chairman s message trust those who nurture hope, nurture life Our vision to ensure that no kidney patient will perish because of the lack of fund for dialysis In 2005, during the tenth year of our operation, the world of charity in Singapore was rocked by a scandal of unheard proportion. Being the second largest kidney foundation locally, the Kidney Dialysis Foundation (KDF) was brought into the limelight. All our activities became an open book, as highlights of our fund-raising, dialysis service; commercial practice and governance were made known to the public. There was instant increased awareness of the role of KDF in our community. As a continuum of the above exercise, KDF is proud to announce the following figures for Financial Year (FY) 2005/06: 21 full time staff helped to raise $3.78 million; 250 patients received affordable dialysis treatment, eight patients had successful kidney transplant; 79 per cent of our expenditures was used for dialysis services and education, 12 per cent for fund-raising, eight per cent for administration, and one per cent for publicity; the average KDF subsidy was $620 per patient month in our Haemodialysis (HD) programme, and the cost of each dialysis was at the competitive rate of $152. This fiscal year also saw a reversal of our fortune in fundraising. Thanks to the support of the general public, we had a surplus of $1.65 million, thus making good the deficit incurred in FY 2004/05. The accumulated reserve fund was $10.6 million, which would enable us to continue our operation for about four years, if we continue to receive public donations and support from the Ministry of Health (MOH). These revelations also illustrated our founding principles of providing high quality and cost-effective dialysis treatment to the needy, maintenance of a lean staff, targeted fundraising, and avoidance of accumulation of excess fund. In short, our practice to maintain transparency and accountability in all our activities has come to fruition. Our HD facilities were reduced due to the expiry of the lease and closure of the KDF-Alexandra Centre. This has put strains on our two other centres at Bishan and Kreta Ayer; both of them are operating at almost ninety per cent capacity. A replacement centre at Ghim Mohis at the planning stage, and is slated to open early In the meantime, our new dialysis patient intake for the needy is restricted to three per month, two for HD and one for Peritoneal Dialysis programme. As this message goes to print, KDF is celebrating its tenth anniversary in a modest way. Our vision to ensure that no needy patient will perish because of the lack of funds for dialysis has been fulfilled, directly by KDF treating these patients, and indirectly, by influencing other kidney foundations to change their policies and participate in our mission. Our ideals, vision, belief and practice have been realised and vindicated. It goes without saying that our modest achievements have been made possible by the generous support of the public. We at KDF are humbled and privileged by the opportunities given to us to achieve these life saving missions. I am ever thankful for the assistance of and subsidies from the MOH and for the unflagging support of our Board members, volunteer doctors and staff, and friends of KDF. Together, we will strive to continue our never ending mission of saving lives. DR GORDON KU FOUNDING CHAIRMAN 01
aboutkdf KDF was set up in February 1996 by Dr Gordon Ku to provide an alternative subsidised dialysis treatment to needy patients. The Foundation is an independent, notfor-profit charitable organisation not affiliated to any kidney foundationsin Singapore. It helps to nurture hope and confidence to make life more meaningful, even for the most needy. KDF does not have a chief executive officer (CEO); an Executive Committee replaces the functions of a CEO and is responsible of overseeing the organisation s operations. The Board of directors and Executive Committee are all volunteers and do not receive any remuneration. As at 31 March 2006, KDF has a total of 21 full time employees. The foundation helps to nurture hope and confidence to make life more meaningful ourvision To ensure that no kidney patient will perish because of the lack of funds for dialysis. ourmission To look after the well-being of needy people stricken with end-stage kidney disease by nurturing hope and confidence to make their lives more meaningful. ourcommitment To provide high quality, low cost treatment to needy kidney patients To offer patient support services to all kidney patients in Singapore To promote public awareness and education of kidney diseases To organise educational programmes on renal-related issues for healthcare and medical professionals To promote live-donor organ transplant 02
trust those who nurture hope, nurture life ourmanagement policy Since its inception, KDF has implemented a number of policies to ensure transparency and that donation funds received are utilised directly for patients treatment and not for costly overheads and miscellaneous expenditure. A fixed-cost subcontracting system allows for minimal administration and overhead costs. To keep costs low while providing quality treatment, KDF keeps its management team lean and trim with a large team of volunteers. Accountability extends to our financial statements that are easily available for scrutiny from our website at and upon the request of donors. ourdialysiscentres KDF started its first Haemodialysis (HD) centre, the KDF- Alexandra Centre, situated in Alexandra hospital in The KDF-Alexandra Centre ceased operations when the lease of the location ended in mid To accommodate the rising number of needy patients, more centres were built over the years. The KDF-Bishan Centre was opened on 20 November 1997 by Mr Wong Kan Seng, Minister for Home Affairs and Member of Parliament for Bishan- Toa Payoh GRC. The Centre is easily accessible to patients living in the northern and central parts of Singapore. KDF keeps its management team lean and trim with a large team of volunteers On 9 October 2004, KDF inaugurated two new dialysis centres - San Wang Wu Ti (SWWT)-KDF Centre and KDF-Peritoneal Dialysis (PD) Centre located at Kreta Ayer Road. President Nathan and Mrs Nathan were invited to grace the occasion as KDF's Guests-of-Honour. Other distinguished guests included Mr Khaw Boon Wan, Minister for Health, and Dr Lily Neo, Member of Parliament for Jalan Besar GRC. The haemodialysis centre, SWWT-KDF Centre, was so named because of the generous contribution by its disciples and devotees of the Society towards the full cost of building and maintenance of the Centre. The establishment of KDF-PD Centre was made possible with the generous combined donation from the Khoo Foundation and Singapore Pools (Pte) Ltd. 03

ourpatients Since its inception, KDF has strived to provide treatment and support to those who are in need so that no one should be denied treatment just because he or she could not afford it. All patients are assessed and referred to KDF by medical social workers from the restructured hospitals. Most of them hold jobs such as cleaners, hawker assistants, store assistants, taxi drivers, etc. In the financial year (FY) 05/06, some patients paid as low as $130 per month out of their pocket after subsidy. In fact, 93% of all patients paid less than $600 per month in cash as compared to the full monthly cost of more than $2,000. 10 education&treatmentcare KDF constantly plays an active role in promoting education on renal-related issues to the medical fraternity, students, patients, and preventive education to the general public. In addition, KDF has a sizeable pool of volunteer nephrologists from both the public and private sectors. They review the patients in the dialysis centres at regular intervals at no additional cost to patients. The Foundation endeavours to provide better treatment care to our patients. As part of KDF s effort to provide holistic care to the patients, it initiated the Subsidised Erythropoietin Injection Programme in This programme helps alleviate the associated problems of anaemia such as fatigue, poor appetite and insomnia, enabling patients to lead a normal lifestyle. As at 31 March 2006, over 60% of KDF s patients have benefited from the programme. In 2005, it introduced two more subsidised medications Calcijex and Venofer to enhance patients treatment. 1) Live Donor Transplant Programme Share a life Launched by the Society of Transplantation on 12 November 2005, Share a Life aims to build support for live donor kidney transplantation by educating the public on the needs and benefits of live donor transplants. KDF has set aside close to $80,000 to support a) Grant for donors surgery, b) Bridge to Transplant programme, and c) Publicity and Education. The fund contributes to helping donors from low income families pay for their hospitalisation for the donors surgery and provides subsidised interim dialysis to the patients while they prepare for transplant. 2) Sale of Injection Calcijex and Venofer Since April 2005, injection Calcijex and Venofer were made available at a subsidised rate to patients. In chronic renal failure, the kidneys produce insufficient vitamin D. Calcitriol (Calcijex) is a man - made form of vitamin D. Vitamin D is necessary to maintain the right calcium level in the blood for strong bones. Venofer is a type of iron sucrose used in the treatment of iron deficiency in dialysis patients. 3) Continuous Quality Improvement A team comprising the charge nurses of the centres and KDF nursing personnel under the guidance of KDF Medical Directors continued to monitor the CQI of patients KT/V, or dialysis adequacy. All centres achieved a dialysis adequacy of more than 80% KT/V >1.3, where the ideal clearance level of KT/V is about 1.2. The overall first year survival of HD patients was 95.4% and five year survival 84.5%. 4) Public Forum: Diabetes and the Kidney The Public Forum was held on 25 June 2005 at Suntec Singapore. A full house of 1,500 participants spent an afternoon learning about diabetes and the kidney. The Forum chairperson was Dr Grace Lee. Speakers from Singapore, Denmark, Hong Kong and the US helped to raise public awareness for diabetic nephropathy. The forum was held as part of KDF public education to raise awareness on kidney diseases. 08
trust those who nurture hope, nurture life 9) Staff Competency Check A yearly competency check on the dialysis procedures is conducted on the service providers staff. This is done in collaboration with the service providers charge nurse. The purpose of the audit is to ensure that the standard of practice is maintained in accordance with the KDF protocols and guidelines. 5) Basic Renal Nursing Education The sixth intake of the Basic Renal Nursing Education course ran from 7 July to 22 September A total of 14 staff from the HD centres and staff from the service provider attended the course. Two new topics were incorporated; they were basic understanding of continuous renal replacement therapy and dialyser re-use program. Continuous Nursing Education points were awarded to the registered nurses. 6) Continuous Renal Nursing Education Continuous renal nursing education was conducted for the dialysis technicians and the enrolled nurses. It is to provide adequate knowledge with understanding towards the care they are providing to the patients. 7) Glucometer Monitoring New staff was trained on how to monitor patients blood glucose level using the Advantage glucometer and how to assess and act when complications arise. The training is on-going. A yearly re-certification test is conducted to maintain the standard. 10) Audits On Infection Control Audit on the dialysis centres infection control measures is conducted half yearly. This is to ensure that the patients in KDF receive the standard of practice and care according to KDF Medical and Nursing Standards. KDF nursing personnel together with the charge nurses performed the audit. 11)Nanyang Polytechnic Post-Advanced Diploma Course Student (Nephro-Urology) Fifteen students had their orientation in July and clinical attachment for a period of two weeks in October 2005 at both the San Wang Wu Ti and Bishan centres. 12)Nanyang Polytechnic Post-Diploma Certificate in Nursing (Diabetic Nurse Educator) The post-diploma students (diabetic nurse educator) visited the Bishan Centre in November The purpose of the visit was to gain an understanding of the dialysis process. 13)Administration of Flu Vaccine KDF once again partnered with the Renal Transplantation Department of the Singapore General Hospital to provide Influenza vaccination to kidneytransplanted patients in September The exercise benefited 230 patients. 8) Intravenous Administration of Medicines A yearly re-certification on oral and practical test is conducted to ensure that the registered nurses are equipped to perform the extended role of administrating the specific intravenous medicines according to KDF guidelines. The KDF nursing personnel and charge nurse of the centres will keep a register of the approved staff for giving intravenous medicines. 09
renalfriends A quiz was conducted at the end of the talk. After which, a sumptuous lunch was provided in a restaurant. After lunch, prizes were given to patients who had answered all questions correctly. A token was also presented to patients who have participated in the skits. The seminar was sponsored by Roche Singapore. Renal Friends Always There To Show We Care RENAL FRIENDS (RF) is a patient support group initiated by Medical Director of KDF, Assoc Prof Lina Choong for all kidney patients and their families in Singapore. Renal Friends aims to bring kidney patients together for interaction and mutual sharing. It consists of committed and enthusiastic volunteers who include kidney patients themselves. Through the kind support of sponsors, KDF is able to arrange for several excursions for the patients and their families. 1) Patient Education Seminar The Renal Friends Patient Education Committee organised a Patient Education Seminar on Pills and Tablets: Am I taking it right! on 24 April 2005 at SAFRA Yishun Country Club. The 143-strong audience was made up mostly of patients from the KDF dialysis centres, Singapore General Hospital (SGH), National University Hospital and People s Dialysis Centre. Ms Lee Puay Hoon, senior pharmacist from the SGH was invited to give the talk in both English and Mandarin. She spoke on the common medications taken by kidney failure patients who are on dialysis so that they would be able to understand better on the medications taken. Besides the talk, patients relayed the medication-compliant message animatedly through skits. 2) Patient Education Seminar-cum-Outing Social and family issues are common problems faced by many patients afflicted with chronic illness like kidney failure. Coping with chronic illness is very stressful to patients and their family. A seminar that focused on social and family relationships was organised on 25 September 2005 at SAFRA Mount Faber for patients and their family members. The seminar was delivered in English and Mandarin by medical social workers, Ms Jessie Chai and Ms Goh Siau Ching from the SGH. The participants spent a meaningful morning listening to the talk and getting tips on how to improve relationships within the family and socially. After the seminar, a balloonist entertained the audience with some short performances and adorable cartoon sculptures. Soon after, the participants eagerly dispersed into five groups of handicraft workshops, which included paper craft, paper flower, origami, friendship band and balloon sculptures. Participants picked up some skills on making simple balloon sculptures, paper cranes, colourful paper flowers and delicate friendship bands. It was a whole new experience for all participants. Right after the sessions, participants partook in a scrumptious lunch. The seminar and outing were sponsored by Roche Singapore. On 9 October 2005, Zhu Mi Mi and a few veteran artistes like Chen Ming Feng gave a 2-hour long performance at Downtown East. The audience, especially KDF s patients, was greatly entertained by their songs. During the festive season from 20 December to 28 December 2005, a sparkling ensemble of actors and dancers from Europe, and the cute and adorable Lilliputians acting as dwarfs, transformed the well loved children s classic, Snow White to life on the big stage. Mixing musical theatre with comedy and dance, the fairy tale unfolds in a whole new light which garnered much laughs, thrills and fun for the audience, especially the children. The patients and their family members were extremely happy to get the chance to attend these two great performances. We at KDF are humbled and privileged by the opportunity given to us to achieve these life saving missions 11
