2 2016 Master Course of Graduate School of Nutrition and Bioscience, Tokushima University Application Guidelines for Primary Admission In the Graduate School of Nutrition and Bioscience, we make efforts to train professionals who can be active in their fields (medical service and local society or industry) and researchers for educational research institutions. We give lectures of extensive education through teaching human nutrition including basic life science and various knowledge of medical care to students and also try to do research so that they can lead their field and contribute to society. We have a goal of contributing to the nutrition field through achievements of research to train students to contribute to society. <Admission Policy of Department of Human Nutrition Science, Graduate School of Nutrition and Bioscience> We admit those who have sufficient and high technical knowledge based on life science and medicine and want to contribute to the efforts of improving eating habits in society while maintaining health quality. - Students who want to become a registered dietitian of clinical management and have high technical knowledge and technique based on present medical science. - Students who plan nutritional policies to contribute to society and globally. - Students who are active in research and institutions of education in the nutrition field. I. Number of Students to be Admitted Department Strength Recruitment Human Nutrition Sciences *Those numbers are not for applicants who are privately funded foreign students. II. Qualifications for Applicants The following individuals are eligible to apply for admission. (1) Individuals who have graduated or are expected to graduate from universities regulated by the prescription on Article 83 of School Education Law by March 31, 2016 in Japan (2) Individuals who have earned or are expected to earn a bachelor s degree of universities regulated by the prescription in Paragraph 4, Article 104 of School Education Law by March, 2016 (3) Individuals who have completed or are expected to complete 16 years of school education overseas by March, 2016 (4) Individuals who have completed or expect to complete 16 years of education through 2

3 distance education from a school of education overseas by March, 2016 (5) Individuals who have completed a degree equivalent to the curriculum designated by the Minister of Education, Culture, Sports, Science and Technology from a foreign university and hold a bachelor s degree or equivalent thereof or are expected to complete their studies by March, 2016 (6) Individuals designated by the Minister of the Education, Culture, Sports, Science and Technology based on Article 155 Paragraph No. 6 of the Ordinance for Enforcement of the School Education Act (7) Individuals who have graduated or are expected to graduate from a technical or vocational school approved by the Minister of Education, Culture, Sports, Science and Technology (Individuals must have received an education of 4 years or more or equivalent of the curriculum approved by the Minister of Education, Culture, Sports, Science and Technology.) (8) Individuals who have completed at least 3 years of university education or 15 years of formal education, and have acquired the necessary credits with excellent records. (9) Individuals who our department accepts has graduated from university or has the scholastic ability to be admitted based on the entrance examination, and are 22 years of age or older as of March 31, (Refer to notes) (Note1) Application Qualification (9) 1. Applicants who are highly educated and have graduated from a university, or applicants who dropped out of their university (stayed and studied for more than two years) but have graduated from junior colleges or technical colleges, and finished programs of educational facilities, quit, or graduated from and have worked or been active in the field of nutrition for more than two years. 2. Applicants who are satisfy the requirements have to submit documents below to Graduate School Section, Educational Affairs Division, Faculty of Medicine, Dentistry and Pharmaceutical Sciences, Tokushima University by Friday, July 10, 2015for prescreening. - Certificate of your academic background - Forms to examine qualification to apply to the entrance examination - Papers which prove that you have experienced business or activities more than two years - Research report ( one page of A4 paper in English), - List of your career or papers of your research (optional) (Note2) Examinations for Privately Funded Foreign Students Only students who have ability to understand Japanese or English are eligible to apply. IV. Application Procedure 1. Documents to be submitted Application Form Ticket for Examination Official Transcript/Grade Report Diploma or Certificate of Graduation 4 Please fill in all required information on the form provided. (with a picture taken these days at the size of 4 5cm without wearing a cap or hat.) Please attach your identification photo. The photo must be recent, and must be of your upper body, facing forward with head uncovered. The size must be 5 cm height and 4 cm width. Official transcript prepared by your previous university or faculty. Certificate of Graduation or Expected Graduation, or Certificate of Completion or Expected Completion. Certificate of Degree Only individuals eligible based on qualification (2) must obtain and submit a Certification of Degree issued from the National Institution for Academic Degrees. Individuals who are expected to acquire a degree of specialized course of junior-college (2-year college) or technical college must attach expected graduation certificate or certificate of degree acquirement application. Proof of Examination Fee Payment The test fee is 30,000. After paying by postal transfer, please attach the receipt to the Proof of Examination Payment form attached below. For details, refer to (1) of 2.

Acceptance Letter Copy of Certificate of Alien Registration/Passport Stamped Envelope for Admission Ticket If you are an applicant residing outside Japan, payment by credit card is available by making an online application through International Student Portal of the University`s website. isc.tokushima-u.ac.jp//english/ Individuals who are currently enrolling in a graduate school or working in a company must submit an Acceptance Letter by an advisor or manager. Foreign nationals residing in Japan must submit a copy of certificate of matters in the alien registration records. Fix a 82 stamp to the envelope and write your name, address and postal code clearly. (unless to bring directly ) 2. Procedure (1)Please hand them in with other required documents by the deadline. (2)Please choose registered mail and write Application for the Graduate School of Nutrition and Bioscience, Master Course on the envelope with a red pen and General Foreign Student or Privately Funded Foreign Student V. Selection Method Selection of applicants is based on the overall results of the interview, achievement test and the academic transcript provided by their previous university. 1. Interview The interview will be held in Graduate School of Nutrition and Bioscience at 1pm on Tuesday, August 18, Transcript Sealed transcript issued by the principal of your previous university will be used as a materials for the selection. VI. Applicants with Disabilities Applicants for Graduate School of Nutrition and Bioscience with disabilities may need special consideration on the examination and need to contact the person in charge at Graduate School Section, Educational Affairs Division, Faculty of Medicine, Dentistry and Pharmaceutical Sciences, Tokushima University by Friday, July 10, VII. Announcement of Successful Candidates The results of the examination will be reported to all applicants on Monday, August 31, The results will be posted at the School of Medicine at 10 am and the applicant will also be notified by postal mail. Results by telephone inquiry will not be responded. VIII. Admission Procedure More information on enrollment and enrollment periods will be provided in early March IX. Entrance Fee and Tuition Fee 1. Entrance fee is estimated as 282,000 yen. 2. The tuition for the first semester will be 267,900 yen and annual tuition 535,800 yen (both are estimated amount). Notice: (1)In the event of a revision in the tuition fees, the new rate will be applied from the date of the revision. (2) The above fees are the currently estimated amount. If revisions are made, the new fees will be applied. X. Miscellaneous (1) After an application is submitted, fees cannot be returned. (2) Other costs (such as medical support fees) of 29,000 yen will be necessary at the time of entrance. Part of fees is not applicable for applicants who are from Tokushima University. (3) If sending application materials by postal mail please include self-addressed return envelope (33cm 24.0cm) with a 250 stamp (specifying Name, Address and Postal code) addressing to Graduate 6
