Please refer to our website to send family condolences, view obituaries, order flowers or for additional information regarding our facilities, cemetery and advanced planning. Prior to drafting any contract for goods or services, the responsible party or the decedent's survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. We are the most preferred family-owned funeral and cremation provider in the Inland Empire.

From traditional funerals and cremation options to small gatherings and elaborate receptions, we can accommodate virtually any type of event. Our caring staff will help you create a truly meaningful tribute and add special touches to reflect your loved one s special life. We also offer an array of high quality merchandise, including caskets, urns, keepsakes and more. Let us handle all of these details so you can focus on what matters most right now: you and your loved one. The goods and services shown below are those we can provide to our customers. You may choose only those items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any item you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Professional Services A. Basic Services of Funeral Director and Staff... $1,205 This charge includes consultation with the family or responsible party; staff and facilities to respond to initial requests for service; coordinating service plans with cemetery, crematory and/or other parties involved in the final disposition of the deceased; securing official documents and necessary registrations and sheltering of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) Cherub Burial Discount (Children 0-12 years of age) 0-12 years of age 40% off professional services Cherub Cremation Discount (Children 0-12 years of age) 0-3 years of age 40% off professional services, 4-12 years of age 20% off professional services B. Embalming...$575 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement which does not require you to pay for it, such as a direct cremation or immediate burial. Additional fee for time and materials when an autopsy has been performed @ $200 Additional fee for time and materials when restorative art is needed @ $75 per hour C. Transportation All transportation charges contained within this list apply to an area with a radius of 30 miles to our nearest facility. If we exceed this service area, in addition to the basic vehicle charge that covers 30 of the total miles traveled from our facility, we will make an additional charge of $4.25 per mile for a staffed service vehicle one way. If an overnight stay is required due to the round-trip exceeding 400 miles, or due to no fault of our own, our driver and vehicle cannot safely return the same day, there will be an additional charge of $250 for overtime, hotel, and meal expenses, in addition to the basic mileage charges. This package reflects a $2005 discount when casket is purchased through Miller-Jones. A complete casket price list will be provided. Graveside Burial Package with viewing... $2,595 This service includes basic services of funeral director and staff, transfer of deceased, embalming, dressing, casketing, cosmetizing, utility vehicle, funeral hearse, use of equipment and staff for viewing (1-4 hours) and graveside service (within 30 mile radius). This package reflects a $1,645 discount when casket is purchased through Miller-Jones. A complete casket price list will be provided. Graveside Burial Package without viewing /embalming....$2,295 This represents a $1,470 discount. Receiving A Deceased Person From Another Mortuary Minimum Delivery to Cemetery... $1,495 This discounted package includes basic services of funeral director and staff (discounted to ref lect already-secured death certificate/disposition permit and reduced involvement of other personnel), transfer to mortuary from airport within a 30 mile radius and the use of funeral hearse or van to a local cemetery. This package presumes no ceremony and no other use of the funeral home and that we may deliver to the cemetery at the time of our choosing. Add to the fee of $1,590 any additional services selected, such as graveside ceremony, other preparation - dressing/casketing, viewing/visitation, and funeral service at chapel or church. This package reflects a $865 discount Forwarding A Deceased Person To Another Mortuary Minimum Transfer....$1,895 This package includes basic services of funeral director and staff, transfer to mortuary from place of death, embalming, use of funeral hearse or van to local point of embarkation (airline or other shipping charges not included), and utility vehicle. Other items that may apply and could be added to the billing are a combination shipping container ($275) or air tray ($195). Minimum Transfer with 1-4 hour viewing at Funeral Home...$2,125 Immediate Burial... $1,695 - $14,170 This package includes basic services of funeral director and staff, transfer to mortuary from place of death, other preparation of the deceased when embalming is declined and utility vehicle to transport to cemetery within 30 mile radius. This service is preferred by members of Jewish, especially Orthodox and Conservative, and Muslim faiths. We offer separate, specially configured facilities for ritual washing as dictated by the religious faiths we serve. Our staff is also fully knowledgeable in serving these faiths. Disinterment Removing the deceased from a grave or crypt... $1,935 Includes basic services of funeral director and staff, transfer to mortuary from place of disinterment, and utility vehicle to transport to airport within 30 mile radius. *If required, placement of the deceased into a new casket the cost of a new casket will be added. *This does not include the 3rd party disinterment fee. 5

All About Cremation Your deceased family member never leaves our care We are among the very few mortuaries to operate our own crematory inside our clean and secure facility. It is important to understand that because of this we are in control of your loved ones care from your initial contact with us until the cremated remains are returned to you or placed in their final resting place. We cremate only one person at a time It takes more effort to ensure that the cremation is handled with care, dignity and high standards. To assure that your loved one is being cared for appropriately, we adhere to rigid standards in every cremation process. Each of our crematory staff is highly trained and certified. They are dedicated to providing dignified cremation services for your loved one and adhere to the cremation Code of Ethics, and all applicable state laws as an assurance to you of our commitment to dignity and honesty. Your personal inspection is encouraged We are the region s first crematory to encourage your personal inspection. We are also the first to offer a special viewing room located next to our crematory to facilitate those who wish to be present for the start of the cremation process. Understanding the Cremation Process Cremation is the process of reducing the body of the deceased to fragments of bone by the way of heat and flame. We prefer to say cremated body or cremated remains rather than ashes because we believe that the term ashes can be misleading. Our funeral directors or crematory staff can explain the consistency of cremated remains in as much detail as you prefer. If you are considering scattering, we would recommend you ask us to prepare you for what you will see. If the deceased is Jewish, Christian Orthodox, Muslim, or Roman Catholic, you may wish to consult clergy or our staff for guidance. For example, the Catholic Church strongly advocates the presence of the whole body not the cremated body for the Funeral Mass with cremation to take place later. The Catholic Church does not accept scattering. Our cremation authorization form goes into some detail in explaining the cremation process, our policies and procedures. You will receive a copy of the completed form. Cremation Service Options It will be necessary to select or provide a container for any viewing and/or the cremation process as well as a container for the cremated remains. We offer a wide variety of rental and combustible containers for cremation. We also offer a wide variety of distinctive and valuepriced urns. We can package, transport, and mail the cremated remains by U.S. Priority Express Mail only (other carriers will not accept cremated remains) anywhere is the United States for $175 (add cost of urn) To be safe allow at least 1-3 business days for Priority Express Mail in the continental U.S. Because we schedule our crematory days in advance, and due in part to the time involved in meeting the legal requirements of the death certificate and disposition permit, we ask for 5-7 days following death to complete the cremation process. Let us know if this creates difficulty - we ll try our best to accommodate you. 6

Traditional Cremation... $3,990 Includes: Basic services of funeral director and staff, transfer to mortuary from place of death, embalming, other preparation, dressing, and casketing, viewing 1-4 hrs, funeral hearse, funeral ceremony, utility vehicle for death certificates and permits, ceremonial oak casket and alternative cremation container and the crematory fee. Add urn. If selecting a casket other than listed above, deduct $1,035. Cremation with Memorial or Graveside Service... $1,695 Includes: Basic services of funeral director and staff, transfer to mortuary, memorial ceremony or graveside ceremony, the crematory fee, utility vehicle for death certificates and permits, and enhanced identification procedure. Add a combustible cremation container, and the urn selected. Cremation with 45 min. Viewing at our Mortuary... $2,195 Includes: Basic services of funeral director and staff, transfer to mortuary, utility vehicle for death certificate and permits, crematory fee, embalming and a 45 minute viewing in our slumber room, Monday through Friday from 8:00am until 4:00pm. You will need to select an alternative container for the viewing. Alternative containers encase the body and can be made of materials like fiberboard or composition materials. Direct Cremation... from $955 - $13,430 Includes: Basic services of funeral director and staff, transfer to mortuary, utility vehicle for death certificate and permits, crematory fee, and enhanced identification procedure. If you want to arrange a direct cremation, you will need to select an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials. The minimum containers we provide are unlined, leak resistant, and made of heavy cardboard.
