Philanthropy Supports Empowered Patients Care Coordinator Pilot program at Hoag Health Center Irvine Patients frequently walk out of a doctor visit with referrals for further testing, treatments or consultations with specialists. Making and keeping these appointments are key to ensuring each patient receives the proper evaluations and care in a timely manner. Checking to see if these services are covered and considered in- or out-of-network by their insurance providers to anticipate any out-of-pocket expenses adds another layer of complexity. Thanks to the generosity of our donors, Hoag relieved the burden from patients by hiring Care Coordinators at Hoag Health Center Irvine. They assist patients and their families by accessing all of Hoag s services and support programs by: Scheduling tests and treatments Coordinating multiple appointments among several different departments, programs and specialties Assisting with the pre-authorization process and acting as a liaison between the pre-authorization team, physician s office and patient Providing patients with itineraries for their care Courtesy Shuttle Care Coordinator assisting a patient at Hoag Health Center Irvine For quality health care to truly be accessible, it must be convenient.
