26 The human factor In the Petro-(chemical) and power industry, the majority of incidents have been primarily attributed to human error. The human error is most often caused by a mismatch between human and machine behaviour. Detailed investigations of serious large scale industrial accidents have shown that so-called human error needs to be viewed in the wider context of the circumstances created by organisational and management structures.
