Well Site Supervisor. 8.3 The Apache Well Site Supervisor must be informed of all transfers of personnel on or off the rig by means of crane lift, and have an opportunity to check on sea conditions, personnel, and equipment before the transfer takes place. 9. MATERIALS MANAGEMENT Procedures for materials management can be found in the following documents: A. Drilling Contractor Material Control Manual B. Prospect Vendor Checklist C. Apache Rig Reference Manual maintained by the Apache Well Site Supervisor on board the Rig Name. D. Waste Management Plan and Record Book (Rig Specific) E. Oil Record Book (Rig Specific) Page D-29-B-4

THIRD PARTY EQUIPMENT (SERVICE COMPANY PROVIDED) 10.1 All third party equipment on location must comply with applicable governmental regulatory requirements and be in good workmanlike condition It is the responsibility of the third party contractor to provide their employees with all required personal protective equipment All third party equipment requiring electrical power will be inspected by Drilling Contractor electricians to ensure compliance with applicable electrical safety regulations All third party moveable pressure containing equipment will be inspected by the Apache Well Site Supervisor as it comes aboard and condition of the equipment documented Equipment test records and inspections on pressure containing equipment done prior to the equipment being sent to the rig must be reviewed and approved by Apache s Senior Drilling Engineer. The part numbers must be checked by the Apache Well Site Supervisor when the equipment arrives at the rig, and the equipment verified as fit for purpose All temporary pressure containing equipment placed aboard the rig will be inspected by the Apache Well Site Supervisor during rig up to confirm proper implementation of procedures for laying out and hooking up the equipment. 11. COMMUNICATIONS/REPORTING RESPONSIBILITIES 11.1 Information pertaining to the well program and related work, as well as safety, environmental, regulatory and other information will be forwarded to the rig via Apache and Drilling Contractor. This information is to be disseminated to affected personnel at the daily tour operational meetings. It shall be mandatory that all affected personnel attend these meetings. The Drilling Contractor OIM and Apache Well Site Supervisor will designate personnel who are to attend the daily meetings and will ensure that topics discussed are documented and list of attendees compiled The Drilling Contractor OIM and Apache Well Site Supervisor will jointly conduct weekly safety meetings Each company will send its own daily report to its respective offices. Apache Deepwate shall have access to Drilling Contractor daily reports Communications with the Rig Name will be through micro wave phone system, satellite phone system, fax machines and radio Personnel on board information will be included on the morning report and transmitted to Apache each morning Personnel responsibilities are as follows: A. The Drilling Contractor OIM is responsible for vessel safety. B. The Apache Well Site Supervisor is responsible for monitoring safety, environmental, and regulatory issues associated with the well, and lease requirements. Page D-29-B-5

6 C. Drilling Contractor personnel responsibilities are as defined in the Safety Policy Manual HSE-. D. The Apache Well Site Supervisor has supervisory responsibility for other Apache and service company personnel and activities related to the well program. 12. INCIDENT/ACCIDENT REPORTING AND INVESTIGATION 12.1 Drilling Contractor first report of incidents and accidents will be used during the Prospect drilling and completion program The Drilling Contractor OIM will provide the Apache Well Site Supervisor with copies of all incident/accident reports, and the Well Site Supervisor will include these as an attachment to the Apache report Apache may elect to make a separate investigation of serious injuries and/or significant actual or potential "near miss" incidents Apache s Environmental & Safety Dept. will be responsible for reporting incidents/accidents to regulatory agencies, civil authorities, and others as necessary. 13. DRUGS/ALCOHOL/CONTRABAND 13.1 The possession or use of illegal drugs, alcohol, and firearms is prohibited on Apache s offshore locations The Drilling Contractor and Apache drug, alcohol, and contraband policy and safety procedures will be used as guidance in dealing with prescription medications, as well as illegal drug use, impairment due to alcohol consumption, and possession of contraband. The policies and requirements will be explained during initial orientation aboard the Rig Name. Personnel who require prescription medications are advised that medical clearance by their personal physician, and/or Drilling Contractor medical representative may be required for certain types of prescription drugs (e.g., psychotropic medications) Personnel who fail to comply with the above-referenced policies and procedures will be removed from the offshore location. 14. MONITORING, INSPECTION AND REVIEW 14.1 The Apache Well Site Supervisor will monitor conformance with this bridging agreement on an ongoing basis Apache and Drilling Contractor may conduct a joint inspection of the rig during the course of the drilling project. The inspection will take no more than one day and will be a general inspection looking at safety and environmental related equipment on the rig as well as general condition and housekeeping The Drilling Contractor OIM and Apache Well Site Supervisor will be responsible for developing and implementing an action plan to correct any deficiencies noted in the inspection.
