Nrthwest Highway, Suite 418, Palatine, IL New Members shuld attach this frm t their Applicatin. Existing members shuld submit the signed frm, with payment, t the Assciatin ffice. PROCEDURES FOR HANDLING REFERRAL REQUESTS: When ptential clients call the NWSBA ffice, a staff member screens the call t determine the area f practice required t handle the matter and the gegraphic area f the caller. The caller is then tld that there wuld be a $25 fee fr a cnsultatin up t 30 minutes, due at the time f the cnsultatin. They are als infrmed that any wrk dne n their behalf by the attrney wuld be billed at the attrney s standard rate. The caller is tld that it is nt a pr bn prgram, nr is it a reduced fee r legal aid service. If the terms are agreed upn, the NWSBA staff member takes the name, address and telephne number f the caller and begins the search fr a referral attrney. Sme clients prefer t see an attrney clse t hme, thers clser t their place f emplyment. In sme instances, callers have limited access t transprtatin and request smene alng a public transprtatin rute. Once the attrney is identified, the caller is given his/her name, address and telephne. The caller is instructed t call the attrney directly t set up the cnsultatin. A fax is then generated by the Assciatin ffice and sent t the referral attrney t alert them t the incming referral. GUIDELINES FOR REFERRAL ATTORNEYS: If/when the ptential client calls the referral attrney s ffice, the referral attrney handles the client in the same manner as any ther client. The referral attrney sets up the cnsultatin. The $25.00 fee is cllected at the time f the cnsultatin. The client shuld be instructed t make all checks payable t the NWSBA. If anther frm f payment is made, the referral attrney shuld reimburse the NWSBA directly. If the referral attrney and client wish t prceed, any wrk beynd this pint is billed at the attrney s standard rate schedule. This must be cmmunicated and agreed t by the client. If the referral attrney wishes t assciate with c-cunsel, the referral attrney must retain primary respnsibility fr the case. N attrney member may refer any case given t them thrugh the referral prgram t anther attrney. If the attrney is unable t handle the matter, the attrney must infrm the client and the Assciatin ffice immediately. The client shuld be instructed t call the Assciatin ffice and anther referral will be prvided. Vilatin f this rule will result in the permanent remval f the referral attrney frm the referral service. Nthing in this rule will prevent an attrney frm assciatin with c-cunsel, regardless f whether said c-cunsel is a member f the NWSBA, prvided that the riginal attrney member remains primarily respnsible fr the matter.

2 If the referral did nt result in a cnsultatin, it shuld be nted n the fax and returned t the Assciatin ffice. The referral attrney shuld re-direct the client back t the NWSBA t arrange fr a new referral. If the referral is accepted, the attrney must return the faxed frm t the Assciatin and fllw up with the submissin f the $25.00 fee within 5 business days f the cnsultatin. Failure t remit the fee will result in the remval f yur name frm the referral list until all requirements have been satisfied. A 10% administrative fee will be assessed by the NWSBA n fees, ver $1,000.00, generated as a result f a referral invlving Trt cases. EXAMPLE: a $2, fee t an attrney wuld result in a 10% administrative fee f $ (e.g. $2, $1, = $1, x 10%). REQUIREMENTS TO PARTICIPATE IN THE REFERRAL PROGRAM: An applicatin must be submitted t the NWSBA ffice. A current cpy f yur malpractice insurance declaratin page and yur ARDC card must be n file in the Assciatin ffice at all times. Failure t update insurance recrds will result in the remval f yur name frm the referral list until all requirements have been satisfied. It is the respnsibility f the attrney t keep their recrds up t date. Attrneys must remit the $25.00 cnsultatin fee t the Assciatin ffice after each initial cnsultatin. FEES: Billed semi-annually in cnjunctin with Membership Dues (dues and fees are prrated fr initial membership nly).
