3 Let the Healing Begin. At Rehabilitation Hospital of Indiana (RHI), we approach every patient with the understanding that every diagnosis, every illness, and every injury is different. It s the collective effort of specialty trained and compassionate team members who value the quality of life of every patient and their caregivers. It s the right kind of evidence-based medicine and treatment for inpatient, outpatient, and follow-up specialty rehab services that is provided by specially trained clinicians. It s one step closer to home. It s a continuum of care. It is RHI. Improving the health of patients and communities is the mission of Rehabilitation Hospital of Indiana. This is achieved in a number of ways by delivering innovation and excellence in clinical care, through research and in serving the community. 4 History and Values Rehabilitation Hospital of Indiana opened in 1992 is a community collaboration between Methodist Hospital of Indiana and St. Vincent Hospital and Health Care Center. Today RHI derives its strength from its co-ownership by the two largest notfor-profit healthcare systems in Indiana - St. Vincent Health and Indiana University Health. We believe that in rehabilitation, our medicine is our people. Our physicians, nurses, therapists and staff work in collaboration with our patients and their families to reclaim, rebuild and repair their quality of life. Our values stem from our heritage and reflect the principles of the United Methodist and Roman Catholic churches. These values define how we interact with each other, our patients, and our community. 5 Poor social and economic factors may contribute to the poor lifestyle choices that are prevalent in the community, such as substance abuse, poor diet, and lack of physical activity. The needs listed below specify the health issues identified through the assessment as priority needs across the entire community served by the hospital. These problems affect most of the community service area counties, but particularly apply to the primary service area of Marion County. Rehabilitation Hospital of Indiana seeks to work with and contribute to non-profit community organizations and health agencies committed to building innovative strategies to serve the needs in our service area. Our goal is to reach the largely underserved communities (minorities, low-income, homeless and at-risk populations) to help reduce the health disparities and improve quality of life for individuals living in our communities. We hope to increase the impact and make measurable advances toward positive health outcomes and to increase the quality of life for individuals living in the identified communities with the greatest need for Rehabilitation Hospital of Indiana s service area. RHI has identified three areas of focus: Access to Healthcare Obesity Prevention Behavioral Health Within those areas of focus, RHI is interested in supporting programs and initiatives that specifically aim to: Serve an identified community need, particularly in the areas of the RHI outreach priorities Reduce health disparities or improve quality of life for individuals living in our communities Reach largely underserved communities (minorities, low-income, homeless and atrisk populations) Rehabilitation Hospital Of Indiana 4141 Shore Drive, Indianapolis, IN P a g e
6 Benefit to the Community Rehabilitation Hospital of Indiana consults with community partners to identify and assess the greatest health needs in the communities we serve. In neighborhoods across Indiana, Rehabilitation Hospital of Indiana works as a driving force to improve community health and wellness by addressing these needs. Rehabilitation Hospital of Indiana follows federal guidelines for reporting community benefit and other community investments. Community benefits include charity care, unreimbursed costs of public programs, such as Medicaid and certain other services that provide treatment or promote health and healing in response to community needs. Financial Assistance and Unreimbursed Care Charitable giving plays an important role in outreach to the community for Rehabilitation Hospital of Indiana. Extending Our Healthcare Reach: High-Quality Care and Access for All Rehabilitation Hospital of Indiana is committed to providing high-quality care regardless of a patient s ability to pay. The RHI s financial assistance program is designed to insure that individuals and those who do not have health insurance or who are under-insured have access to the quality rehabilitative care they need. 7 In 2012, Rehabilitation Hospital of Indiana provided financial assistance and community benefits 2.78 million dollars. Total cost of financial assistance provided was $703,000 and benefitted 193 individuals. Quality Care and Coverage RHI is committed to charity, equality and justice in healthcare. Using federal poverty guidelines to determine eligibility, RHI offers a robust financial assistance program to ensure that all patients have access to quality, affordable care. Financial assistance, also called charity care, means providing free medical care or care at a reduced cost for people with low-incomes and those who are under-insured or do not have Rehabilitation Hospital Of Indiana 4141 Shore Drive, Indianapolis, IN P a g e
8 RHI Sports Program (RHISP) The Rehabilitation Hospital of Indiana Sports Program (RHISP) serves hundreds of people who participate in an array of recreational and competitive activities. Our sports programs are always evolving with new sports and activities each year. From beep baseball to wheelchair racing, quad rugby, and wheelchair basketball, whatever the physical disability, RHISP offers sports opportunities for individuals with traumatic illness or injury. RHI Sports Program Mission Rehabilitation Hospital of Indiana Sports Program s Mission is to provide competitive and non-competitive athletic opportunities to individuals with physical disabilities to enhance quality of life, promote physical fitness, introduce recreational and wheelchair sports and challenge participants to perform at a competitive level. RHI Sports Program specializes in serving youth and adults with spinal cord, orthopedic, neuromuscular, and visual impairments. Specially designed equipment is available for many of the activities. Hand cycles, racing chairs, tennis chairs, water skies and adaptive golf carts enable our participants to enjoy activities comfortably and safely. RHI Sports Program Community Fitness The RHI Sports Community Fitness program is free to individuals in the community and encourages physical fitness and mobility for individuals with disabilities. 9 In 2012, approximate 30 different individuals participated in the RHI Sports Program Community Fitness Gym. The Community Fitness Program is held each week on Tuesdays and Thursdays, and staffed by15 volunteers comprised of (RHISP staff and RHI therapists). In 2012, the RHI Sports Program provided nearly 15,000 hours of sports activities to close to 310 participants. The RHI Sports Program supports nearly 120 athletes who participate in a variety of sports programs from beep baseball to wheelchair racing, quad rugby, and wheelchair basketball, whatever the physical disability, RHISP offers sports opportunities for individuals in the community. Over the past decade quad rugby, wheelchair basketball, power soccer, water skiing, track and swimming, beep baseball, wheelchair tennis, wheelchair fencing, archery, hand cycling, and racing have all grown into staple programs of RHI Sports. In addition, families enjoy weekend activities like rowing, horseback riding, sailing, scuba diving, rock climbing, and water skiing. 10 Community Health Needs Access to high-quality care is just one factor that influences the healthcare is just one factor that influences the health of individuals and communities. Various social and environmental factors also plan an important role. To identify and commit to this year s community health outreach priorities, RHI analyzed community needs assessments from local and national organizations, Indiana University Health, including the county health departments, Indiana State Department of Health, and the Centers for Disease Control and Prevention. ACCESS TO HEALTH CARE Flu Shot Clinic Flu shots for the community in October and November free of charge to residents of the nine counties of greatest need that RHI services o Clinics will be held at the RHI-Main Hospital in Marion County. o Administered by RHI medical residents, nursing staff and phlebotomists Brain Injury Support Group Monthly Meetings Monthly meetings will be held at the RHI-Main Hospital in Marion County RHI therapy or nursing staff to conduct monthly meeting Light meal provided Stroke Support Group Monthly Meetings Monthly meetings will be held at the RHI-Main Hospital in Marion County RHI therapy or nursing staff to conduct monthly meeting Light meal provided Rehabilitation Hospital Of Indiana 4141 Shore Drive, Indianapolis, IN P a g e
11 PREVENTING AND REVERSING OBESITY Nutrition education programs Classes to be organized and held at RHI-Main Hospital in Marion County Weekly nutrition classes for RHI patients and their families Diabetes education program to inform and educate at-risk or newly-diagnosed patients take an active role in managing their diabetes so that they can enjoy life. Program to be held at RHI-Main Hospital in Marion County MENTAL HEALTH INITIATIVES Partner with IU Health Academic Centers to provide Mental Health Screenings for persons in the identified counties. Meet with Mental Health America of Greater Indianapolis to see how could partner with them on a mental health initiative for Marion County. Collaborate with Anne Gilbert, MD, Medical Director of Behavioral Health for IU Health and the IU Health Behavioral Health team on a community initiative.
