How to use this Document There are two ways to search for services using this document. 1. If you know the name of the service provider you are looking for, you can search the alphabetical list of providers in the index at the end of the document (page 43). Example: You need to access home health care services and would like to start your search with Preferred Home Health Care to find out more about the various services they offer. You can search the Appendix at the end of the document where service providers are listed alphabetically. This will tell you on what page(s) you can find information about the many services offered by Preferred Home Health Care. 2. If you don t know the name of a service provider, but you know the type of service you are looking for, you can look at the table of contents at the beginning of the document (page 1). Example: You need to access resources for a teenager and don t know the types of resources are available. You can look in the table of contents to find out what page numbers the resource list for Teens in Need begins. Starting on that page you will find a listing of various agencies providing resources for teens in need. Note: Some providers may be listed under several types of services. Therefore, the agency index lists all page numbers in which specific agencies can be found. 2
