The selected provider will work collaboratively with other Advanced Practice Providers, Medical Doctor and other members of the Interdisciplinary Team (IDT) to provide exceptional care to the elderly population of our program. The duties of this opportunity include performing physical assessments of new LIFE enrollees, semi-annual reassessments of participants, the development and implementation of appropriate plans of care to LIFE St. Joseph of the Pines program participants. The provider will also be responsible for evaluating and treating physical complaints appropriately. The qualified candidate will be licensed as an RN in the State of North Carolina and certified as a Nurse Practitioner by a national certifying organization recognized by the State of North Carolina Board of Nursing. Candidates must have prescriptive authority to be considered for this opportunity. Employment preference will be shown to the individual certified as a Geriatric Nurse Practitioner (G.N.P.).
Our ideal candidate possesses a thorough knowledge of physical, mental and social needs of frail older adults and of current concepts, theories and practices related to home and community-based care for the elderly and disabled adults. Additional skills needed for success as an Advanced Practice Provider within the LIFE program include strong organizational skills, effective oral and written communication skills, the ability to lead and work within an interdisciplinary setting and the ability to manage changing priorities per needs of the PACE program and SJP. SJP - LIFE has an abundance of convenient services onsite, including a laboratory, pharmacy and mobile x-ray unit. The program also utilizes Mediture's TruChart electronic medical record, therefore previous experience with EMR's is preferable. This opportunity offers the following variety of duties to be completed and/or monitored by the Medical Director. Administrative Functions 15% Advises the Medical Director and Primary Care Physician in ways and means to establish better accountability of LIFE St. Joseph of the Pines services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands. Maintains participants medical record and fulfills LIFE St. Joseph of the Pines charting and reporting requirements. Maintains appropriate diagnostic coding. Protects privacy and maintains confidentiality of all company procedures, results and information about employees. Participates in the development, implementation, and promotion of compliance programs. Participates in and supports Quality Improvement Initiatives. Participates in the development, implementation, and promotion of policies and procedures and standards of care. Provides information about LIFE St. Joseph of the Pines Program to interested individuals and groups in adherence to PACE regulations. Follows all LIFE St. Joseph of the Pines Policies and Procedures and OSHA safety guidelines. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions. Performs other duties as required or requested. Leadership Functions 15% Assists medical/clinical staff to assure practice and physical areas of practice is in compliance with OSHA regulations and agency policies and procedures. Ensures that a safe and clean environment is maintained within the clinic setting for staff and program participants. Provides on-call medical coverage after hours and on weekends as scheduled by Medical Director. Performs initial and annual competencies on clinic staff as directed by Primary Care Physician.
IDT Functions 15% Functions as a member of the LIFE St. Joseph of the Pines program Interdisciplinary Team (IDT). Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for twenty-four (24) hour care delivery. Provides updates to the Interdisciplinary team of the medical management of participants. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant. In conjunction with the Interdisciplinary team plans and coordinates participant discharges from hospitals, skilled nursing facilities, etc. Staff Development Functions 10% Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications. Provides guidance and training to staff on a variety of relevant topics. Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Provides initial and ongoing training of clinic staff as directed by Primary Care Physician. Participant Care and Treatment Functions 15% Acts within the scope of his or her authority to practice Evaluates and treats participants during acute illness. Provides primary care responsibilities for medical management of participants as directed by Primary Care Physician. Performs in person comprehensive history and physical on new LIFE St. Joseph of the Pines participants. Performs in person semi-annual and annual assessments on LIFE St. Joseph of the Pines participants. Following consultation with the Primary Care Physician refers participants to medical specialist as indicated. Manages care of participants in the nursing home or assisted living settings by providing regular visits as dictated by nursing home standards and participant need as directed by Primary Care Physician. Provides information to caregivers about health concerns and conditions and other appropriate in-service education. Participant Rights Functions 5% Assists participants and caregivers in filing grievances. Protects privacy and maintains confidentiality of all results and information about participants and families. Monitors care to ensure that all participants are treated fairly and with dignity, kindness and respect. Ensures that all participant care is provided in privacy.
Ensures that all personnel are knowledgeable of and honor the participants responsibilities and rights. Assists in investigation of all allegations of participant abuse.
