2 Joint Replacement at the Center for Bone and Joint Health Pain in the bones and joints can interfere with almost all aspects of life. Mobility is critical to enjoying family, staying active and performing day-to-day tasks. The Center for Bone and Joint Health is committed to all aspects of bone and joint care with the goal of restoring quality of life as quickly as possible. You and your doctor may decide that joint replacement surgery is necessary after non-surgical options have been exhausted for chronic conditions of the bones and joints. At Hunterdon Medical Center our team of physicians, dietitians, nurses and physical therapists is dedicated to all aspects of your care before, during and after surgery. There is no longer a need to travel long distances for specialized orthopedic care. All of your options are now available right next door at Hunterdon Medical Center. Our Wellness Approach At the Center for Bone and Joint Health, we offer a unique wellness approach. Our focus is on getting you back to a normal quality of life weeks sooner than with traditional approaches. Each of our specialists is committed to building a relationship with you and offering an individualized treatment plan. They have made advances in joint replacement and use minimally invasive techniques whenever possible. For you that means smaller incisions, less cutting of muscles and tendons, less post-operative pain, faster recovery and improved strength. We also know that even though you will be spending time in the hospital for joint replacement surgery, you are probably not sick and don t want to be treated that way. In the joint replacement unit, you will be with healthy patients, like you, who are eager to get back to normal activity as quickly as possible. We encourage you to trade in your hospital gown for your own clothes shortly after surgery. In most cases, you will begin rehabilitation the day of your surgery and be leaving the hospital within a few days. Once the operation was over, I was back in the room and up and walking in about two hours. The therapists were great. They challenged me in an adventurous way. KNEE REPLACEMENT PATIENT, WASHINGTON, NJ Your motivation and participation in the physical therapy program are important to the speed and success of your long-term rehabilitation. This means you are the greatest influence to a successful recovery. But you will not be alone. We encourage you to select someone to be your coach throughout the joint replacement process. Your coach will support you as you prepare for and recover from surgery, keep you motivated, assist you with your exercises and remind you of any post-operative precautions. An Informed Patient is a Successful Patient As you prepare for surgery, we offer a Patient Education Class and a Patient Guide to all joint replacement candidates and their coaches. The class and guide enable you to understand the surgical process and what you should expect before and after surgery and when you return home. You will learn exercises that you can perform prior to surgery to prepare you for rehabilitation. You will learn how to have an active role in managing your pain and recovery and minimizing side effects. 1 2
3 At the Patient Education Class we will review the following topics: Preparing for surgery Pain Management What to bring to the hospital Complication prevention How to control risk factors Discharge options What is joint replacement surgery? Rehabilitation Will joint replacement be painful? Some pain is expected with joint replacement surgery but we have many ways to minimize it. You will be a partner with the medical staff in ensuring your comfort. Starting your rehabilitation exercises as soon as possible is the single most important factor in having an optimal outcome with the lowest risk of complications. With the use of effective pain management, physical therapy may begin as early as the day of surgery. During your stay in the Joint Replacement Unit you will have the option to participate in alternative therapies such as aromatherapy and Reiki therapy. These services are offered to relieve anxiety, assist with pain control and support healing. "Ten years ago I chose a big hospital to have my first knee replacement. My pain was never addressed and my care, in general, was poor. For my second knee, I decided on Hunterdon Medical Center. My surgery and care were excellent. Pain control was far superior. It is a pleasure to be able to walk around our farm without pain!" KNEE REPLACEMENT PATIENT, MILFORD, NJ Comfortable Accommodations After your surgery you will stay in the Joint Replacement Unit, which is an area of the hospital dedicated to your recovery and physical rehabilitation. It is designed specifically with wellness in mind. In the unit you and your coach will be comfortable and ready to get started on the road to recovery. Your room will be furnished with everything you need to facilitate movement, and your physical therapy equipment will be only a few steps away. Our Care Follows You Throughout Your Rehabilitation Upon discharge from the hospital, we offer physical therapy and nursing services in your home or outpatient physical therapy at one of the Hunterdon Health and Wellness Centers or physical therapy facilities. In some cases, a short stay in a local rehabilitation facility is necessary. We will be there to make sure your discharge needs are met and our staff will make all arrangements. 3 4
4 During my recent post-operative stay at 5 North, I not only received the utmost professional care from the nurses and staff, but also friendly, personal attention, which made the recovery period so much easier. HIP REPLACEMENT PATIENT, FRENCHTOWN, NJ Your doctor may authorize skilled nursing care or physical therapy in your home. A home health professional will conduct a safety assessment and offer recommendations for modifications. Physical therapists and home care nurses will be available until you are able to transition to outpatient physical therapy at one of the Hunterdon Health and Wellness Centers or Outpatient Physical Therapy Facilities. As you recover and the skilled services of a nurse or therapist are no longer needed, you may still need additional home care. If needed, we will assist you in arranging private pay services through Visiting Health and Supportive Services, which may include additional service from personal care providers, such as certified Home Health Aides, NeighborCare or the Statewide Respite Care Program or private pay RN services through Care Connections. Most patients go home after discharge from the hospital but there are three paths you can take: 1. Go home, have home health care, then move on to outpatient physical therapy. 2. Go home and start outpatient physical therapy right away. 3. Go to a rehabilitation facility for a short in-patient stay, then return home and start home health care or outpatient physical therapy. Your Physician, the Joint Care Coordinator, Physical Therapists, Nurses and Social Worker will help you, your coach, and your family decide what is best for you. 5 6
