1 Services available to people without a Family Physician or Nurse Practitioner Most people receive the majority of their primary health care through their family physician or nurse practitioner. There are services available to people to both supplement the primary health care they receive from their physician or nurse practitioner and these services are also available to those without a family physician or nurse practitioner. Accessing primary health care: - Hospital emergency departments are available to all requiring emergency care as well as to support those people without a family physician or NP for care such as prescription refills, referrals for tests, etc. - Non-emergency medical issues can be discussed with a registered nurse 24/7 by dialing Community pharmacies often hold blood pressure monitoring and blood glucose (sugar) monitoring. Call your local pharmacy to find out what services are available in your area. - Women (aged 40 and over) can access screening mammograms by calling for an appointment. Screening mammograms are done at Yarmouth Hospital Diagnostic Imaging department and on the mobile breast screening van. Women can self refer for a screening mammogram. - Victoria Order of Nurses (VON) offers a wide array of in home and community based services. Foot care clinics, wound care, in home nursing support, meal programs, hospice/palliative care, etc. Contact VON for more information Continuing Care (Home Care) offers home support (such as personal care, respite, and light housekeeping), nursing (such as dressing changes, catheter care, and intravenous therapy) and home oxygen. Call Pregnant women can self-refer to the Prenatal Clinic located on the Maternal Child Unit of Yarmouth Regional Hospital. Call ext Wound and Ostomy Clinic provides inpatient and outpatient care for acute and chronic wounds and ostomy care. Wounds such as leg and foot ulcers, skin tears and surgical wounds are treated. Call the Wellness Centre at ext or the resource nurse ext Clinics are held at Yarmouth Regional, Digby General and Roseway Hospitals. - Cardiovascular Care Clinic supports people to keep blood vessels leading to your heart and your head open and healthy. Offered through the Wellness Centre located at Yarmouth Regional Hospital and twice monthly in Meteghan call ext or ext Diabetes Education Centres help people of all ages who are at risk or have diabetes. Located at each hospital site; call for an appointment. Digby ext Shelburne ext. 230 Yarmouth ext Rehabilitation Services for injuries/conditions that have impacted mobility. Located at each hospital site; call for an appointment.
2 Digby ext or ext Shelburne ext. 204 Yarmouth ext Palliative Care Program referrals can be made by can be made by self, a family member (with patient consent), physicians, and etc. the only requirement is that the patient must give their consent and that they are palliative. The patient can see a nurse or a social worker in their home setting or in office. They can also access a volunteer. They can access the services of our palliative care physician with a referral from a physician. Support Care Program offers medical social workers that can be accessed through the same process. Yarmouth Shelburne Digby Addiction Services provides support for people experiencing problems or support to quit alcohol, other drugs, tobacco, or gambling. Toll free or call the nearest office. Barrington Passage Shelburne Yarmouth Digby Clare Public Health Services provides support to families and individuals to prevent illness, protect and promote health and achieve well-being. Yarmouth County Digby Area Clare Municipality Shelburne Co Outpatient Dietitians can support people who want advice on a variety of nutrition concerns. Call your local hospital and ask for the outpatient dietitian.

You can make an appointment yourself or be referred by another health care provider. The clinic is on the second floor of Yarmouth Regional Hospital in the Prenatal Clinic area. 4 Heart/Stroke - Cardiovascular Clinic - Nurse Practitioner provides support to cardiovascular patients without a family doctor who are taking Warfarin and/or have hypertension. Located in the Wellness Centre at the Yarmouth Regional Hospital: Call Ext Live & Learn sessions are an opportunity to learn about cardiovascular (heart) disease: risk factors, healthy eating, exercise and activity, an opportunity for sharing and gathering informative take-home materials. Call ext Heart Healthy Class held monthly for those who want to learn how to make healthy food choices to reduce their risk of heart attacks and stroke. Held at Digby General Hospital, Weymouth Medical Center and the Clare Medical Centre call ext and leave a message. - Shelburne County Stroke Club for stroke survivors, families, friends and caregivers. Held the 3 rd Thursday of each month at Roseway Hospital call ext Yarmouth County Stroke Club for stroke survivors, families, friends and caregivers. Held the 1 st Tuesday of each month at Yarmouth Regional Hospital call ext Healthy Eating - Heart Healthy Eating session held monthly at Yarmouth Regional Hospital for individuals with high cholesterol and heart disease. Call ext for more information. - Nutrition Class is held a few times a year for anyone interested in learning about nutrition. Call ext. 239 for more information. 5 - New Beginnings Program is for individuals who turn to alcohol, drugs or gambling as a way to cope with life s challenges. Held once a month in the Detox Unit Program Room at Yarmouth Regional Hospital. Diabetes Yarmouth - Diabetes Management When on Insulin for insulin dependent diabetes patients and their families.
