Contact AIS for information on In Home Supportive services, elder abuse, cool zones and more. * Access to Independence Center This is the primary resource for San Diegans with disabilities. They provide: information and referrals, support networks, assistive technology, housing resources and more. Southern Caregiver Resource Center SCRC can help you with information, support, counseling, respite care and finding the best options for you and your loved ones free or charge. Alzheimer s Association hour Helpline providing information and support and finding appropriate community resources. Provide a home setting, and counseling. Assists with housing, income, etc. Domestic Violence Hotline Health Insurance Resources The Consumer Center For questions related to Medicare, Medical, Medicare Part D and more. Also, please call if you need help accessing health services or applying for benefit programs. HICAP Provides assistance and information regarding Medicare health insurance program. Medicare 800-MEDICARE/ For questions related to Medicare. Medi-Cal For questions related to this health coverage program covering low income individuals (California s Medicaid). Legal Help * Elder law and Advocacy Free legal services for individuals 60 and older. Call to schedule an appointment. University of San Diego School of Law A free legal clinic handling Landlord Tenant Disputes, SSI Appeals, Consumer Issues, Section 8 Appeals, and more. Call to schedule an appointment. 5

6 Legal Aid Society of San Diego Free legal help for low income residents of San Diego County. Lawyer Referral & Information Service Provides a free consultation and makes appropriate referrals to local lawyers. A service of the San Diego County Bar Association. Tenants Legal Center Provides guidance to tenants regarding legal/housing issues. There is also a 24 hour information line available at Utility Assistance Programs CARE Energy Bill Discount Program % discount on your SDG&E bill, for low income individuals. California Telephone Access Program Free adaptive phones for the hearing and vision impaired. Alert San Diego Medical alert system Health Services In Home Support Services IHSS provides homemaker and personal care assistance to low income individuals who are in need of assistance with personal care. Heritage Mental Health Care Management Mental health care management and counseling services for seniors. Family Health Centers of San Diego Accepts a range of insurance plans and fee s are determined on a sliding scale basis. San Diego Hospice & Palliative Care For end of life care and support and bereavement services. 6

Minor Home Repairs SDG&E Energy Team Minor home repairs and weatherization projects at no cost. Interfaith x 225 In North County only. Minor home repair and smoke detectors free of charge. Jewish Family Services May assist with minor home repairs. Must be 60+ and live within service area. Financial Services Social Security Call for Social Security questions or to locate a local office near you. Revisions Resources A local non-profit agencies that educated consumers about reverse mortgages. SSI Advocacy Program x 1 For questions and assistance with Supplemental Security Income. Senior Leisure Senior Citizen Services- City of SD Call to learn about local activities, field trips, dances, and more. Friendship Line for the Elderly Offers emotional support 7 days/week from 8am to 8pm. 7

8 * Senior Community Centers Gary and Mary West Senior Wellness Center downtown provides opportunities for socializing and participating in activities. Other services include congregate meals, physical, mental health and social services. Senior Companions Program This program offers companionship to seniors from seniors. Senior companions are given a tax free stipend and travel reimbursement. Retired and Senior Volunteer Program RSVP facilitates matches between local agencies and volunteers. Many placements available. Volunteers opportunities for seniors 55+. OASIS Lifelong Learning OASIS offers a very wide variety of classes and trips. Call to have a catalog of classes sent to you. Senior Community Centers Offers a wellness center, home delivered meals, and information & referral. Senior Employment Community Options Support for people with developmental disabilities and help seniors with employment. Must be a low income senior to participate. Senior Companion Program Help provide other seniors with companionship and transportation. Receive travel reimbursement and an hourly stipend. 8

9 Transportation FACT FACT provides a comprehensive list of transportation services (private and through social service agencies). This information can be accessed through their website at In-Home Help * Keepsake Companions Provides care givers to assist with light housekeeping, shopping & errands, meal preparation and personal care needs. * Senior Helpers Provides assistance with personal care needs as well as shopping, errands, and housekeeping. Live-in care givers also available. Member of ElderHelp s Proferred Provider Network. * Comfort Keepers In-home companionship and care services to assist with meal preparation, housekeeping, laundry and personal care needs. Also provides respite care for caregivers. * Mission Home Health Works with you and your physician to develop a plan of care to meet specific needs. Medicare certified. Will accept Medi-Cal and private insurances. 9

Electricians * JW Electric Specialize in electrical wiring of buildings, stationary machines and related equipment. Handles the installation of new electrical components or the maintenance and repair of existing electrical infrastructure. General Contractors * Grondin Consturction Certified Aging-in-Place specialists who provide home modifications including grab bars, roll-in showers, easy grip controls, stair lifts, ramps, handrails and hall widening. Rehabilitation * Challenge Center Offers a wide array of services focused on improving health, function and quality of life. Services include adapted fitness training, aquatics fitness (in warm water), circuit training, physical therapy and more. 11

12 Home Delivered Meal * Meals on Wheels Offers choice of one or two nutritious meals provided 7 days a week (including holidays). * Susan s Healthy Gourmet Provides fresh, nutritionally balanced, and calorie-controlled meals. Vegetarian options offered. Meals for breakfast, lunch and dinner are available. Relocation * Senior Move Masters Works with seniors and their families with planning and packing, moving and resettling senior into new residence. 12
