HealthSource of Ohio Job Description for: CERTIFIED NURSE PRACTITIONER-FAMILY PRACTICE (CNP-FP) REPORTS TO: Clinical: Supervising Physician Administrative: Medical Director SUPERVISES: No supervision exercised. CLASSIFICATION: 22 POSITION SUMMARY: Provides nursing and other health care services to individuals, groups, and families emphasizing health promotion and disease prevention, as well as diagnosis and management of acute and chromic diseases. The CNP-FP, in collaboration with physicians or podiatrists, may provide preventative and primary care services and evaluate and promote patient wellness within the nurse s nursing specialty, consistent with the nurse s education and certification, and I accordance with rules adopted by the Ohio State Board of Nursing (OBON) and Section 4723.43 of the Ohio Revised Code (ORC). The Certified Nurse Practitioner-FP practices through collaboration with, and under the supervision of, a licensed physician, within the framework of a jointly approved standard care arrangement, following established protocols, and in compliance with applicable regulations. Minimum Knowledge, Skills, and Abilities Required: EDUCATIONAL REQUIREMENTS: Bachelor s degree in nursing and a Master s degree from an accredited Nurse Practitioner program in the clinical specialty for advanced nursing practice. Certification through an accreditation body recognized by the Ohio Board of Nursing. LICENSURE: Holds a current license to practice nursing as a registered nurse in the State of Ohio. Holds a current certification in his/her specialty through an accreditation body recognized by the OBON. EXPERIENCE: Must have at least three years of clinical experience as a registered nurse, preferably in the capacity of an Advanced Practice Nurse. PHYSICAL: Must be able to carry necessary equipment. Must have full mobility. Must be able to communicate effectively verbally to majority of patient population and staff. Must be able to communicate directly with patients and staff in writing. Must be able to hear through stethoscope and other equipment necessary for diagnosis. Must have visual acuity that enables visualization of gross abnormalities and microscopic detail through a microscope. Must have knowledge of necessary tests, labs, and treatment for patient population or area of practice. Must possess ability to coordinate and prioritize multiple responsibilities. Must be able to maintain confidential information and exercise discretion. Must be able to comprehend and apply clinical information. Must be able to maintain complete, accurate, and detailed medical records. EQUIPMENT: Must have the ability to access medical records and charts. Must be able to communicate by telephone. Must have valid Ohio driver's license. Must be able to drive own vehicle to various Network offices and other destinations. Must be able to use medical equipment as necessary for patient contact, diagnosis, and treatment. Must be able to use medical transcription equipment. ESSENTIAL JOB DUITES: Within the framework outlined in the standard care arrangement and consistent with approved protocols, the CNP-FP performs the following essential job duties: Chart Documentation Administrative: 1. Dictates or writes all notes from each patient encounter, including history, physical examinations, diagnosis, treatment, and follow-up. 2. Documents information per Network protocol in designated areas of the chart. 3. Reviews laboratory reports and provides or designates appropriate action and follow-up. 4. Complies with Network policies and procedures. 5. Attends physician meetings, both for the entire Network and countywide meetings. Communicating Information 1. Effectively communicates with supervising physician, nursing, and office personnel. 2. Effectively communicates information to patients including instructions, diagnosis, treatment, and follow-up care. 3. Participates in appropriate meetings. 4. Provides patient education (both verbal and written) to appropriate patients. Professional 1. Maintains membership in necessary professional organizations. 2. Maintains certification and licensing requirements of the Ohio State Board of Nursing. 3. Participates in CME training required to maintain license/certification.
4. Maintains certification of courses required to provide appropriate patient care (ie. CPR, ACLS, PALS). Quality Assurance 1. Follows Network policy on QA and helps to assure other staff members follow policy. 2. Follows OSHA regulations. CNP-FP Clinical Duties 1. Performs well-child exams to include HEENT, neck, lungs, heart, abdomen, extremities, genital; orders vaccinations, lead level, Hqb/Hct, vision and hearing tests. 2. Obtains complete health history, including mental health; elicits details for acute and chronic illness, and records findings. 3. Performs work physical exams to include HEENT, neck, lungs, heart, abdomen, extremities, genital; orders vaccinations, lead level, Hqb/Hct, vision and hearing tests. 4. Performs sports physical exams to include HEENT, neck, lungs, heart, abdomen, extremities, genital; orders vaccinations, lead level, Hqb/Hct, vision and hearing tests. 5. Performs well-woman exams to include gynecological/papsmears/pelvic, breast exams, and STD tests. 6. Performs exams for other patients. Physical examination of all systems relevant to acute and chronic complaints. Technical Skills 1. Performs in-office surgical procedures to include the following: suturing, excision of lesions, incision and draining of lesion, foreign body removal (from ear, skin, vagina), injections (trigger point/joint) and electo dessication of lesions, with supervising physician approval. Diagnosis 1. Diagnoses various acute and chronic illnesses through findings from physical assessment and patient history. 2. Reads and interprets laboratory reports. 3. Conducts and interprets microscopic examination of specimens. Treatment 1. Determines treatment plan based on diagnosis within approved protocols. 2. Determines appropriate medication required to treat illness utilizing pharmacological knowledge, within approved protocols. 3. Provides appropriate anticipatory guidance for infants and children. 4. Determines when referral to supervising physician is necessary per protocols.
5. Determines when a referral to a specialist is necessary per protocols. Treatment (Continued) 6. Determines when specialized diagnostic tests are necessary to diagnose patient s condition per protocols. 7. Provides appropriate nutritional counseling for developing infants and children; provides counseling for conditions related to chronic disease (for example, HTN, DM, IBS), and for weight loss. 8. Provides health education and counseling with emphasis on health promotion and maintenance, along with disease prevention and management.
