5 GENERAL INFORMATION Registration Information Check the class listings in this catalog for dates, times, fees* (if applicable), and class locations. Information is subject to change. Our classes are open to all Kaiser Permanente members, and some classes are open to the community. Classes are filled on a first-come, first-served basis, and require preregistration unless otherwise noted. A session is defined as the number of classes a participant should attend to get the most value from the educational experience. The number of sessions does not imply a limit to the number of classes a Kaiser Permanente member may attend. For information or to register, visit your local Health Education Center or call the number listed underneath the course description. Refunds will be given for cancellation requests made at least 72 hours prior to the class start date. On rare occasions, a class may be canceled and a refund will be issued. This overview explains the signs and symptoms of chemical dependency, how lives are affected by substance use or abuse, and offers resources and treatment options. Open to the community. We offer programs for adults and teens, as well as support services for families. Services are based on an individualized treatment plan. Available in Spanish. Cancer Education Survive and Thrive Cancer Survivor Program The Survive and Thrive program at Kaiser Permanente Santa Clara can help nourish and strengthen you in mind, body, and spirit during all stages of cancer treatment and survivorship. These classes and workshops are open to all cancer patients members and nonmembers and several are open to family members and caregivers. Survivor... Now What? The conclusion of cancer treatment can be a challenging time. With the rigors of treatment behind you, this class examines potential challenges and issues that might arise as you move forward. This class is intended to provide a bridge between life during treatment and life as a survivor. For patients who have completed active treatment. No class fee for members; $20 nonmembers. 8 Mind-Body Stress Management This eight-week class will help empower you with effective skills to help manage the unique stressors of cancer treatment and/or post-treatment. Learn to reduce stress, foster self-care skills, discover new thinking patterns, and enhance your quality of life. Open to the community, including family members and caregivers. No class fee for members; $95 nonmembers. Sessions: Eight 2-hour sessions Information: Call Nutrition Basics This class will help guide you through the benefits of healthy eating and the effects of nutritional changes in survivorship and general wellness. Learn to identify healthy choices and how to make them everyday habits. Recommended for patients who have completed active treatment. Open to the community, including family members and caregivers. $20 members; $40 nonmembers. Sessions: One 2-hour session Information: Call coming soon! Nutrition Basics Expanded This comprehensive class focuses on the powerful benefits of plant-based nutrition for cancer prevention. You will learn simple ways to transition to a plant-based diet, along with delicious easy recipes, cooking demos, and tastings. Open to the community Sessions: Three 2-hour sessions Information: Call Restorative Yoga Learn how breath work, guided meditation, gentle movements, and supported resting postures can help you improve circulation and reduce stress during cancer treatment and in survivorship. This class is especially beneficial for those going through treatment. $60 members; $120 nonmembers. Sessions: Six 1-hour sessions Information: Call Strength Training: Body & Mind Everyday activities are made easier by increasing your balance, core strength, and flexibility. In this six-week series you will learn to strengthen and tone your muscles using an assortment of fitness props and weights. 9 Diabetes Management Prediabetes Learn how to help prevent or delay the onset of diabetes by eating well and exercising. These healthy lifestyle changes will help you take charge of your health to reduce your risk of diabetes and its complications. This is a covered benefit for members. Portable Electronic Medical Record With a Portable Electronic Medical Record (PEMR) flash drive, any physician with a computer can review a summary of key portions of your recent medical history including your allergies, office visits, lab results, and X-rays. The device is $5 and there is no charge updates. To get a PEMR flash drive, go to the Medical Secretaries or Member Services Department at the facility nearest you. Diabetes Basics Feel better and be active and healthy by learning the basics of managing your type 2 diabetes. Explore the five key areas of type 2 diabetes management: healthy eating, exercise, monitoring your blood sugar levels, proper use of medication, and managing stress. Available in Spanish in Milpitas and Santa Clara. This is a covered benefit for members. Learn new ways to prepare your favorite foods as well as how to enjoy eating out healthfully. This is a covered benefit for members. Blood Sugar Monitoring Learn how to test your blood sugar using a blood glucose meter. Please bring your OneTouch Ultra 2 meter and testing supplies to your appointment. No class fee. This video series will teach you how to prepare and inject insulin and understand why insulin is one of the best tools available to manage your blood sugar. We ll assign a diabetes care manager to team up with your physician to help you control your diabetes. It includes short videos that demonstrate posture tips and strengthening exercises to help you manage your pain now, get better faster, and reduce the risk of pain returning in the future. Heart Health for South Asians In this class, you will learn how people from India, Pakistan, Sri Lanka, Bangladesh, and Nepal are at greater risk for cardiovascular disease at a younger age. A health care professional will help you identify self-management skills and how to make changes that can help lower your risk and lengthen your life. No class fee.
