2 Years Peace River Center is a Private, Not-For-Profit Community Mental Health Organization serving the residents of Polk, Highlands and Hardee counties. The Mission of Peace River Center is to improve the mental health of individuals in our community to achieve our Vision of a higher quality of life through treatment, education and advocacy.
3 Welcome to Peace River Center! Peace River Center (PRC) is a private, not-forprofit, community mental health organization that has been serving the mental and behavioral health needs of Polk, Highlands and Hardee Counties for over 60 years. PRC has a long history of serving clients and equipping them to live to their fullest potential. PRC was originally established as a children s guidance center in 1948 and has since grown to an organization that provides a variety of services and programs designed to assist clients in improving their quality of life. Services offered by PRC include 24-hour crisis intervention, outpatient counseling in individual or group settings, 24-hour residential programs, case management for children and adults, domestic violence shelters and a Rape Recover Resource Center. We work with many third party providers and in some cases, a sliding scale is used to determine fees. Crisis Response Team, Domestic Violence, Rape Recovery Resource Center services are offered free of charge. Our funding partners include: Department of Children and Families, United Way of Central Florida, Polk county Board of County Commissioners (BOCC), Hardee County BOCC, Highlands County BOCC, VOCA (Victims of Crime Act), individuals, businesses, private foundations and community organizations. This directory is an overview of the programs and services offered by PRC. If you have questions regarding services offered at PRC, please call the number listed for that specific program or the Administration Office at (863) If you are in crisis and in immediate need of services, please call our 24 hour Crisis Line at (863) or toll free at (800) CRISIS SERVICES Crisis Response Team On Site Service Address: 1255 Golfview Avenue, Bartow 24-hour crisis line: (863) Toll-free: (800) Mobile Service Population: Polk and Hardee County residents On-Site Description: This mobile outreach service is designed to provide immediate crisis assessment, crisis intervention and counseling, linkage and referral, crisis follow-up contact to promote resolution for individuals (adults and children) experiencing any type of emotional crisis which can include: suicidal intentions, self destructive behaviors, or acute, severe, and intense emotional distress. Crisis Stabilization Unit Physical Address: 1255 Golfview Avenue, Bartow 24-hour crisis line: (863) Business line: (863) Description: A locked 30 bed public receiving facility for Florida s Baker Act. The Crisis Stabilization Unit provides psychiatric stabilization services including group therapy, medication management, and education, discharge planning and referrals, and family education. Gilmore Avenue, Lakeland Population: Western Polk and Hardee County residents Description: Assesses people with mental illnesses for service needs and links them to the needed community resources such as social security, Medicaid/ Medicare & food stamps. Assists people in planning short and long- term goals for recovery while monitoring progress on reaching the recovery goals. Also, case management serves as an advocate in people s recovery. Gilmore Ave., Lakeland (863) Population: Western Polk and Hardee County residents Description: The Florida Assertive Community Treatment Team is a comprehensive and intensive multidisciplinary service model designed to serve 100 adults with severe and persistent mental illness to help them improve their quality of life and avoid long-term hospitalization. Residential Program Facilities: Stepping Stones (Group Home I) (863) Success House (Group Home II) (863) * Out-of-area referrals accepted based on bed availability. Description: Assists severely and persistently mentally ill residents in the development and refining of daily living skills so thy may attain the goal of living in the least restrictive environment possible. Treatment plans are developed which pair the resident s strengths with their personal goals. Residents may also receive medication management, case management, vocational, forensic, and psychosocial rehabilitation services. HUD Subsidized Housing Program-Peace River Apartments (863) x2157 Office Hours: 8:00am 5:00pm Mon-Fri Population: Polk, Hardee and Highlands County residents Description: Provide income based, permanent housing opportunities to individuals with mental disablilites. Orange St., Wauchula (863) Client Participation Hours: 9:00am - 2:00pm Mon-Fri Description: Provides opportunities to develop essential communication skills, coping skills, daily living skills and a deeper understanding of emotional well-being using two tracks: Track 1 - Independent Living/Community Tenure helps clients achieve independent living in the least restrictive environment possible and avoid hospitalization. Track 2 - Vocational Track helps clients develop workplace and employment skills. Vocational and Supported Employment Services Physical Address: 715 N. Lake Avenue, Lakeland 1835 N. Wabash Ave. Lakeland (863) Hours of Operation: 9:00am - 5:00pm Mon- Fri Description: A rehabilitation club for people with mental illness where real-life job skills are taught and used to achieve meaningful work as a contributing member of society. Club members learn responsibility, develop self-esteem and learn to interact professionally. Provides transitional employment with local community partners, guidance to secure housing and/or advance their education and promotes on-going psychiatric care, while maintaining government benefits. Competency Restoration Services operate in cooperation with the justice system to avoid state hospital admissions. Adult Overlay Services (863) ext Population: Designated Western Polk and Hardee County assisted living facility and nursing home residents. Description: Provides individual and group therapy, family counseling, and case management to residents and their families. Increases the knowledge and skills of caregivers. CHILDREN & ADOLESCENT SPECIAL SERVICES Resiliency Team (863) ext Population: Western Polk and Hardee County residents. Description: Provides a needs assessment, develops a service plan, links and refers to community resources to meet identified needs; monitors progress for the treatment team and advocates on behalf of the client and family/foster family to keep emotionally disturbed children in the home. Children s Advocacy Center Physical Address: 1260 Golfview Avenue, Bartow (863) Population: Polk and Hardee County residents. Description: Provides brief crisis assessments and intervention and referrals. The Children s Advocacy Center is a multi-agency collaborative community effort to provide services to abused children. Family Builders Program (863) x1156 Physical Address: 1239 East Main Street, Bartow, FL Office Hours: Flexible hours, 7 days a week Description: An in-home crisis intervention/stabilization program for high risk families. Designed to provide the family with the necessary skills and case management services to keep the child in the home. Assists in reunifying families with children in out-of-home placements. Teaches various skills: problem solving, behavioral modification, parenting, conceptual change, communication. 6 7

Description: Each child placed by the State of Florida in shelter care is required to receive an in-depth and detailed assessment of their emotional, social, behavioral and developmental functioning within the family home, school and community. This assessment and resulting recommendations are used to develop the child and family s case plan. Services involve the child, the family and appropriate community members such as preschool providers. Sebring Domestic Violence Outreach Service Outreach services by appointment (863) Population: Polk, Hardee and Highlands County residents primarily but not exclusively. Description: Provides a free and confidential assessment, shelter and related services to domestic violence survivors and their dependents including crisis intervention, problem solving skills, selfhelp groups, safety planning, information and referrals, and outreach services including external support group.
