2 PROGRAM OVERVIEW Sno-Valley Adult Day Health is a small but vital part of the Sno-Valley Senior Center. Open Monday through Thursday, our Adult Day Program serves a maximum of 12 frail or elderly participants a day. The Social Worker/Program Coordinator will assist our long-time, competent staff to serve our participants and their families, and provide outreach to the community. This program is a part of the Sno-Valley Senior Center, a vibrant anchor in the rural Snoqualmie Valley, which served 2000 seniors last year. We invite you to explore being a part of this fantastic and fun team. The Sno-Valley Senior Center and Adult Day Health are a part of Senior Services, described below. ORGANIZATIONAL OVERVIEW Senior Services, with an annual budget of approximately $15 million, offers a comprehensive network of services to more than 54,000 older adults and their families throughout King County. Our programs and senior centers are passionate about promoting the emotional, social, and physical well-being of older adults. Since 1967 Senior Services has been a leader in connecting older adults with high quality services that support them to live active lives while enabling them to make their own choices to suit their lifestyles and preferences. Our services: Inform, protect, support and advocate Senior Information & Assistance: help and advice from trained advocates who have access to more than 4,000 resources in our databank Senior Rights Assistance: help and advice from highly trained volunteers on issues covering consumer protection, debt and bankruptcy, homeowner, estate planning, substitute decision making, social security, and landlord tenant issues as well as advocacy and support for older victims of crime SHIBA HelpLine: free health care coverage counseling for people of ages and all backgrounds to help save money on health care, make sense of confusing information, and make better choices Caregiver Outreach and Support: practical resources, caregiver support and counseling, caregiver training, and help navigating the maze of potential services Kinship Care: resources, support and advocacy for people raising a relative s child(ren) Senior Outreach: assistance sorting out complex issues and reducing barriers to accessing health and social services, referrals to critical services, encouragement and connection Promote health and strength EnhanceFitness: balance, strength, low-impact aerobics, and flexibility exercises specifically designed for older adults at their fitness level, in one-hour classes, three times a week EnhanceWellness: education, motivation, and encouragement for older adults to take on health challenges and maintain or regain control of their lives Enable seniors to remain independent and safe at home Meals on Wheels and Mobile Market: nutritious frozen meals and home delivered groceries to homebound seniors Minor Home Repair: basic plumbing, carpentry, electrical and other repairs and modifications such as installation of grab bars, handrails and ramps Transportation: to medical appointments, community dining sites, senior center activities, grocery shopping, and errands Page 2 of 5
3 Promote relationships and meaningful engagement in the community Community Dining: 12 community dining sites, ethnic dietician, nutrition education, supportive services Community Engagement: opportunities for more than 4,600 volunteers to stay connected after retiring from fulltime work and to find meaningful roles with Senior Services or other organizations Senior Centers: take classes, keep fit, meet new, interesting people and visit with old friends, or share a meal at one of our 7 centers Senior Services approximately 250 caring and experienced employees are led by a dynamic, highly-effective visionary who has been a leader in the aging field for over 30 years. We also have a truly dedicated, supportive board and a whole network of community partners. We have been a major player in the aging field for more than 40 years and are looking forward to inspiring the newest cohort, the Baby Boomers. JOB SUMMARY Independently manage the day-to-day operations of the Adult Day Health program and provide social work services to maintain and, if possible, improve the physical, mental, and emotional well being of frail and disabled adults. KEY RESPONSIBILITIES: 1. Perform one on one as well as public outreach activities to ensure program remains near or at capacity. 2. Perform intake registration for new participants to determine eligibility and appropriateness for the program. 3. Conduct initial assessment, initiate ongoing reviews and refer as necessary to other services the participant or caregiver might need. Assess participants for mental illness, dementia, alcohol/substance abuse, physical abuse and/or neglect and make appropriate referrals for follow-up, treatment, or counseling. Assist nurse in developing care plan. 4. Provide discharge planning and assist in transition. 5. Assist Director in monitoring program budget. Keep necessary records to assist with billing. 6. Provide information to participants, caregivers, staff and the community regarding community resources/services. Coordinate with Information and Assistance and case management as needed. Provide information and referral for persons not appropriate for adult day care. 7. Provide individual and group counseling for participants and families as necessary. 8. Provide assistance with feeding, mobility, and toileting and occasionally subbing for activities specialist as necessary. 9. Demonstrate an eagerness to learn and enhance skills that promote cultural competence. Page 3 of 5
4 10. Participate in events related to understanding institutional racism and building cultural competence. 11. Recruit, orient, supervise, and coordinate training opportunities for staff and volunteers. 12. Other duties as necessary and/or assigned. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 1. Bachelor s degree in social work or counseling, or masters in social work, gerontology, or other related field. 2. Two years experience in a human service field, preferably working with frail or disabled adults and their families. 3. Physical ability to assist disabled participants with transferring and ambulating; must be able to lift at least 50 pounds. 4. Willingness to obtain a yearly TB Test, Statement of Good Health, Food Handlers Permit and CPR Certification. 5. Ability to work cooperatively with other agency staff. 6. Computer experience necessary. Salary and Benefits $ $19.88 per hour. This position is PT 18+ hrs/wk with some flexibility. Benefits are negotiable.
